Completes all daily documentation by the close of business each day. * Reports inmate protection investigation (IPI) and inmate threats to others to security as instructed by Safe Prisons Plan. * Processes cancellations of referrals to Behavioral Health Facilities. * Assessments written based upon balanced clinical judgment. * Effectively prioritizing job duties while maintaining risk management as a priority. About the Role Albany College of Pharmacy and Health Sciences (ACPHS) is seeking a Licensed Mental Health Counselor or Licensed Clinical Social Worker to provide contracting counseling services to clients of The Collaboratory, a provider of health services in Albany's South End. The counseling services will be offered free of charge to clients, funded through the Highmark Blue Fund grant, and supervised by faculty and staff of ACPHS. Services at The Collaboratory are delivered by ACPHS's Public Health Pharmacy Team (PHPT) and Trinity Alliance's Wellness Advocates Linking Communities (WALC) team. The mission of The Collaboratory is to leverage the talents of ACPHS students, faculty members, and partner organizations to help narrow health disparities in the South End. Our team of health care professionals includes pharmacists, educators, and current ACPHS students enrolled in the B.S. in Public Health program. Our goal is to leverage this new model of care to improve the health of our local community.
