    The role of the Planning Supports Specialist is to provide quality planning and technical support, as it relates to the Person-Centered Planning (PCP) process and Long-Term Services and Supports (LTSS) billing. Additionally, this role provides basic case management oversight as it relates to information contained in ICM and communications with teams. Manage and follow the PCP process for people who receive community living, meaningful day, CDS, employment, supported living, or other support services from The Arc of Howard. Review the draft and final person-centered plans for accuracy as it relates to the person. Plan high-quality, individually tailored goals and provide resources, outcomes, goals, and completion for the people supported. Lead and train agency staff to support person-centered initiatives, plans, outcomes, and goals. Manage PCP scheduling and ensure adequate representation of The Arc staff and coordinating with the Coordinators of Community Services. Work with teams and the person supported to explore all waivered services to ensure needs and desires are met. Ensure individuals are supported in living the life of their choosing through their Person-Centered Plan (PCP). Maintain and monitor files for those we support for compliance with regulatory agencies. Maintain up-to-date records and documents in the Electronic Health Records, iCare Manager (ICM) and program files including the PCP documents, entry of outcomes and goals, face sheet, behavior support plan, tracking charts and other documents relevant to the person and their services. Monitor existing benefits and assist the Benefits Coordinator in the submission of documentation for eligibility paperwork (i.e. SSI, SSA, Medicaid Waiver, Medical Assistance, food stamps, HUD). Maintain regular contact with all members of the team in support of the person. Maintain bi-monthly contact with all persons supported. In-person visits are a priority. Qualifications A bachelor's degree in a relevant field is required. Three years of experience in the field of IDD is preferred. Person-Centered planning development and implementation experience is strongly preferred. Knowledge of Code of Maryland Regulations (COMAR) & Home and Community-Based Services (HCBS) Final Rule, waiver and service definitions and regulations, guidelines, procedures & philosophy.

    A Community Living Program Manager (PM) oversees multiple homes for The Arc of Howard. Each home overseen will have a Direct Support Manager (DSM) who will work closely with the PM on all facets of the day-to-day operation of the homes. Some of those tasks include recruitment, staffing issues, conflict resolution, and logistics. The PM is part of a larger team that works as part of the support network for people who receive services from The Arc. The PM will be required to communicate with other stakeholders regarding the services including, but not limited to, the person's family, medical personnel, and the Planning Support Specialist Essential Functions Supervision of DSMs: Annual performance evaluations for DSMs Attend and fully participate in weekly 1:1 meeting with DSM and monthly house meetings Along with the DSM: reviews and approves documentation in iCare Manager (ICM), monitors Paycom daily for timecard corrections, leave requests, and applicants, plan and schedule activities for the person we support, including booking vacations. Maintains records under policy and State and Federal regulations and ensures uploads of documentation including but not limited to doctor appointments, Person-Centered Plans, Behavior Plans, Nursing Plans of Care, Nursing Assessments, and MARs. Responsible for maintaining communications with internal and external partners regarding day-to-day operations Review and submit payroll in accordance with the required deadline Oversees operating monies (personal use funds, and supplies) to appropriate staff and reviews receipts to ensure that purchases are being charged to appropriate budgets. Assists the DSM with keeping an inventory of the house and the person's belongings. Provides supervision on periodic weekend and evening hours in conjunction with the RPM team. Has a full understanding of the hours associated with the house and people in the house and approves the DSM's monthly staff schedule and submits to the Assistant Director. Reports/monitors incidents, injuries, worker's compensation, terminations, etc. on the required forms promptly to the appropriate department (QA, HR, etc.). Conducts Quarterly site visits and informs the Operations Department if there are any needs that should be addressed. Reviews emergency drills and follows up on identified issues. Interview and hire potential DSMs. Coaching/mentoring DSMs in all facets of management; serve as a support to DSM including but not limited to performing as a med tech and assisting with staffing crises. Work closely with Directors and Administrators on interviews and tours to prospective persons served, their families, guardians, and coordinators of community services.     The Direct Support Professional (DSP) will play a vital role in supporting Persons with intellectual and developmental disabilities to live fulfilling, independent lives in their community. Reporting to the Program Supervisor and working closely with the interdisciplinary team, the DSP will provide person-centered support while promoting dignity, choice, and self-determination. The DSP will assist Persons in developing life skills, accessing community resources, and achieving their personal goals as outlined in their Person Support Plans (ISP). Position Responsibilities: Essential Functions Provide Personalized support to promote independence and community integration. Assist Persons with daily living skills including personal care, household management, medication administration, and community navigation. Support Persons in developing and maintaining relationships, recreational activities, and community connections. Implement person-centered planning strategies to help Persons achieve their goals and preferences. Facilitate access to community resources, services, and activities based on Person interests and needs. Document daily activities, progress notes, and incident reports in accordance with regulatory requirements. Collaborate with families, case managers, and other professionals to ensure coordinated care Ensure health, safety, and rights of Persons served. Monitor and support Persons' physical and emotional well-being, recognizing signs of illness or distress. Facilitate opportunities for Persons' to observe their religious or personal beliefs in a way that is meaningful to them. May be required to Administer medications following proper protocols and maintain accurate medication records in the electronic record. Implement behavioral support plans and crisis intervention techniques as needed. Ensure all safety protocols are followed and report any safety concerns immediately.
