Essential Job Functions:

EJF 1 (40%) Performs public healthnursing duties for the Immunization program, including but not limited to:

(10%) POPULATION-BASED SERVICES: Provides Essential Public Health Services to assigned communities. Participates in community assessments, identifies health needs, plans and implements strategies, and evaluates outcomes in conjunction with community stakeholders/partners and Regional work plans. Develops and implements community health education programs, establishes and maintains communication and partnerships with stakeholders including local/county officials, community leaders, other agencies, groups, healthcare providers, schools, and the general public. JOB DESCRIPTION:
Under the general direction of the Family & Community Health (FCHS) Services Nurse Supervisor II, the Program Manager and medical direction of the Regional Medical Director, this position performs routine public healthnursing work in clinical and/or non-clinical settings based out of Big Spring, Texas. It will require regular and frequent travel to the field offices located in Brady, Sonora, Fort Stockton and other satellite sites.
This position coordinates the delivery of public healthnursing services and essential public health services through the application of education, assessments, quality assurance and direct client services. Will work under moderate supervision with limited latitude for the use of initiative and independent judgment.
Essential Job Functions:
(35%) DIRECT CLIENT SERVICES:
Administers medications, immunizations, testing and treatments according to regional and DSHS Standing Delegation Orders and procedures. Assesses and plans the delivery of direct public health services and essential public health services to individuals, families, and communities. Conducts physical assessments of patient health needs and implements appropriate actions. Prepares and maintains patient records, program reports and patient assessments. Instructs, counsels and assists patients in meeting their health care needs. Responsible for using independent clinical judgement when making referrals for medical and social services with follow-up as necessary. Responsible for the management of pharmaceuticals, vaccine inventory and storage.
(35%) POPULATION BASED COMMUNITY OUTREACH ACTIVITIES:
Provides and explains information regarding Child Health, Maternal Health, and Perinatal Infant Health and Tobacco free information to the public through one:one education sessions, presentations and community events. Must plan events, attend trainings, and network with community stakeholders and community members. Assists with public health need assessments and verification of reports data. Assists other public health staff in communicating with agencies/individuals on the treatment, control, or prevention of diseases or injury.
(20%) DISEASE PREVENTION/CONTROL
Identifies community resources and makes referrals to health care and related services. Support the implementation and monitoring of a comprehensive case management service plan for clients and families. Makes home visits independently. Supports the implementation of the surveillance of communicable diseases and other health conditions, and plans interventions at the community or regional level. Assures compliance with Immunizations; Tuberculosis Control; HIV/STD, vector-Bourne diseases and prevention of vector-Bourne diseases.
(10%) OTHER RELATED DUTIES AS ASSIGNED
Other related duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations activation. Such participating may require an alternate shift pattern assignment and/or location. Safe operation of either a state-owned vehicle or personal vehicle is required to conduct state business and an acceptable driving record must be maintained in accordance with DSHS Policy OS-3901, Fleet Operations and Management & Procedures and standards developed by the Texas Department of Public Safety. Complete all trainings related to job duties on a timely manner.

Public HealthNurse I, under the supervision of a PHN III Nursing Supervisor, the PHN I is responsible for the delivery of public healthnursing services and day to day direction of a public healthnursing clinic and the support staff. Provides individualized care according to policy, procedure, program standards, physician orders and standing delegation orders. Participates in the development and revision of policies, procedures, and standing delegation orders. Develops and monitors individualized plans of care, which may include lab testing, counseling, home visits, administering medications and vaccines, education, case management, referrals and follow-up. Performs disease intervention, surveillance and prevention functions for TB, HIV, STDs, perinatal hepatitis B and immunizations, including administering vaccines. Applies epidemiological principles and methods to assess, prevent, and control disease and promote health.
COORDINATION AND TEAM LEADERSHIP (15%): Participates in the evaluation of nursing services; training/orientation of staff; interpreting program operations, policies and procedures; and writing and monitoring annual clinic and MCH goals. Coordinates schedules. Assigns and delegates work to assure that services are provided. Manages clinical resources and supplies. Conducts and documents quality assurance monitoring activities to include review of medical records and clinic operations. Makes recommendations for improvement based on findings. Prepares and maintains program, statistical and special reports as assigned. Participates in interdisciplinary teams to plan, develop and assess patient care and population-based services. Serves as the team leader for the Community Health Improvement staff in the field office. Provides significant input to the Manager in the development, review and updating of job descriptions and participates in the screening and selection process. Orients, trains and monitors the performance of the field office staff. Makes recommendations regarding discipline and reward. Provides guidance to staff, collects data, and provides input to the manager/supervisor regarding staff performance for evaluations.

