Job Description: The Licensed Practical Nurse will provide nursing support for clinic operations to ensure patients receive safe, quality patient care throughout their treatment and disease process. ESSENTIAL FUNCTIONS: * Reviews patients' medical records to ensure that all required data has been entered or is available prior to patient's scheduled appointment. * Generates, reviews and distributes daily assigned clinical reports to appropriate personnel. * Obtains patient vital signs, weight, height, pain score and or other data collection and documents in Electronic Medical Record (EMR) * Manages /performs "rooming" process for provider clinics including escorts patients to exam rooms * Reviews with patient health history, allergies, and current medications and records in the EMR * Completes required/ assigned screening questionnaires and records information in the EMR * Assists with provider performed procedures /exams /acts as a chaperone (i.e. bone marrow biopsies, skin biopsies, etc.) * Stocks exam and treatment rooms with necessary supplies. * Prepares and maintains supplies and equipment for treatments. * Verifies accuracy of/collaborates with Front Office staff related to scheduling of lab, provider, treatment and injection appointments. * Obtains pre-authorizations for oral medications. * Manages prescription / refill extension requests per Tennessee Oncology (TO) policy, procedure, standards * Manages IV therapy access devices per TO policy, procedure, standards, including accessing and deaccessing implanted ports, after competency validation, under the supervision of a RN. * Uses the drug storage cabinet to enter and remove appropriate dose of medications ordered. * Completes required daily/weekly/monthly drug cabinet activities as assigned, per TO policy, procedure, standards * Prepares and administers medications as directed by the provider and in accordance with TO and nursing policy, procedures, and standards. * Monitors PT/INR results and works with provider to manage anti-coagulant /Coumadin therapy * Completes patient supplemental paperwork for insurance, leave and/or disability, etc. * Performs telephone triage under the supervision of a Registered Nurse (RN) and or a Physician/ NP/ PA (provider). * Assists with patient education and informed consent process per TO policy * Schedules patient psychological services and or arranges community agency /support services referrals, under the direction of the provider and or RN. * Enters orders accurately into the EMR system for labs and tests per verbal/written order from provider * Manages provider clinic productivity by identifying and resolving problems that impede patient flow * Monitors, observes, records, and reports patient's condition and or changes to patient condition, reaction to medications and treatments to the RN and or provider. Assists with or performs nursing evaluations under the supervision of a provider or RN. Works in the infusion rooms under the supervision of an RN. * Documents services performed and medications administered accurately and in a timely manner * Enters charges for services performed accurately and on time * Completes assigned evaluation tools within designated timeframe. * Informs clinic managers of clinic needs and concerns relating to the work environment. * Participates in quality improvement initiatives. * Participates on committees as requested * Attends all mandatory in services, training, meetings. Completes required training, continuing education and competency validations on time * Performs other additional duties as assigned KNOWLEDGE, SKILLS & ABILITIES: * Knowledge of medical terminology and anatomy * Familiarity with heath care/oncology regulations, policies and functions * Knowledge of equipment, supplies, and materials needed for medical treatment * Understanding of basic laboratory procedures including preparation and screening. * Knowledge of infectious disease management and control and safety standards. * Skills in reading and following written and verbal medical orders. * Skill in handling a number of tasks simultaneously. * Ability to establish and maintain effective working relationships with coworkers and patients population. * Communicates positively and constructively at all times * Demonstrates a respectful and professional manner while maintaining confidentiality * Ability to empathize with patients and families and deal with grief and difficult patient interactions. * Ability to collaborate with health care team to provide care for each patient. * Ability to cope with death and dying. EDUCATION & EXPERIENCE: * Current state LPN License. * Oncology experience in an outpatient facility preferred * Minimum of one year LPN experience required. May be waived with the approval of the EDONS * ONS Oncology Basics (introduction) chemotherapy course required within 6 months of hire then every two years * Current TO approved BLS provider card. Cooperate with other resident services when coordinating nursing services be certain that the resident's total regimen of care is maintained. Verify that all nursing service personnel are in compliance with their respective job descriptions. Participate in the development, maintenance, and implementation of the facility's quality assurance program for the nursing department. Participate in surveys (inspections) made by authorized government agencies. Periodically review the resident's written discharge plan. Participate in the updating of the resident's written discharge plan, as required. Assist in planning the nursing portion of the resident's discharge plan, as necessary. Admit, transfer, and discharge residents, as required. Complete accident/incident reports, as necessary. Charting and Documentation Complete and file required forms/charts upon resident's admission, transfer, and/or discharge. Encourage attending physicians to review treatment plans, record and sign their orders, progress notes, etc., in accordance with established policies. Receive telephone orders from physicians and record on the Physicians' Order Form. Transcribe physician's orders to resident charts, cardex, medication cards, and treatment/care plans, as required. Chart nurses' notes in an informative and descriptive manner that reflects the care provided to the resident, as well as the resident's response to care. Record new/changed diet orders. Forward information to the Dietary Department. Report all discrepancies noted concerning physician's orders, diet change, charting error, etc., to the Nurse Supervisor. Fill out and complete transfer forms in accordance with established procedures. Enter appropriate documentation concerning unauthorized discharges in the resident's medical record in accordance with established procedures. Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures. Sign and date all entries made in the resident's medical record. Write nurses' notes to reflect that the care plan is being followed when administering nursing care or treatment. Medication Administrative Functions Prepare and administer medication as ordered by the physician. Responsible for IV medication/fluid administration. Verify the identity of the resident before administering the medication/treatment. Verify that prescribed medication for one resident is not administered to another. Verify that adequate supply of floor stock medications, supplies, and equipment is on hand to meet the nursing needs of the residents. Report needs to the Nurse Supervisor. Order prescribed medications, supplies, and equipment as necessary, and in accordance with established procedures. Verify that narcotic records are accurate for your shift. Notify the Nurse Supervisor of all drug and narcotic discrepancies noted on your shift. Review medication cards for completeness of information, accuracy in the transcription of the physician's order and adherence to stop order policies. Notify the attending physician of automatic stop orders prior to the last dosage being administered. Immediately notify the Nurse Supervisor and attending physician of any medication administration errors. Nursing Care Functions Perform physical assessment through observation, inspection, palpitation, percussion and auscultation. Deliver and maintain optimum resident care and comfort by demonstrating knowledge and skills of current nursing practices. Promptly respond to call lights and other resident needs. Make rounds with physicians, as necessary. Consult with the resident's physician in providing the resident's care, treatment, rehabilitation, etc., as necessary. Review the resident's chart for specific treatments, medication orders, diets, etc., as necessary. Make periodic checks to confirm that prescribed treatments are being properly administered by CNAs and to evaluate the resident's physical and emotional status. Notify the resident's attending physician when the resident is involved in an accident or incident. Notify the resident's attending physician and responsible party when there is a change in the resident's condition. Administer professional services such as: catheterization, tube feedings, suction, applying and changing dressings/bandages, packs, colostomy, and drainage bags, taking blood, giving massages and range of motion exercises, care for the dead/dying, etc., as required. Monitor seriously ill residents, as necessary. Review care plans daily to confirm that appropriate care is being rendered. Confirm that CNAs are aware of the resident care plans. Verify that the CNAs refer to the resident's care plan prior to administering daily care to the resident. Other Duties Attend in-service education programs in order to meet facility educational requirements. Be familiar with Standard Precautions, Exposure Control Plan, Fire Drill & Evacuation Procedures and know how to use the information. Maintain confidentiality of resident and facility records/information. Protect residents from neglect, mistreatment, and abuse. Protect the personal property of the residents of the facility. Others as directed by the supervisor or administrator. Minimum Qualifications Registered Nurse or LPN with current state license. Excellent technical, assessment and documentation skills. Good Leadership qualities. Organized and detailed in work performance. Good communication skills with excellent self-discipline and patience. Genuine caring for and interest in elderly and disabled people in a nursing facility. Comply with the Residents Rights and Facility Policies and Procedures. Education and Communications * Patiently and clearly explains purpose and nature of exam to patient prior to starting of procedure, to minimize anxiety and facilitate cooperation using AIDET; Acknowledge, Introduce, Discuss, Explain, and Thank you of all patients and family members. * Ensures that patient's questions, concerns or contraindications are satisfactorily addressed, notifying physician when this cannot be accomplished, prior to the initiation of the procedure. * Participates in the orientation and training of departmental employees. * Required documentation is concise, legible and includes precise terminology. * Reads email each day scheduled to work and follows through with assignment. * Completes mandatory departmental and Hospital‐wide education without prompting. Quality Control * Consistently produces acceptable images, as evidenced by radiologist feedback and quality audits. * Ensures that equipment is fully operational prior to each use, by checking for proper calibration and operating deficiencies. * Sends all studies to PACS immediately after completion, prints CDs as needed. * Studies that have been transmitted after hours to teleradiography are printed and scanned in PACS, if applicable. * Completes all studies in RIS before completing in PACS. * Prints completed log sheet from teleradiography and submits to Radiologist, if applicable. Operations * Restocks linens and supplies, and maintains NM department in clean and orderly condition. * Transports patients, positions patients, and helps them on and off procedure table. * Performs patient charging activities on the day the service is rendered. * Applies supportive and immobilization devices and equipment. * Willingly lends knowledge and assistance to others upon request as needed and routinely as time is available, without prompting, so that all working time is used for departmental advantage. * Performs routine duties of clerical and support personnel in accordance with procedure when warranted to ensure continuity and quality of services. * Keeps supervisor informed on all responsibilities on an ongoing basis, ensuring prompt notification in the event of problems or potential problems. * Maintains an individual productivity rate equal to other co‐workers. * Ability to operate NM equipment within the department. * Maintains Joint Commission standards and any other regulatory agency standards. * Any other duties as assigned. * Takes call when necessary. Assists with or performs nursing evaluations under the supervision of a provider or RN. Works in the infusion rooms under the supervision of an RN. * Documents services performed and medications administered accurately and in a timely manner * Enters charges for services performed accurately and on time * Completes assigned evaluation tools within designated timeframe. * Informs clinic managers of clinic needs and concerns relating to the work environment. * Participates in quality improvement initiatives. * Participates on committees as requested * Attends all mandatory in services, training, meetings.
