Provides psychosocial services to patients treated by the facility including in-center and home dialysis patients (if applicable) utilizing Social Work Theory of Human Behavior and accepted methods of social work practice. In collaboration with the interdisciplinary team, informs, educates and supports staff in understanding the emotional, psychological and behavioral impact of Chronic Kidney Disease on the patient and family to ensure comprehensive quality care of our patients. PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Assessment / Care Planning / Counseling
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
Utilizes FKC patient education programs, established social work theory and methods, social work focused interventions, and quality of life measurement instruments as part of assessment and care planning to address barriers and meet patient treatment goals.
Provides monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.
Provides supportive counseling services to patients as permitted within the scope of their clinical training and state license.
Provides educational and goal directed counseling to patients who are seeking transplant.
Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes.
Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.
In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient and/or family understand and make an informed decision about their care.
Knowledgeable of and adheres to FMCNA Social Work Policy and Measuring Patient Physical and Mental Function Policy, including documentation.
Documents based on MSW interaction and interventions provided to patient and/or family.
Quality
Provides psychosocial support and/or Social Work Focused Interventions to address non-adherence, quality outcome, and quality of life concerns for all patients based on acuity level.
Participates in monthly Quality review meetings with the interdisciplinary team. Reports on quality indicators related to adherence, such as Missed and Shortened Treatments, Quality of Life Trends, and Service Recovery.
Patient Education
Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style, communication barriers, and needs.
With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Patient Admission and Continuity of Care
Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.
Understands the referral and admission process and supports the clinic in regard to the patient needs for scheduling to maximize adherence and adjustment.
The Social Worker will interview the patient to identify root causes or concerns for the discharge request, (i.e. transfer to hospice, relocation, dissatisfaction with services or staff) and share causes/concerns with operational leadership.
Insurance and Financial Assistance
Collaborates and functions as a liaison for patient with Insurance Coordinators to address issues related to insurance.
In collaboration with Insurance Coordinators, provides information and education to patients about payment to dialysis (federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs).
Collaborates with the Insurance Coordinator of any changes to patient state that impacts insurance i.e. transplantation, discharged, loss of coverage, or extended travel.
Refers patients to patient billing solutions (PBS) department for questions/concerns in regard to treatment related bills
Staff Related
Assists with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager.
Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources).
Provides training to staff pertaining to psychosocial topics as needed.
Contributes and participates with weekly team huddles. Discusses any urgent patient issues with staff.
Adheres to work defined caseload guidelines based on state regulatory requirements.
Performs other related duties as assigned.

Qualifications
Bachelor's degree in a clinical and/or administrative field or actively enrolled in an accredited bachelor's degree program with completing expected within two (2) years of assuming role.
Current State of Michigan Registered Nurse (RN) license.
Minimum of three (3) years of health care management experience required; five (5) years preferred.
A minimum of three (3) years of hospice/home health experience required; five (5) to seven (7) years' experience and clinical expertise in hospice preferred.
Certification in hospice nursing (CHPN) preferred.
Expertise in regulatory requirements and compliance as it pertains to hospice required.
Must be able to travel extensively through region, including overnight stays if needed. Frequently works variable hours/days; activities and workload may require extended days.
Excellent interpersonal skills with the ability to comfortably interact at all social levels and with colleagues from many different background, ethic origins and skill levels required.
Understands performance improvement and can communicate and operationalize performance improvement initiatives at the departmental and organizational level.

Essential Functions:
Leads the planning and execution of office moves, renovations, and space upgrades.
Serves as a collaborative partner with internal teams to meet space and facility needs; demonstrates empathy, neutrality, and professionalism in handling stakeholder concerns.
Manages space planning using AutoCAD, including creating accurate layouts and identifying construction needs. Develops and maintains floor plans and layouts using AutoCAD.
Develops and manages facility project timelines, budgets, and resource allocations.
Oversees the setup of utilities, janitorial services, and general maintenance vendors at new sites.
Sources, hires, and oversees contractors, movers, and service vendors; works with construction trades for office build-outs and renovations.
Conducts on-site measurements and space assessments.
Oversees furniture procurement, plans furniture layouts and orders furnishings based on budget and use-case.
Supervises the Property Management staff and actively coaches and/or mentors direct reports to ensure that a culture of accountability and responsibility is maintained.
Provides 24/7 on-call coverage across all offices, with the Property Management Team providing real-time support and feedback.
Conducts regular site visits to monitor progress and ensure standards are upheld; travels locally to multiple office locations each week, with monthly statewide travel based on the specific needs of each site/region.
Inventories assets and manages the system used for tracking.
Ensures all inspections and maintenance comply with all national and local laws, safety and building regulations, and CHAP accreditation.
Prepares facility management reports and project plans as needed and presents plans to the Executive Team for decision making.
Monitors the upkeep of the outdoor gardens at the Oak Valley headquarters site, and at any other owned properties with landscaping needs.
Oversees and is responsible for working with the Property Managers/Emergency Planner for the organization's emergency preparedness site plans and ensures annual safety training is conducted.
Ensures internal and external education, training and activities for self and staff to promote personal and professional growth and to ensure staff competency is maintained at all times.
Upholds organizational policies and procedures and all regulatory and legal requirements.

Three (3) or more years of supervisory experience is preferred.
Hands-on experience and proficiency in AutoCAD or similar software is required.
Strong knowledge of furniture systems, office design, and space utilization is required.

Position Summary
Under direct supervision by department leadership, performs phlebotomy collection of whole blood and apheresis products. Interacts with donors and coordinators, providing a high level of customer service to deliver an excellent donation experience. Takes pride in and is committed to delivering high quality services. Works cooperatively and is a supportive member of the team/group. Contributes positively to the achievement of team objectives. Responsible for operating the mobile drive vehicle and transporting the mobile staff in a safe and efficient manner.

Job Duties :
Prepares collection site, performs set up of equipment, including technology systems
With a valid driver's license, may operate mobile drive vehicles. Following all DOT guidelines, safely transports staff, supplies and equipment and arrives at site on time.
Performs accurate medical interviews to determine donor eligibility, performs venipunctures/blood draws, ensures donor safety, and re-books donors for future appointments
Observes and responds to donor questions, concerns, and reactions appropriately
Other duties as assigned

Responsibilities
Follows all DOT guidelines when operating full-size commercial van/Promaster, and safely transports staff, supplies, and equipment.
Travels to other local collection sites, as determined by business need.
Prepares collection site and performs set up, including technology systems (may include quality control).
Performs donor screening (collects donor histories), mini-physical, and phlebotomy.
Observes donors for reactions, provides reaction care and assists other staff in handling reactions if needed. May provide post-reaction care at mobile drives including assessing reactions for recovery, escorting donors, and serving as liaison to coordinate care with host organizations (e.g. high school nurse, drive coordinator).
Conducts automation (Alyx and/or Trima), including recognizing and responding to automation reactions. Recruits and converts donors for automation.
Rebooks donors for future donation appointments.
Understands and performs to all applicable regulatory and compliance requirements.
Recognizes when the customer is distressed and responds appropriately with tact. Creates a safe environment for donors and staff members.
Treats donors with customer service excellence, presenting a positive image. Responds to customer inquiries, requests, and complaints with a timely and complete response and escalates to leader when appropriate.
May prepare and pack units for transport.
Maintains product integrity and ensures proper identification of all donors, performs routine maintenance on equipment, and completes department records as needed.
Requires evening, weekend, and holiday hours.
Performs other duties as required or assigned which are reasonably within the scope of the duties in this job classification
