The Child Assessment Center (CAC) investigations team handles a wide range of cases such as serious child injuries, near fatalities, medically fragile and sexual abuse. Due to the nature of these cases, collaboration with law enforcement is common. Investigators may also participate in forensic interviews at children's advocacy centers. This role provides extensive on-the-job experience and exposure to critical cases, offering a learning opportunity that many investigators may not encounter.
Brief :
Child Protective Investigations (CPI) Investigators investigate claims of child abuse and neglect. They have the difficult task of figuring out what happened and predicting what will happen in the future. CPI receives and investigates reports of abuse and neglect 24 hours per day, every day of the year.
A CPI investigation includes interviewing and gathering information to see if abuse or neglect happened and if intervention is necessary. The investigator considers both risk and safety issues and may recommend services for the child and family to reduce the risk of further abuse or neglect.

* Determines action to be taken to remove or to reduce an immediate threat to the safety of a child to include working with families to identify family members who can assist with keeping the child safe, testifying in court to seek emergency protective services, placing children in substitute care, referring family for immediate crisis intervention therapy or other community resources.
* Documents all relevant and appropriate information gathered during the investigation and completes all required forms accurately and in a timely manner.
* Gathers family and kinship information to support the child in a placement, should the child be placed in DFPS custody.
* Participates in a regular on-call rotation that requires response to situations of abuse/neglect after normal business hours including overnight and weekend.
* Develops and maintains effective working relationships with law enforcement officials, judicial officials, legal resources, medical professionals, and the community.

Additional Information:
This position may be filled at any level from a Child Protective Investigations Specialist I to a Child Protective Investigations Specialist IV. Factors such as education and experience may be considered when establishing the starting salary.
Applicants selected for this position are also eligible to receive a $416.66 monthly increase in addition to the base salary. Applicants hired into an investigative position will receive this increase the first of the month following 120 days of tenure. Once employed, the incumbent must remain in an investigative position to continue to receive the additional pay. If you leave Family Investigations or AR and return within the last 12-months, the INV stipend is given immediately. All other situations must wait the 120 days to receive the INV stipend.
This position will be in a mobile unit which means the majority of the work will be conducted using mobile technology, such as a tablet, while away from the office. Being mobile requires working independently yet still being responsive to supervision and your assigned unit.
Newly hired DFPS employees in eligible positions will be assigned a DFPS cellular phone.

HCS Surveyor - Starting Market Rate Salary $60,431.88
As part of a collaborative team of public servants, a surveyor protects individuals' health and safety by conducting on-site residential surveys, monitoring, and investigations of Home and Community-Based Services (HCS) Medicaid program providers to assess their compliance with state and federal laws and rules. These include investigations into allegations of abuse, neglect, and exploitation. They have the difficult task of figuring out what happened and predicting what will happen in the future. A CPI investigation includes interviewing and gathering information to see if abuse or neglect happened and if intervention is necessary. The investigator considers both risk and safety issues and may recommend services for the child and family to reduce the risk of further abuse or neglect.

Additional Information:

The QIDP III is central to the overall responsiveness and effectiveness of an individual's active treatment program at an SSLC through oversight of the Individual Support Plan (ISP) process, which is centered on the individual's preferences, strengths, and needs, and includes the creation of the ISP. Responsible for coordination, monitoring, documentation, and follow-up on all programs, services, and plans for the individuals on their caseload. Leads and coordinates the interdisciplinary team (IDT). Monitors and provides oversight of the active treatment program of all individuals on the assigned caseload. Determines the need for program revision, identifies and resolves inconsistencies in training approaches, as well as assures timely follow-up for any IDT recommendations. Facilitates the acquisition of skills and adaptive behaviors and promotes competent interactions of staff with individuals in program implementation and behavior management. Complies with federal and state standards governing Intermediate Care Facilities for Individuals with Intellectual Disabilities (ICF/IID), the requirements of the Settlement Agreement with the Department of Justice, and the SSLC's policies and procedures. Ensures follow-up occurs for any recommendations for supports, services, equipment, or programs so that needed services and supplies are provided in a timely manner to meet the individuals' needs.
Routinely observes and interacts with the individuals across settings in program areas.
Collects and analyzes data on ISP implementation and effectiveness documenting information accordingly. Coordinates with the IDT when modifications are necessary due to progress, lack of progress, significant change in behavioral or health status, or for the continued or improved effectiveness of the treatment, service, or support.
Trains/coaches staff or coordinates training for staff in the implementation of services, supports, and teaching methodologies. Provides feedback to staff to promote competent interactions with individuals in program implementation and behavior management. Provides leadership, ad hoc training to all levels of staff on specific issues related to their assigned individuals, and serves as a role model to home, program, and professional staff.
Assists in the training, coaching and mentoring of other QIDP staff as needed.

