Brief Job Decription:
Under the supervision of the Director of Quality Management, the Program Specialist I in Quality Management (QM) assists in coordinating the compliance, evaluation, and continuous improvement of healthcare services and programs within the hospital. This position supports hospital compliance with Joint Commission, Centers for Medicare & Medicaid Services (CMS), and applicable state and departmental regulatory standards.
Primary responsibilities include researching regulatory standards, developing and maintaining hospital policies and procedures, conducting routine audits and mock tracers, supporting mock survey activities, and assisting with the development and monitoring of Plans of Correction (POCs) for identified compliance gaps. The Program Specialist I works closely with the Quality Management team, hospital leadership, medical staff, and operational departments to support accreditation readiness and ongoing quality improvement efforts.
This position works under limited supervision with latitude for initiative and independent judgment within established guidelines.

HCS Surveyor - Starting Market Rate Salary $60,431.88
As part of a collaborative team of public servants, a surveyor protects individuals' health and safety by conducting on-site residential surveys, monitoring, and investigations of Home and Community-Based Services (HCS) Medicaid program providers to assess their compliance with state and federal laws and rules. These include investigations into allegations of abuse, neglect, and exploitation. This position offers a competitive starting salary; excellent state benefits, including health insurance and retirement; and the opportunity to regularly travel to different program providers within a region and across the state. In this front-line field position, a surveyor is mobile and flexible and performs minimal work at an office, though the role does require writing reports detailing deficiencies/violations against a provider, remedies to correct those deficiencies, and narrative to support regulatory determinations. Additional Information:

The Program Supervisor I (Assistant Supervisor) performs routine (journey-level) administrative and supervisory work within the State Hospital System security program. Prepares the security schedule and meets with staff on regular daily basis at shift change.
Reviews security incident reports for accuracy and timeliness. Reports will be sent to Security Director, Safety Risk Management personnel, other departments, and senior leadership as warranted. Archives the final report according to the incident title, date, and year in a database for future retrieval.
Assists with evaluating and developing processes, guidelines, and standards to achieve the overall organizational goals for the security department annually. Implements improved processes based on best practices.
Develops security department program procedures, policies, rules, and regulations in conjunction with Texas Private Security Bureau.
Prepares the security department budget requests by evaluating resources and requesting additional resources such as uniforms, vehicles, and supplies.
Prepares and updates the training and operational manuals, educational safety and security programs by revisions, suggestions, and evaluations. Takes an active role in the production and development of documentation for the Security manuals. Assists with developing training and at times conducts training for staff.
Conducts monthly and annual fire drills as required on campus buildings, including patient units. Documents the drill response on the fire drill form. Responds to alarms accordingly.
Conducts various types of investigations by actively initiating a preliminary investigation, including but not limited to, thefts, collisions, missing or damage to State property, and criminal mischief.
Provides bi-monthly training or technical assistance at pre-services training to include new employees in the form of fire drill response, safety response, handicap parking on campus-fire lanes, key security, traffic enforcement, wearing ID badges, and calling 911 vs. 2911.
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Child Protective Investigations (CPI) Investigators investigate claims of child abuse and neglect. They have the difficult task of figuring out what happened and predicting what will happen in the future. As part of a collaborative team of public servants, an Inspector V protects residents' health and safety by conducting on-site inspections and investigations in Texas long-term care facilities (licensed-only nursing homes, assisted living facilities, day activities and health services (DAHS) sites, DAHS-ISS providers, presribed pediatric extended care centers) to assess their compliance with state and federal laws and rules.

This position supports the APS Division Manager and aids and support to the APS Contract Unit housed throughout the state. Performs contract management work which involves coordinating, developing, and evaluating a broad range of contracts. Essential Job Functions:
* Conducts quality assurances checks to ensure contact management compliance within the APS Purchased Client Services unit by assuring the unit's contracts adhere to contracting guidelines, handbooks, rules, regulations, policies, and procedures.
* Develops and maintains the unit's resources through documenting and maintaining contract processes for the unit. Assists with developing APS contract guidelines and procedures. The Laundry/Sewing Room Worker III (Laundry Worker III) performs complex (senior-level) work and is responsible for overseeing the safe and efficient operations of commercial laundry equipment in a commercial laundry. The Laundry Worker III assists the Laundry Manager with overseeing and cross training other Laundry Workers, ensuring other Laundry Workers follow all laundry equipment procedures that are in place for safe laundry equipment operation. The Laundry Worker III reports all employee and equipment issues or problems to the Laundry Manager and Regional Laundry Director. The Laundry Worker III assists the Laundry Manager with planning, scheduling, facilitating, and recording the efficient and safe processing of linens. Work involves both light and heavy physical labor. Essential Job Functions (EJFs):