Functions under the Community Health Services Manager with direct supervision by the 6/5S Nurse III under the general direction of standing delegation orders of the Regional Medical Director. Develops individualized plans of care, which may include lab testing/counseling, field visits, providing/administering medications and immunizations, providing education and counseling case management, referrals, and follow-up. Responsible for accurate documentation of services rendered in the client record. Responsible for assuming the proper handling and storage of vaccine, pharmaceutical and medical supplies. Phlebotomy skills required.
Coordination & Leadership-20%: Participates in evaluation of nursing services, training/orientation of staff as needed, interpreting program operations and policies and procedures, writing and monitoring annual clinic and/or team goals. May coordinate schedules and assign/delegate work to assure the provision of services. Responsible for management of clinical resources/supplies. Conducts and documents Quality Assurance monitoring activities to include review of medical records and clinic/team operations. Makes recommendations for improvement based on findings. Prepares/maintains program, statistical, and special reports as assigned. Participates in interdisciplinary teams to plan, develop, and assess appropriateness of patient care. Brief Job Description:
Under the direction of the nursing supervisor, Regional Nurse Manager, and the Regional Medical Director, provides routine (journey-level) public healthnursing work in clinical and non-clinical settings for the assessment, care, and treatment of individuals, families, and communities according to standing delegation orders. Utilizes the nursing process in assessing, planning, implementing, coordinating, delivering, and evaluating public health services in assigned counties. Services are focused on the promotion of optimum wellness and disease prevention such as immunizations against vaccine preventable diseases, and comprehensive case management for clients utilizing community resources and referrals to needed services. This position works with moderate supervision with limited latitude for the use of initiative and independent judgment. Must be able to independently travel throughout the State of Texas and out-of-state to complete essential job functions. Member of the Incident Command Structure in response to public health emergencies. Must comply with HHS, DSHS, Texas Department of Public Safety applicable policies/procedures/standards. This position is office-based, Monday-Friday, 8:00 am - 5:00 pm.
Essential Job Functions:
DIRECT SERVICES: Provides specialized and comprehensive public healthnursing services in the clinic, home, and community. The nurse will utilize components of the nursing process in assessing, planning, implementing, and evaluating the physical, emotional, and psychosocial health status of clients, families, and communities. Provides individualized care according to policies, procedures, program standards, physician orders and standing delegation orders which may include lab testing/counseling, independent home visits, providing/administering medications, education and counseling, case management, referrals, and follow-up. Performs disease intervention, surveillance, and prevention functions around Tuberculosis, HIV/STD, Perinatal Hepatitis B and Immunizations and other identified diseases. Prepares and maintains appropriate confidential client records per agency and HIPAA standards. (50%)
COORDINATION AND LEADERSHIP: May participate in assessing client and clinical needs, developing materials, and/or conducting education, orientation, and training for staff and contractors. May participate in quality assurance reviews and provide input for corrective action. Assesses and plans the delivery of direct public health services based on program guidance and standing delegation orders. Coordinates vaccine management to include inventory, vaccine ordering, reporting, receiving, and returning, and day to day cold chain management. May coordinate schedules and assign/delegate work to assure the provision of services. Responsible for management of clinical resources/supplies. Prepares/maintains monthly activity reports. (25%)
POPULATION-BASED SERVICES: Provides Essential Public Health Services to assigned communities. Participates in community assessments, identifies health needs, plans and implements strategies, and evaluates outcomes in conjunction with community stakeholders/partners and regional work plans. Develops and implements community health education programs, establishes and maintains communication and partnerships with stakeholders including local/county officials, community leaders, other agencies, groups, healthcare providers, schools, and the general public. Provides consultation, training, and technical assistance to healthcare providers, academia, and the community. Represents DSHS on local coalitions/collaborations as needed or assigned. (10%)
REGIONAL SUPPORT AND DISASTER RESPONSE/RECOVERY: Participates in regional assignments/duties/special initiatives. May be required to work other than normal business hours in order to respond to public health emergencies; actively participate and/or serve in a supporting role to meet the agency's obligations for disaster response/recovery/Continuity of Operations (COOP) activation-requires completion of at least ICS 100, 200, 700, and 800 and may require an alternative shift pattern assignment
and/or location. Maintain compliance with DSHS Policy, Fleet Operations and Management & Procedures and Standards, and a current valid Texas Driver's License. (10%)
OTHER DUTIES AS ASSIGNED: Performs other duties as assigned including maintaining CPR certification. (5%)