As part of a collaborative team of public servants, a surveyor protects individuals' health and safety by conducting on-site residential surveys, monitoring, and investigations of Home and Community-Based Services (HCS) Medicaid program providers to assess their compliance with state and federal laws and rules. These include investigations into allegations of abuse, neglect, and exploitation. The Materials Management Specialist (Vehicle Driver) reports to the Regional Business Services Manager (BSM) in each region within Regional Administrative Services (RAS). The Training Specialist II performs routine training work designed to ensure employees are effectively trained in the core and specialty competencies required for their position. Work involves planning, designing, organizing, conducting and evaluating educational and training programs. Trains new employees during New Employee Orientation, conducts Refresher Training for employees and serves as a liaison to all departments and programs of the facility. Assignments, products, tools, equipment and procedures are specified by HHSC Facility Support Services Competency Training Department, but the trainer can exercise creativity in the selection of procedures and/or methods used to accomplish tasks and to make the training interactive and interesting for trainees. Completion of work requires originating new techniques, establishing criteria or developing new methods of teaching information of a routine nature as needed. The Training Specialist II works under the general supervision of the Facility Competency and Training Department (FCTD) Director and assists other trainers in class-type settings including training other trainers as needed. Must understand that duties, location of job assignments, and shift patterns are subject to change as determined by the administration to meet the needs of the facility. Employee actively participates and/or serves in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
Plans, organizes, and conducts training classes as assigned in group class -type settings or computer lab environments to include New Hire Orientation, refresher training and continuing education classes to ensure trainees' competency. Evaluates employee competencies and skills as outlined in policy and/or course requirements. Acts as a substitute trainer for other FCTD team members in case of absences to ensure constant delivery of training classes when scheduled.
Promotes training quality and effectiveness; customizes HHS FCTD standardized training to increase trainee perception of relevance to the facility's environment and uses creativity and/or other training techniques to inject interactivity and interest to increase trainee engagement and retention of training modules; ensures all aspects of training support the delivery of services to the facility's patients and/or clients while providing safety for the employee.
Maintains accurate and complete training records as required in order to correctly document staff training and to develop and provide reports of training due or delinquent. Ensures proper attendance records are completed for all assigned classes and submitted to the appropriate FCTD team member and/or entered in all applicable record-keeping systems.
Assists with FCTD team's development of curriculum materials, including multimedia presentations, as needed for classes in order to utilize all available resources including state-of-the-art techniques in training. Demonstrates an awareness of safety and infection control practices to ensure safe work experiences. Maintains department equipment per guidelines so that equipment remains clean and in good repair. Reports any problems with equipment in a timely manner.
Assist facility managers and other FCTD team members with identification of needed training and/or training issues arising in classes or identified by participants, and development of training responses to those needs or improvement of training delivery. Conducts post-orientation (on-the-job) training assessments once new employees have been trained in the residential setting, when needed, and completes required documentation.
Maintains necessary certifications to teach assigned classes including specialty HHS classes developed for interaction with SSLC individuals such as trauma-informed care, response blocking (Ukeru) and Safe Use of Restraints (SUR) and/or general public response classes such as CPR. Serves as a Master Trainer to train other trainers, as assigned. Serves as a subject matter expert on training curriculum.
Attends and participates in facility meetings as assigned to provide training perspectives, quality improvement recommendations or discussions of training for facility's programs, and reports of training statistics of the facility's employees. Conducts needs assessments and program assessments to determine training sufficiency and areas for improvement.
Performs other duties as assigned in order to ensure services are provided.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Nursing Facilities Social Services Surveyor - Starting Market Rate Salary $73,867.50
As part of a collaborative team of public servants, a surveyor protects residents' health and safety by conducting on-site inspections and investigations in Texas long-term care facilities (nursing homes, assisted living facilities, day activities and health services) to assess their compliance with state and federal laws and rules. This position offers a competitive starting salary; excellent state benefits, including health insurance and retirement; and the opportunity to regularly travel to different facilities within a region and across the state - Please see the Additional Information section below for more detail on travel. In this front-line field position, a surveyor is mobile and flexible and performs minimal work at an office, though the role does require writing reports detailing any deficiencies/violations against a facility, remedies to correct those deficiencies, and narrative to support regulatory determinations. Under the supervision of the Manager of the Contract Accountability and Oversight Unit (CAO), the Contract Specialist IV (CSIV) provides advanced consultative and technical assistance to HHSC program staff, HHSC contract management staff and Local Intellectual and Developmental Disability Authority (LIDDA) staff. Work involves oversight and monitoring of contractual and program requirements for LIDDAs with HHSC. The CSIV conducts desk reviews and on-site reviews to determine compliance with rules, policies, and procedures. The CSIV routinely reviews and analyzes LIDDA performance data to assess risk of contract and program non-compliance and determines trends by reviewing, monitoring, and evaluating LIDDA contract deliverables (e.g., record reviews, review of policy and procedures, etc.). Conducts follow-up activities with LIDDA staff and reports analysis of data to HHSC Contract Management staff. At the conclusion of each on-site and/or desk review, the CSIV presents a verbal summation of all areas of non-compliance and provides consultative services and technical assistance to LIDDA staff as well as develops report of findings to be shared with LIDDA executive staff, stakeholders, and HHSC staff. Participates in unit and cross-agency workgroups related to LIDDA performance, functions, and planning. May assign and/or supervise the work of others while in a facilitator role within the review. Works under limited supervision with considerable latitude for the use of initiative and independent judgment.