Assists the Laundry Manager with training, cross training, and assigning the work of other Laundry Workers. Informs and updates the Laundry Manager on training deficiencies. Ensures rotation of trainees so proficiency is maintained. Informs and updates Laundry Manager of other laundry worker personnel issues and achievements that need to be acknowledged. Informs and updates the Laundry Manager of scheduling problems and achievements. Ensures open communication with the Laundry Director, Laundry Manager, Laundry Maintenance, and other Laundry Workers.
Sorts, classifies, weighs, and records all soiled laundry in order to determine what laundry formula will be used. Ensures all carts are labeled with a tag for the Facility/Unit that is being sorted. Ensures sorting and loading are done one facility at a time. Ensures sorting conveyor stays full, changes out full sorting carts so sorting can continue without interruption, and helps stage the loads for the CBW, while keeping the sort area organized. Ensures dirty carts are taken to the cart washer after they have been emptied. Sorts all soiled laundry by Facility/Unit, type of material, texture, and item's soil content. Records all weights accurately by Facility/Unit on the Dirty Shipping Info form. Turns in completed papers to office daily. Removes any article soiled with feces, food, vomit or medicine, and places articles in the heavy soiled bin.
Operation of the Continuous Batch Washer (CBW) system. Ensures the proper weights are loaded into the CBW to ensure the safe and efficient processing of linens. Ensures linens are properly sorted prior to loading, selects the proper wash formula and Facility/Unit code according to the degree of soil and articles being washed, and that linens are cleaned to required specifications. Ensures proper weights, Facilities/Units and formulas are programmed for efficient laundry processing. Follows all operating procedures for CBW, press, shuttle, dryers, un-loader belt, and lint collector. Ensures proper chemicals are maintained and connected for efficient cleaning not letting any chemical barrel run empty. Checks the chemicals before running and during processing if needed for changing out of barrels. Troubleshoots and corrects faults at the CBW, press, shuttle, dryers, un-loader belt, and lint collector for maximum productivity and maintains an efficiency of the CBW at 84% or higher. Keeps Laundry Manager, Laundry Director, and Laundry Maintenance advised and up to date on all productivity problems and efficiency situations immediately.
Operation of the large dryer unload conveyor. Ensures a cart is ready at the unload conveyor before the light comes on signaling it is time to unload. Unloads the conveyor as soon as the light comes on signaling it is time to unload and dryer door closes. Checks the Multitrac System to see what facility is being processed and if there is a question, checks with the CBW operator. Makes labels with Facility/Unit name, item description, and tapes to all carts as they come off the unload conveyor. Regularly checks processed laundry for dryness, stains, smells, re-washing stained, and bad odor linens. Brings any questionable linen item to the attention of the Laundry Manager, Laundry Maintenance, and Laundry Director for direction. Sorts out items per dryer area procedures. Covers all carts. Assists with the cart washer, guiding the cart out, wiping out the bottom of all carts, and placing a discard towel in the bottom for pooling. Removes discard towel before putting any linens/clothing into the cart, and keeps all carts organized on the clean side. Monitors the Multitrac System in the dryer area to ensure proper drying and location of goods during processing.
Operation of commercial washer/extractors and dryers. Loads washer/extractors within the range of 90% to 100% of maximum capacity. Records all loads loaded/unloaded from the washers on the required paperwork. Unloads washer/extractors, separating material by drying requirements. Selects the proper wash formula according to the degree of soil and type of articles being washed. Loads dryer and sets the drying and cooling times and temperature for each load according to the type and texture of material. Unloads the dryer only when the load is cooled to below 120 degrees Fahrenheit. Dries re-dry items from the CBW and folding/worker program as needed.
Operation of the Feeder, Ironer, Folder, and Stacker. Ensures all procedures and daily preventive maintenance is completed before running machines. Observes all safety rules, standards, and warnings posted on the machines and covered during in-service training. Ensures the waxing temperature is met before waxing the Ironer. Makes labels with Facility/Unit name, item description, and tapes the label to all carts as they come off the unload conveyor. Neatly stacks all folded sheets in the cart. Ensures sheets with knots are not run through the ironer. Inspects each item being processed for damage, excessive wear or stains, removing items and placing them in the appropriate area. Brings any questionable linen item to the attention of the Laundry Manager, Laundry Maintenance, and Laundry Director for direction. Addresses all errors immediately when the alarm goes off, before running any more linens. Follows the shutdown procedures after running the sheets or before the end of the day. The Food Service Worker II performs routine (journey-level) food service work on a designated shift and rotation in the central kitchen and/or at a satellite kitchen/ serving area. Work involves preparing, portioning, distributing, and serving food for regular and therapeutic diets, and sanitation of equipment, work areas, dishes, utensils, pots, and pans. Other responsibilities include, but are not limited to, training other Food Service Workers; handling food in accordance with federal, state, and Hazard Analysis Critical Control Point (HACCP) guidelines; following kitchen and food safety procedures and practices. Works under moderate supervision, with limited latitude for the use of initiative and independent judgment.
