Nursing Facilities Investigator V - Starting Market Rate Salary $ 75,827.44
As part of a collaborative team of public servants, an investigator protects residents' health and safety by conducting on-site inspections and investigations in Texas long-term care facilities (nursing homes, assisted living facilities, and day activities and health services sites) to assess their compliance with state and federal laws and rules. This position offers a competitive starting salary; excellent state benefits, including health insurance and retirement; and the opportunity to regularly travel to different facilities within a region and across the state - Please see the Additional Information section below for more detail on travel. In this front-line field position, a surveyor is mobile and flexible and performs minimal work at an office, though the role does require writing reports detailing any deficiencies/violations against a facility, remedies to correct those deficiencies, and narrative to support regulatory determinations. Essential Job Functions (EJFs):
Under the direction of a program manager, an investigator V conducts on-site licensure inspections, complaint and incident investigations, and annual comprehensive surveys in facilities to determine compliance with federal and state health and safety regulations. Completes inspection or survey reports detailing deficiencies.
Communicates clearly and accurately with others (internally and externally) to provide, exchange, or verify information, answer inquiries, address issues, or resolve issues or complaints in Texas long-term care facilities.
Conducts exit conferences with facility staff regarding findings.
Visits long-term care facilities to verify correction of previously cited regulatory violations.
Performs quality assurance reviews of documents prepared by others, for example, publications, assessments, reports.
Occasionally might need to testify at hearings or in court proceedings to provide information regarding policy/procedures, history, findings, or activities.
In the event of emergency licensure action, be available to assist with non-skilled care and oversight to residents in a facility. If the employee is a current registered or licensed nurse, there is a potential for skilled care.
Be available for on-call duties for emergencies such as weather events or high-priority situations in facilities or at providers.

* Keeps the child's, parents, caregivers, court-appointed attorney and guardian ad litem(s) informed about the child's circumstances and significant events.
* Works with the department's attorney to prepare for contested-court hearings and trials.
* Works with kinship caregivers and foster parents to ensure that they have what they need to care for the child or youth placed with them i.e., keeping them informed about developments in the case, returning phone calls, and in some areas of the state being available 24 hours a day / 7 days a week at certain times.
* Transitions children home during reunification services and provides support to the family until the legal case is closed.
* Supervises adoptive placements until the adoption is final or until the case is transferred to an adoption caseworker.
* Using effective time-management skills to make sure all key tasks are done.
* Documents case records by completing forms, narratives, and reports to form a written record for each client.

Additional Information:
Community-Based Care (CBC) is a new way to provide services than traditional foster care because it gives local communities the flexibility to draw on local strengths and resources and find innovative ways to meet the unique and individual needs of children and their families. CBC includes many of the services that Child Protective Services (CPS) normally provides. This includes foster care, case management, kinship, and reunification services. A single contractor in each designated community area creates a network of services, foster homes, and other living arrangements and, when ready, provides case management for each child and their family as well. CPS works with each contractor (Single Source Continuum Contractor or SSCC) to carefully manage the transition from traditional foster care to community-driven care. Full implementation of CBC is projected for the year 2029 across the state of Texas. With open proposal options, any given community or regional area may be selected for CBC transition. This position along with your job duties and function will shift form a state position with Texas Department of Family Protective Services to the SSCC, which is a private and non-profit agency serving the same foster care population. Brief :

Additional Information:
Human Service Technicians may be required to work together with DFPS caseworkers in providing supervision of children without placement (CWOP). Children can vary in age, but primarily the population that will be worked with is children that can range from 10 to 17 years of age. Responsibilities of the position include assisting the DFPS caseworker with monitoring children and ensuring needs are met during their office or community location stay to include: safety, food, hygiene, education and entertainment/recreational activities. This could require working outside of the normal work schedule. If so, overtime compensation may be earned.

The position supervises a unit of caseworkers and clerical staff responsible for providing child protective services. The position oversees the unit in accordance with Child Protective Services Handbook, Title III of the Texas Family Code and Minimum Standards of child placing agencies. The role of a Conservatorship supervisor is a fast-paced, high energy position requiring daily assessment and decision-making of child safety issues, and permanency. The Nurse Manager performs advanced (senior-level) nursing functions such as; planning, developing, coordinating, and evaluating the provision of nursing care on a 24-hour basis. The Psychiatric Nursing Assistant II (PNA II) performs moderately complex (journey-level) rehabilitative patient care services work under the clinical supervision of a Registered Nurse (RN) to provide quality psychiatric nursing care in an inpatient setting. Work involves providing emergency, health, and personal care services to patients; observing, reporting, and documenting, in an electronic record, patient conditions and behaviors; interacting with patients therapeutically; and participating in individualized and unit-based programming. The PNA II has a primary responsibility to maintain direct and ongoing therapeutic interactions with patients to promote living and social skills, thus encouraging progress towards a lesser restrictive environment. As an integral member of the recovery team, provides verbal and supportive documentation concerning individual patient behaviors, including progress or lack thereof, towards individualized goals of the recovery plan. Participates in shift reports and recovery team meetings. Performs delegated nursing duties commensurate with competency. May provide guidance to the PNA Is. Works under general supervision, with limited latitude for the use of initiative and independent judgement. Completion of work requires the performance of moderately complex duties, while decision-making is based on well-defined guidelines. The PNA II has no financial responsibility but is accountable for assigned property. Must understand that duties, location of work, and shift patterns are subject to change as determined by the administration to meet the needs of the hospital. Must understand that duties may include exposure to blood or other potentially infectious materials. This employee has full access to a limited set of records in accordance with HIPPA regulations.
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