Essential Job Functions (EJFs):
Task 1: Facilitate, coordinate, and conduct on-site contract and program monitoring reviews of the LIDDAs' performance to determine compliance with rules, laws, regulations, policies, and procedures, which includes record reviews and interviews with staff, individuals, and families. Review supporting evidence regarding systemic and monitoring activities identified in plans for improvements. Review and monitor the LIDDAs' data and documentation about individual services, service delivery systems and the provision of the delegated functions (e.g., local planning, policy, and procedures). Review and analyze the current and prior year report of findings to determine whether additional on-site reviews are required according to the performance contract. (35%)
Task 2: Prepare for on-site, hybrid, or desk reviews. Ensure that the General Revenue (GR), Texas Home Living (TxHmL), Preadmission Screening & Resident Review (PASRR), Home and Community-based Services (HCS), Permanency Planning and Community Living Options Information Process (CLOIP) samples are drawn and shared with LIDDAs for reviews. Review critical incident reports and complaints. Review corrective action plan (CAP) results from the previous year to determine the review period and identify and address consecutive findings of contractual non-compliance. Develop and make travel arrangements. Prepare and send announcement letters to the contract provider. Make team assignments for monitoring reviews and prepare packets (e.g., agenda, interview activity information, CARE screens, and electronic review tool, review packets). (15%)
Task 3: Review, monitor and evaluate LIDDA contract deliverables by reviewing evidence of compliance or review additional records. (20%)
Task 4: Conduct desk-review of LIDDA performance of program and contract compliance by reviewing documentation for permanency planning for individuals under 22 years of age and CLOIP for residents in state supported living centers (SSLC). (10%)
Task 5: Produce well-written reports (e.g., debriefings, develop comprehensive reports of findings related to review activities, develop corrective action documents). (10%)
Task 6: Maintain current knowledge of all rules and policies as it relates to LIDDA program requirements to provide consultative services and technical assistance on a complex level to LIDDA staff and other stakeholders, HHSC staff, and DSHS staff. (5%)
Task 7: Participate in unit and cross-agency workgroups related to LIDDA performance, functions, and planning. Represent the department at meetings and conferences. Work effectively with staff from LIDDAs and other external stakeholders. Brief Job Description:
The Public Health Nurse II performs complex (journey-level) public health nursing work in a clinical or non-clinical setting. Works under general supervision, with moderate latitude for the use of initiative and independent judgment. The Nurse II is responsible for services in the region, including participation in current and long-range planning for HIV/STI prevention and intervention in assigned areas. In coordination with the applicable program managers and/or supervisors ensures program goals and objectives are met. To provide additional PHN direct patient care services, case management and other duties as assigned, as needed for Communicable Disease support.
Essential Job Functions (EJFs):
Evaluates patients for Congenital Syphilis risk factors, clinical presentation, and screening results in various settings (e.g. patient's home or public health clinic). In collaboration with the authorizing physician develops a treatment plan; establishes mutual goals, monitors the patient's treatment response. Engages patients in behavior modification to promote treatment adherence. Will provide consultation, training, and technical assistance to agency staff, care recipients, provider agencies, and the medical community to ensure treatment is completed. Collects specimen for laboratory testing to include urine, blood via venipuncture or fingerstick, and self-collection swabs. Maintains detailed records of patient encounters in state databases. 40%
Community Education: Serves as a technical consultant and resource to individuals, providers and stakeholders by providing information on CDC/DSHS standards regarding the current testing, diagnoses and treatment of Congenital Syphilis, as well as other STIs and HIV. Facilities involved can be health facilities, county correctional facilities, residential substance abuse facilities, and other community agency facilities. Communicates with private healthcare providers in the community providing specific and detailed clinical guidance according to the recommendations given from the CDC. Disease Intervention Activities: Independently performs detection, intervention, reporting, notification, and referral activities related to persons with CS infections and their contacts, at high risk for CS transmission or other HIV/STI cases as needed. This includes injection drug users, and other hard-to-reach and special populations, including, homeless persons, sex industry workers, transgender and intersexed persons, incarcerated populations, and persons whose primary language is not English. Ensures statutory disease reporting, achievement of program performance standards, and abides by the DSHS DIS guidelines, DSHS Congenital Syphilis guidelines and HHS HR manual. 20%
Consulting: Consults initially with the regional medical director (RMD) on complicated Congenital Syphilis cases or HIV/STI cases as needed. Consults with Texas DSHS recognized medical experts on complicated Congenital Syphilis cases or HIV/STI cases as needed. 10%