Performs other duties as assigned. Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Operates state-owned motor vehicles such as Full-Size Van, Wheelchair Van, Paratransit/Small Bus, truck and/or car to transport ambulatory and non-ambulatory individuals to on and off-campus appointments and activities. Secures any cargo such as food and beverage coolers, clothing items, and/or medical supplies in accordance with the instructions provided. Complies with best practices for vehicle operation and safety such as obeying posted maximum and minimum speed limits. During periods of heavy traffic, inclement weather, low visibility, or other poor driving conditions, adjusts speed so that accidents will be avoided. Only uses electronic devices (such as cell phone) when the vehicle is safely parked and avoids other "distracted driving" practices in accordance with training. Follows agency protocols following an accident in accordance with the training received including immediate post-accident on-site action, notification of emergency personnel and/or law enforcement (if applicable) and SSLC administration and completing required reports.
Assists designated staff in the safe loading and unloading of passengers in accordance with their individual care plans and in a manner consistent with facility rules and regulations and using established methods and/or techniques (including lifting, carrying, an/or pushing). Ensures ambulatory passengers use safety restraints. Operates vehicle power lifts to safely load and unload wheelchairs of various sizes and configurations. Ensures tailgate lifts, lift-gates, and wheelchair lifts are kept in the closed position except during actual loading and unloading. Ensures lifts are used only for the purpose intended by the manufacturer and the rated capacity is not exceeded. Ensures the vehicle is not driven with the tailgate lift in the open position except when necessary for appropriate alignment when backing up to a loading area. Ensures that vehicle capacity is not exceeded. Ensures that all individual seat belts and the provided wheelchair securement devices are securely fastened before placing a vehicle in motion. Reports any issues with passenger compliance to the designated support staff.
Follows the assigned schedule for pick up and drop off to the scheduled activity and/or appointment(s). Determines trip routes and coordinates and pre- and post-trip tasks to ensure timely transport of passengers to the assigned activity our appointment whether on or off the SSLC campus. Type of trips includes vocational or work activities, medical appointments, family visits, and group active treatment activities or outings.
Maintains vehicles at optimal condition. Conducts safety checks of the assigned vehicle before departure including performing a vehicle "walk-around" visual inspection for tire inflation; glass, light, lens, and mirror condition; any visible fluid leaks; and visible body damage. Reports any issues in accordance with local policy and procedures. Performs basic vehicle maintenance such as checking tire pressure., fueling the vehicle and removing all litter when the vehicle is returned from use. Ensures vehicles are scheduled for general maintenance.
Ensures vehicle is parked safely. Before leaving a vehicle whether on- or off-campus, ensures that the transmission gear selector is in the proper position for parking, the parking brake is set, all switches are in the off or locked position, if so equipped, the keys are removed, all occupants have exited the vehicle and the windows are closed, and the doors are locked.
Prepares and maintains records and logs related to vehicle operations. After each trip, documents any vehicle problems and reports the need for minor vehicle maintenance and repairs to the designated person. Accurately completes required paperwork such as updating information on the Monthly Use Report to track and reconcile trip information. Enters and totals the number of trips, passengers, and stops in the correct areas of the form. If the Driver has a specific vehicle(s) assigned. ensures that the Monthly Use Report is turned in to the motor pool or other designated department/person by the third working day of each new month.
Provides guidance and on-the-job training to other drivers, in accordance with experience and competency, in the proper operation of vehicles to transport passengers, proper method to ensure passengers and wheelchairs are secured in the vehicle, the safe operation of lifts to load and unload wheelchairs, and techniques for maintaining an on-time schedule.
Maintains eligibility to operate state vehicles. Complies with state and local traffic laws as evidenced by no citations. Attends and passes required training including Defensive Driver Certification and Van Driver Training courses.
Other duties as assigned include but are not limited to actively participating and/or serving in a supporting role to meet the agency's obligations for disaster response and/or recovery or Continuity of Operations (COOP) activation. Such participation may require an alternate shift pattern assignment and/or location.

