The centers provide campus-based direct services and supports at 13 centers located throughout the state. The Facility Investigator, Investigator II, performs moderately complex (journey-level) investigative work and administrative tasks at the State Supported Living Center (SSLC). Performs field investigations of Abuse, Neglect, and Exploitation (ANE) and injuries involving residents of the facility. Performs comprehensive preliminary investigations to identify protection from harm and ensure reporting guidelines are followed. Follows-up on outstanding external cases such as those through Health and Human Services Commission Provider Investigations (HHSCPI) and the Office of Inspector General (OIG) to ensure documentation guideline dates are adhered to. Works in concert with Campus Administrators to ensure incidents that occur on non-business hours continue to move through the investigative process to remain in compliance with ICF and Settlement Agreement requirements. Verifies information, collects and properly maintains evidence, data and documents for use in investigations. Evaluates and summarizes investigative findings and writes reports. Prepares cases for presentation and presents testimony and evidence at administrative hearings, such as disciplinary hearings, or in court proceedings. Serves as a member of the Incident Management Team, conducts Root Cause Analysis meetings and participates on other assigned committees as needed. Completes data entry for unusual incidents and follows-up with unusual incident recommendations. Assists with tracking injury reports. Consults with State Office Incident Management on a regular basis. Works collaboratively with campus administrators to ensure incidents that occur after normal business hours are investigated promptly and move through the investigative life-cycle as required by applicable policy and procedures. Collects and reviews evidence and data through field investigations and data collection, including direct observations and review of video surveillance. Contacts and interviews witnesses and complainants. Evaluates, summarizes and documents investigative findings. Prepares reports and communicates findings to designated supervisory staff. Prepares cases for presentation at administrative hearings such as those involving staff terminations and presents testimony when required. Attends daily meeting related to Abuse Neglect and Exploitation (ANE) Unusual Incidents, peer-to-peer aggression and falls. Prepares detailed meeting minutes and follows up on identified issues. Serves as member of the Incident Management Team Serves as part of the critical incident team for unusual incidents after hours, providing technical assistance and support. Serves on other teams as appointed by the supervisor. Works effectively with external agencies including HHSC, State-Office, law enforcement (including OIG), HHSC-PI and other entities, to complete collaborative investigations as assigned by the supervisor. Reviews and summarizes information received and other evidence collected to produce investigative findings. Effectively utilizes the assistance of other staff to complete investigations successfully. Follows up with HHSC-PI and OIG when necessary to ensure documentation guidelines are adhered to, supplying them with information necessary for timely resolution of their cases. Completes data entry and coordinates actions following unusual incidents, serious client injuries, and abuse/neglect allegations, and ensures incidents are reported to HHSC Regulatory, HHSC State Office, and other agencies within required timeframes. Assists with tracking of these reports. Complies with the requirements and protocols of the essential elements of incident management. Reports incidents to the HHSC-PI per incident management requirements. Participates in, and facilitates, individualized root cause analysis related to specific investigations and makes recommendations for proactive measures to prevent future injury occurrences and/or policy violations. Participates on work groups and committees related to incident management and root cause analysis to analyzes the general SSLC operations to ensure compliance with statutory standards and regulations. Manages the automated work order program, Computer Assisted Facility Management (CAFM), in order to establish and update annual goals and needs of the physical plant and equipment to ensure building deficiencies are properly entered and tracked for capital construction projects. Uses online systems for vendors such as FedEx and LoneStar to create shipping accounts for HHS customers. (25%) Performs standard warehouse duties including the safe and efficient storage and inventory of materials, equipment, and supplies. Operates necessary motorized and manual equipment for moving and lifting pallets, boxes, and any other large item to be stored or transported. Monitors stock levels, advises manager when reorders are needed, receives refill orders, and fills racks to maintain sufficient stock levels. To provide additional PHN direct patient care services, case management and other duties as assigned, as needed for Communicable Disease support. Essential Job Functions Evaluates patients for Congenital Syphilis risk factors, clinical presentation, and screening results in various settings (e.g. patient's home or public health clinic). * In collaboration with the authorizing physician develops a treatment plan; establishes mutual goals, monitors the patient's treatment response. * Engages patients in behavior modification to promote treatment adherence. * Will provide consultation, training, and technical assistance to agency staff, care recipients, provider agencies, and the medical community to ensure treatment is completed * Collects specimen for laboratory testing to include urine, blood via venipuncture or fingerstick, and self-collection swabs * Maintains detailed records of patient encounters in state databases. (40%) Community Education: * Serves as a technical consultant and resource to individuals, providers and stakeholders by providing information on CDC/DSHS standards regarding the current testing, diagnoses and treatment of Congenital Syphilis, as well as other STIs and HIV. o Facilities involved can be health facilities, county correctional facilities, residential substance abuse facilities, and other community agency facilities. * Communicates with