Registrations, Licensure Requirements or Certifications:
Must be currently licensed to practice as a RegisteredNurse in the State of Texas or a state that recognizes reciprocity through the Enhanced Nurse Licensure Compact.
Must possess a valid Class C Texas Driver's license or equivalent from another state. Driving record will be verified with law enforcement to ensure compliance with TDSHS driving policy.

The Direct Care RN performs routine level nursing work providing for the assessment, care and treatment of individuals who reside in a State Supported Living Center (SSLC) setting. Evaluates, treats and documents healthcare needs and/or issues and reports needs and/or issues to the appropriate person(s). Essential Job Functions (EJFs):
Attends work on a regular basis and may be required to work a specific shift schedule or, at times, even a rotating schedule, extended shift and/or overtime in accordance with agency leave policy and performs other duties as assigned.
May serve in an on-call status on a rotating schedule as required to meet operational needs of the facility.
Performs routine nursing tasks and procedures in accordance with HHSC, SSLC and state regulations, such as tracheostomy care, gastrostomy/jejunostomy care, wound care, breathing treatments. Collects specimens for laboratory examination and conducts appropriate screening tests when requested by lab personnel.
Administers medications, immunizations, and treatments and observes patients for unusual symptoms and reactions. Makes rounds and/or home visits as needed and works in other locations at the SSLC as needed. Monitors individuals' weight, diet, feedings, and vital signs. Monitors for signs and symptoms of illness and uses appropriate nursing judgement to provide adequate care for the individual, including initiating acute care plans.
Assists in evaluating nursing intervention outcomes and provides input to other members of the healthcare team including recommendations for improvements. Instructs direct support professionals on how to monitor individuals after a treatment or procedure using instruction sheets such as that used for post-sedation monitoring of signs and symptoms.
Timely and accurately documents and reports pertinent treatments and observations in the individual's medical record (electronic health record or paper chart) in accordance with SSLC procedures and protocols. Prepares reports as required.
Assists with services provided by LVNs and/or Certified Medication Aides (CMA), as needed, and makes recommendations for improvements, including acclimating them to the individuals, their needs and preferences.
Assists LVNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Provides assistance with completing follow up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and med variances.
Provides support to Nurse Manager's including reviews and follow up on controlled drug counts, medication counts, emergency checklist logs, refrigerator temp logs, and glucose controls.
Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the Quality Assurance Nurse in the implementation of therapeutic nursing programs. Participates in facility Quality Assurance/Quality Improvement initiatives.
Responds to emergency situations to ensure safety of individuals. Follows safety and infection control procedures of the SSLC. Provides educational and safety training to SSLC staff upon request. Provides treatment to HHS employees injured in course and scope of employment.