The state supported living centers serve people with intellectual and developmental disabilities who are medically fragile or who have behavioral problems. The Music Minister reports directly to the Chaplain. The Music Minister conducts music performance groups for regular and special religious services as well as facility events and community performances. Groups include hand bells, vocal choir, sign choir, percussion/band. The ability to perform music with at least one portable instrument is required to lead worship in a variety of indoor and outdoor settings. The Music Minister plans, organizes, and holds annual musical events such as local music festivals or trips, awards ceremony, holiday musicals and pageants. Ability to serve as lay leader for a variety of religious beliefs. The Music Minister is also responsible for providing individual and group music therapy including assessment and ongoing sessions as well as assisting with active treatment options for individuals at the center. The position works regularly hours on the weekends as well as some weekdays.
Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.

Registrations, Licensure Requirements or Certifications:
Music Therapist board certified or obtain with 1 year. Must possess valid Texas driver's license or obtain it no later than 90 days after hire date. Applicants with an out-of-state driver's license must provide an original certified driving record from the state of driving licensure. Eligible driving record required based on HHSC Fleet Management policy.
Initial Screening Criteria:
Board Certified Music Therapist preferred
Experience working with people with Intellectual Disabilities
Music performance experience
Experience conducting musical groups

Additional Information:
Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aide, Medication Aide and Employee Misconduct and HHS List of Excluded Individuals/Entities (LEIE).

Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours. Must be willing to work weekends and holidays.