private healthcare providers in the community providing specific and detailed clinical guidance according to the recommendations given from the CDC. * Participates or leads multidisciplinary meetings and trainings to review all aspects of Congenital Syphilis case management and provide guidance and recommendations to promote consistent and optimal patient care. (20%) Disease Intervention Activities: * Independently performs detection, intervention, reporting, notification, and referral activities related to persons with CS infections and their contacts, at high risk for CS transmission or other HIV/STI cases as needed. o This includes injection drug users, and other hard-to-reach and special populations, including, homeless persons, sex industry workers, transgender and intersexed persons, incarcerated populations, and persons whose primary language is not English. * Ensures statutory disease reporting, achievement of program performance standards, and abides by the DSHS DIS guidelines, DSHS Congenital Syphilis guidelines and HHS HR manual. (20%) Consulting * Consults initially with the regional medical director (RMD) on complicated Congenital Syphilis cases or HIV/STI cases as needed * Consults secondarily with Texas DSHS recognized medical experts on complicated Congenital Syphilis cases or HIV/STI cases as needed as needed. (10%) Emergency Preparedness and Response - Emergency response duties as assigned to include planning for a disaster event response, actively participating during a disaster response and/or recovery effort. Serving in a supporting role during a disaster response and/or recovery effort and assisting with agency continuity of operations during disaster response and/or recovery efforts. The Rehabilitation Therapy Technician (RTT) performs moderately complex habilitative and rehabilitative therapy work. Provides assistance to therapists for an assigned therapeutic specialty, (e.g., music therapy, therapeutic recreation, dance/movement therapy, occupational therapy, physical therapy, psychology, or vocational /day programming). Provides direct contact instruction/training for individuals with intellectual and developmental disabilities on their treatment plans for the RTT's assigned therapeutic specialty. Trains others to ensure continuity of treatment plans. Valid driver's license and ability to drive a state vehicle may be required. Works under the general supervision of a Registered Therapist, Psychologist, or Vocational/Day Programming Supervisor with moderate latitude for the use of initiative and independent judgment. This position is required to drive a state-owned vehicle. Essential Job Functions (EJFs): Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned. Directly implements treatment plans through "hands on" methods, monitors treatment goals and objectives through direct contact observation. Assists in the development and evaluation of treatment plans for assigned therapeutic specialty (e.g., music therapy, therapeutic recreation, dance/movement therapy, occupational therapy, physical therapy, psychology, or vocational rehabilitation). Assists the assigned therapist or vocational/day programming supervisor in conducting functional skills assessments (e.g., preparing materials for assessments, reviewing prior assessments, conducting direct contact observations). Schedules, assists with and/or provides staff training to Direct Support Professionals (DSP's) to ensure continuity and follow through of treatment plan objectives during hours when Therapists or RTTs are not available and depending on SSLC needs. Monitors the implementation of the program objectives by the DSP's, gathers data and report this to the interdisciplinary team as well as the therapist or supervisor overseeing their respective discipline. Ensures the satisfactory completion of paperwork in program related areas in order to meet applicable standards and procedures. Ensures the collection of data related to treatment plans in order to accurately assess the individual's progress. Participates in the interdisciplinary team process including providing information regarding the results of functional skills assessments and response to interventions as well as the results of monitoring and staff training efforts as mentioned above. Provides feedback to the team about the individual's progress, works with Direct Support Professionals (DSP) to maintain a connection and assists with the development of treatment plans for the assigned discipline. Maintains current skills in relation to assigned responsibilities. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location. Knowledge, Skills and Abilities (KSAs): Knowledge and skills in relevant therapeutic techniques (e.g., applied behavior analysis, sensory integration, etc). Ability to train staff in treatment methods and related areas. Ability to communicate effectively, orally and in writing. Ability to assist in development of treatment and support plans. Ability to develop and maintain data collection systems for assigned therapeutic specialty (e.g., music therapy, therapeutic recreation, dance/movement therapy, occupational therapy, physical therapy, psychology, or vocational rehabilitation). Ability to utilize personal computers and relevant software. General knowledge, or ability to gain knowledge, of ICF/IID regulations and state supported living center policies and procedures. Ability to meet the same physical requirements as those for direct contact staff positions. Ability to work cooperatively and effectively with others. Registrations, Licensure Requirements or Certifications: Valid Texas driver's license or ability to obtain it no later than 90 days after hire date, is required or preferred. Applicants with an out-of-state driver's license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Management policy Initial Screening Criteria: High school diploma or GED preferred. At least six months of work experience with people who have intellectual and developmental disabilities or in performing therapeutic work. Experience in the areas of applied behavior analysis, sensory integration, music therapy, therapeutic recreation, dance/movement therapy, occupational therapy, physical therapy, psychology, and/ or vocational rehabilitation is preferred. Experience in the development of treatment and support plans for people with intellectual and developmental disabilities is preferred.