The Nurse Quality Monitor III reports to the Quality Monitoring Nurse Manager. This position makes a significant contribution to MCS's mission by conducting monitoring visits in Texas nursing facilities. The Nurse Quality Monitor III utilizes expertise in the area of nursing to assess conditions that could place facility residents' health, safety, and welfare at risk and assists providers in improving systems. This position must be able to apply the complex knowledge of nursing practice as well as the rules outlined in the Texas Nursing Practice Act. The Nurse III will be familiar with local, state, and federal laws and regulations relevant to long-term care.
The Quality Monitor Nurse (QMN) III works under limited supervision, with considerable latitude for the use of initiative and independent judgment. Performs highly complex (senior-level) nursing work. The Quality Monitor Nurse utilizes expertise in the area of nursing to assess conditions that could place facility residents' health, safety, and welfare at risk and assist providers in improving systems. Work may include serving as a resource on nursing practices and education programs by performing consultative and administrative work. May serve as a mentor providing direction to others.
Essential Job Functions
* Attends work on a regular and predictable schedule in accordance with agency leave policy and performs other duties as assigned.
* Works collaboratively across MCS to identify innovative and effective solutions. Maintains cooperative and collaborative relationships with colleagues and facility staff. Performs other work as assigned by Nurse Manager III or Nurse Program Manager V.
* Coordinates and/or conducts various types of visits to nursing facilities; including quality monitoring visits, rapid response team visits, mentoring visits and in-service visits. Acts as a team coordinator on Rapid Response Team visits offering leadership and direction to team members. Monitors and evaluates facility systems through observations, interviews, and record reviews identifying facility strengths and weakness making recommendations on opportunities for improvement. Instructs, counsels, and assists nursing facilities to achieve best practice. Reviews facilities policy/procedures and performance reports to determine if the facility's systems are consistent with evidence-based best practice. (50%)
* Documents accurate and complete reports of monitoring visits. Communicates accurate information, findings, recommendations, and technical assistance to the facility administrator and staff as well as quality monitoring leadership. Disseminates and teaches from appropriate education tools and materials. (20%)
* Communicates with various audiences (internally or externally) to provide clinical information, answer inquiries, address issues, and/or resolve problems. Maintains current knowledge in assigned areas by attending unit meetings, seminars, or other training and by continually reviewing program policies, procedures, and any changes. May conduct literature reviews and analyze complex information to update nursing-related evidence-based best practices technical assistance material. (10%)
* Develops and provides in-service education and other training to facility-licensed and non-licensed staff regarding evidence-based best practices. (10%)
* May train and mentor others in the context of a quality monitoring team visit, and orientation to peers as needed. May assists with the development and updating of program policy/procedures, evidence-based best practice standards, and educational programs. (10%)
Registration, Certification, or Licensure
* Must be licensed as a RegisteredNurse in the State of Texas or a state that recognizes reciprocity through the Nurse Licensure Compact and a current Texas driver's license.

Initial Screening Criteria
* Graduation with a bachelor's degree from an accredited four-year college or university with major coursework in nursing preferred. An associate in nursing with a four-year degree in an applicable healthcare-related field will be considered. Experience may be substituted for education on a year-for-year basis with a maximum of four years.
* A minimum of 3 years of nursing experience is required.
* Experience in mentoring peers is required.
* Experience with or knowledge of Long-Term Care Nursing Facilities with knowledge of CMS requirements or a SMQT (surveyor minimum qualifications test) is preferred.

Nursing Facilities Nurse Surveyor - Starting Market Rate Salary $89,690.71
As part of a collaborative team of public servants, a nurse III protects residents' health and safety by conducting on-site inspections and investigations in Texas long-term care settings (nursing homes, assisted living facilities, and day activities health services) to assess their compliance with state and federal laws and rules. This position offers a competitive starting salary; excellent state benefits, including health insurance and retirement; and the opportunity to regularly travel to different facilities within a region and across the state - Please see the Additional Information section below for more detail on travel. In this front-line field position, a surveyor is mobile and flexible and performs minimal work at an office, though the role does require writing reports detailing any deficiencies/violations against a facility and narrative to support regulatory determinations. Assists in evaluating nursing intervention outcomes and provides input to other members of the healthcare team including recommendations for improvements. Instructs direct support professionals on how to monitor individuals after a treatment or procedure using instruction sheets such as that used for post-sedation monitoring of signs and symptoms.
Timely and accurately documents and reports pertinent treatments and observations in the individual's medical record (electronic health record or paper chart) in accordance with SSLC procedures and protocols. Prepares reports as required.
Assists with services provided by LVNs and/or Certified Medication Aides (CMA), as needed, and makes recommendations for improvements, including acclimating them to the individuals, their needs and preferences.
Assists LVNs with implementation of protocols, procedures and programs to improve overall health and safety of staff and individuals. Provides assistance with completing follow up assessments, guidelines, acute care plan (ACP) interventions, injury reports, restraints, and med variances.
Provides support to Nurse Manager's including reviews and follow up on controlled drug counts, medication counts, emergency checklist logs, refrigerator temp logs, and glucose controls.
Assists the Infection Control nurse by implementing measures that prevent the spread of disease, assists the Quality Assurance Nurse in the implementation of therapeutic nursing programs. Participates in facility Quality Assurance/Quality Improvement initiatives.
Responds to emergency situations to ensure safety of individuals. Follows safety and infection control procedures of the SSLC. Provides educational and safety training to SSLC staff upon request. Provides treatment to HHS employees injured in course and scope of employment.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation.