* Uses basic computer skills, documentation skills, and reporting procedures to communicate patient care information clearly and concisely in a timely manner.
* Assists the licensed nurse in monitoring patients for medication administration or treatment. Includes taking and monitoring vital signs and performing other assigned tasks within competency level.
* Proactively promotes safety through continuous environmental awareness and positive engagement of patients to minimize unsafe situations and injuries.
* Promotes classes and programs offered and encourages patient participation. Co-facilitates groups as assigned or requested.
* Responds in crisis or psychiatric emergency situations using the least restrictive intervention necessary by applying Treatment Intervention Protection Strategies (TIPS), including verbal de-escalation, under the direct supervision of a RN to ensure the health and safety of all persons.
* Performs CPR/Heimlich techniques and use of an Automatic External Defibrillator (AED) correctly and safely according to the specifications of the American Heart Association.
* Provides RN with timely, clinically relevant patient information (such as possible side effects of medication, disease/injury, behavioral escalation, etc.) gained by direct observation/interaction with patients to promote safety.
* In the absence of the PNA III and IV, provides guidance and preceptorship, as requested, to the PNA Is in the performance of duties assigned by a RN to maintain smooth, efficient provision of patient care.
* Serves in both unit-based and hospital-wide committees as appointed, contributing to interdisciplinary efforts focused on quality improvement, patient safety, and strategic planning. Responsibilities include attending regular meetings, collaborating with team members, and offering insight to enhance patient care and support departmental goals.

Registrations, Licensure Requirements, or Certifications:
* If transporting patients, must possess a valid Texas driver license or obtain it no later than 90 days after hire date to drive a state vehicle. Eligible driving record required based on HHSC Fleet Management policy.
* CPR certification or the ability to obtain certification for CPR within the first 60 days of employment.

Discusses with parents their progress towards making changes to behaviors that pose dangers to their child(ren).
* Meets with children, parents, family friends, or foster homes in public as well as in their own homes.
* Collaborates with a Placement Team, including Kinship staff, for placements, as needed.
* Participates in meetings and conferences at times and places convenient for the family members as well as everyone involved in the case.
* Visits children monthly to assess the child's feeling of safety in their current home, to plan for permanency, and to discuss their needs, wishes, and progress while in care
* Attends and participates in court hearings about the child and family. This includes contacting the parties in the case before hearings, preparing court reports, and testifying in court on the child's needs, the family's progress, and the department's efforts to achieve permanency for the child.
* Keeps the child's, parents, caregivers, court-appointed attorney and guardian ad litem(s) informed about the child's circumstances and significant events.
* Works with the department's attorney to prepare for contested-court hearings and trials.
* Works with kinship caregivers and foster parents to ensure that they have what they need to care for the child or youth placed with them i.e., keeping them informed about developments in the case, returning phone calls, and in some areas of the state being available 24 hours a day / 7 days a week at certain times.
* Transitions children home during reunification services and provides support to the family until the legal case is closed.
* Supervises adoptive placements until the adoption is final or until the case is transferred to an adoption caseworker.
* Using effective time-management skills to make sure all key tasks are done.
* Documents case records by completing forms, narratives, and reports to form a written record for each client.
* Develops and maintains effective working relationships between Child Protective Services staff and law enforcement officials, judicial officials, legal resources, medical professionals, and other community resources.

Additional Information:
Community-Based Care (CBC) is a new way to provide services than traditional foster care because it gives local communities the flexibility to draw on local strengths and resources and find innovative ways to meet the unique and individual needs of children and their families. CBC includes many of the services that Child Protective Services (CPS) normally provides. This includes foster care, case management, kinship, and reunification services. A single contractor in each designated community area creates a network of services, foster homes, and other living arrangements and, when ready, provides case management for each child and their family as well. CPS works with each contractor (Single Source Continuum Contractor or SSCC) to carefully manage the transition from traditional foster care to community-driven care. Full implementation of CBC is projected for the year 2029 across the state of Texas. With open proposal options, any given community or regional area may be selected for CBC transition. This position may be filled at any level from a Child Protective Services Conservatorship Worker I to a Child Protective Services Conservatorship Worker IV. Factors such as education and experience may be considered when establishing the starting salary.

This position will be in a mobile unit which means the majority of the work will be conducted using mobile technology, such as a tablet, while away from the office. Being mobile requires working independently yet still being responsive to supervision and your assigned unit.

Child Protective Investigations (CPI) Investigators investigate claims of child abuse and neglect. They have the difficult task of figuring out what happened and predicting what will happen in the future. CPI receives and investigates reports of abuse and neglect 24 hours per day, every day of the year.