The Custodial Manager III performs highly advanced administrative and custodial supervisory work. Preferred Criteria
* Completion of coursework or a degree in Social Work, Criminal Justice, Sociology, Counseling, Education, or a related field.

* Prior experience in case management, investigations, law enforcement, or courtroom testimony related to protective services, social services, or family and adult welfare.

Additional Information:
This position may be filled at any level from an APS Specialist I to APS Specialist IV. Factors such as education and experience may be considered when establishing the starting salary; however, the starting salary for this position may not exceed the maximum of the pay group.

The APS Professional Education Program reimburses eligible staff for the cost of tuition and fees up to $10,500 per fiscal year. An applicant must be an APS employee and must have one year of full-time continuous APS employment before the date of the application is due. APS Investigator Specialists are mobile caseworkers. Mobile casework is not the same as working from home. APS Investigator Specialists spend the majority of their workday "in the field" conducting home visits to interview clients and alleged perpetrators. Although each workday may begin and end at home, APS Investigator Specialists are expected to be in the field making visits and engaging the community every day.

The Psychiatrist performs highly advanced psychiatric assessment work and reports directly to the State Hospital Medical Director. Work involves conducting psychiatric activities, monitoring trends, diagnosing, and treating State Hospital patients. Reviews and analyzes system data to ensure recovery plans are individualized and decisions are made in conjunction with the Recovery Team. Establish physician-patient relationship by direct face-to-face contact or telemedicine. For newly admitted individuals, contact must be established immediately upon admission. Treats each individual with the most effective medication and the most effective dosage. Reviews lab work, modifies recovery plans, and records deliberations and recommendations accurately and timely in the electronic health record.
Develops and implements psychiatric recovery plans documenting how supports and interventions are customized for the individuals' psychiatric needs as well as their strengths and preferences. Documents how recovery plans are person centered and least-restrictive interventions based on risk vs. benefit analysis and treatment history, and how psychiatric supports and interventions are integrated with the supports and interventions from other disciplines. Develops and organizes implementation timeframes and provides guidance to the recovery team, and direct care staff while following HHSC and local policies and procedures, TJC standards, CMS conditions of participation, TAC and other state statutes.
Reads and analyzes medical records, leads discussions in staff meetings, participates in conference calls concerning clinical psychiatric issues, and interacts with and makes recommendations to family members, guardians, and Legally Authorized Representatives (LAR) when requested, and obtains timely medication consents.
Ensures collaboration between psychiatry and neurology for dually diagnosed individuals. Provides Neurologist with necessary information and ensures that psychiatry notes reflect this collaboration.
Consults with other psychiatrists, medical staff, psychologists, and others to collaboratively develop psychiatric interventions, identify environmental factors that may impact psychiatric care, and participate in restraint reduction efforts by coordinating medication and behavior therapy. Manages acute psychiatric care including authorizing emergency restraints and writing orders for the emergency use of medication.
Reviews and monitors system data and trends related to the quality of psychiatric care provided at the State Hospital. Addresses issues identified through monitoring of data and presents changes to the recovery plan. Attends meetings and chairs discussions to inform and create buy-in to necessary changes and follows through on hospital performance improvement plans. Participates in desk and onsite audits by reviewing records and applying audit tools.
Maintains open and regular communication with the Medical Director, Superintendent, and other staff to ensure the transfer of needed information for the continuity of services. Participates in various Medical Staff Committees and Sub-committees (Utilization Management, Infection Control, Pharmacy and Therapeutics, etc.) designed to ensure patients are receiving quality care.

Ability to participate in annual training as well as any other required specific training in order to assure up to date skills and knowledge in required areas such as CPR and safe use of restraints.

Registrations, Licensure Requirements or Certifications:
Applicant must have or be able to obtain an unrestricted Texas medical license and be board certified or board eligible (completed residency in Psychiatry) prior to start date.
Initial Screening Criteria:
At least three years' experience working as a psychiatrist.
Twelve (12) months experience working with people with psychiatric disorders is preferred.
Applicants must be a Doctor of Medicine (M.D.) or Doctor of Osteopathic Medicine (D.O.)

Applicants must pass a fingerprint criminal background check, pre-employment drug screen, and registry checks including the Client Abuse/Neglect Reporting System (CANRS), Nurse Aid, Medication Aide and Employee Misconduct, List of Excluded Individuals/Entities (LEIE). Flexibility in work hours may be required for this position. The position may be required to work overtime and/or extended hours.

According to the Centers for Disease Control and Prevention, healthcare workers are considered to be at significant risk for acquiring or transmitting hepatitis B, measles, mumps, rubella, varicella (chicken pox), tetanus, diphtheria, pertussis (whooping cough), and influenza. All these diseases are vaccine preventable. As a result, state hospital policy requires employees be vaccinated according to their level of contact with individuals. In the event you choose to not be immunized for the influenza virus, you may be required to wear a mask and take other protective measures.
