A LicensedPracticalNurse is responsible for:
Providing direct care to residents under individual plans of care, administering care and medications per doctor's orders.
Administer medications and/or treatment in accordance with physician's orders and monitors for effectiveness/response.
Document resident care using charting software.
Notify Nurse Manager, attending physicians, and family members of significant changes in a resident's condition, and take follow-up action, as necessary.
Work to coordinate, oversee, and provide guidance to NARs,
Display a positive attitude and respect for all residents, families, and staff.
Position available includes working every other weekend and every other holiday:
2:30 p.m. - 10:30 p.m. (5 shifts per two-week pay period)
Qualifications:
Degree obtained from an accredited nursing program.
Must be in good standing with the Minnesota Board of Nursing and have a current MN LPN license.
Minimum of one-year previous experience as a nurse is required, preferably in memory care setting.
Special knowledge, skills, and abilities:
Demonstrated knowledge of state and federal regulations
Excellent written, verbal, and interpersonal communication skills
Proven history of good judgement and integrity
High level of commitment to compassionate customer service
Ability to meet the physical demands of position, including lifting over 50 lbs. while transferring a resident.

SUMMARY
Under general supervision, LicensedPracticalNurses provide basic public school nursing services in the examination and treatment of patients; as well as promote, maintain and restore good health at Minneapolis Public Schools (MPS) facilities. Incumbents provide nursing care to students and participate in the health program for the general school population; including students with physically handicapping conditions. Additionally, incumbents administer medications to students by nebulizer, gastrostomy tube, or rectal valium; maintain current files of student emergency cards; assist with vision, hearing, and scoliosis screening of students; assist with daily routine care of students who have a physical handicap; and perform lab work to test blood sugar.
ESSENTIAL FUNCTIONS
Provides nursing services within scope of authority and training; duties may vary according to job assignment.
* Follows protocols, procedures and written care plans to administer medications and treatments; delivers first aid and episodic care under the delegation and guidance of the licensed school nurse.
* Utilizes physical assessment skills such as lung auscultation, monitoring of vital signs, and nursing observation skills to determine student health status and take appropriate action.
* Participates in plan-of-care for students with special or chronic health needs to include nursing care for medically fragile or technology dependent students.
* Provides ongoing observations and communication in regard to student's health condition and response to plan-of-care to the licensed school nurse, teacher and parent/guardian.
* Follows District and State protocols/procedures to ensure infection control practices are maintained.
* Follows communicable disease protocols/procedures including prevention, tracking and follow-up.
* Assists with vision, hearing and growth screenings following District and State guidelines as requested by the licensed school nurse or per parent/teacher request.
* Participates as a member of the Site Emergency Response Team to prepare for and respond to emergency situations.
* Maintains accurate and complete health records for students and enters data into the Electronic Pupil Health Record.
* Participates in special education time studies and appropriately logs IEP nursing services for reimbursement.
* Maintains inventory of first aid supplies and equipment; re-orders supplies as needed.
* Demonstrates courteous and cooperative behavior when interacting with students, clients, visitors, and MPS staff; acts in a manner that promotes a harmonious and effective workplace environment
* Enthusiastically promotes the Superintendent's goals and priorities in compliance with all policies and procedures.

MINIMUM QUALIFICATIONS
Education, Training and Experience Guidelines
Graduate of an accredited LicensedPracticalNurse program; AND two (2) years of experience as a LicensedPracticalNurse; OR an equivalent combination of education, training and experience as determined by Human Resources.

As a LicensedPracticalNurse in the State of Minnesota you will work directly under the supervision of Proctor Public School District - Licensed School Nurse. This individual will work in the nurse's office overseeing general student health needs at the Elementary level. Pediatric and Triage nursing will be a part of the daily routine. The nurse will be responsible for health data input in the online Power School program in order to maintain accurate and up-to-date health file records. Various educational programs will be taught to students throughout the school year. The LPN will assist the Licensed school nurse with hearing and vision screening and follow-ups and referrals.
Main Qualifications and Certificates:
Requires a minimum of two years (LPN)
Certified in CPR and First Aid.
Knowledgeable in diabetes, allergies, asthma, and seizure management.
Other Requirements:
In good physical health; able to lift 50-100 pounds seven to eight times/day from floor to waist height.
While performing the duties of this job, the employee is regularly required to reach with hands and arms and, simultaneously, talk and hear using a mobile communication device. (ie., cell phone, walkie talkie)
While performing the duties of this job, the employee is frequently required to stand and stoop, kneel, walk, bend and squat.
Assist Diabetic students with blood sugar monitoring and carbohydrate counting under the supervision of LSN, students Doctor and the parents/guardians.
Enjoys working with children and has experience with special needs children; to include children with Autism.
High energy level, enthusiastic, high standard of work ethics.
Ability to develop effective working relationships with students, and staff.
Ability to independently supervise students.
Strong independent thinking, time management, and priority setting skills.
Computer Proficiency
Assist school nurse with the management of MA health reimbursement.
Knowledge of principles, practices, and trends in nursing, health care, and child growth and development issues.
Experience working with health impaired, developmentally disabled, or emotionally involved children.
Develop, within 10 days, a general understanding of job duties and responsibilities.
Will demonstrate correct body mechanics during lifting or transferring, considering employee safety, as well as student safety.
Ability to multi-task and switch from one role to another with ease.
Ability to be an independent thinker with quick decision-making abilities.
Essential Duties and Responsibilities:
Provide routine first aid and care to ill and injured students.
Administer medications and treatments as ordered by student's physicians and as directed by the Licensed School Nurse.
Assist School Nurse with hearing and vision screening at the beginning of the year and as needed throughout the school year.
Observes and communicates student health issues with the licensed school nurse, school counselor, parents, teachers, & principal as needed.
Perform clerical and computer duties as assigned; to include monitoring student health files for immunization compliance. (as guided by MN state immunization laws)
Maintains and prepares student health records including health histories, immunization dates, and prescription orders from physicians.
Assist teachers as directed by the licensed school nurse and building principal.
Notify and consult with other professionals in the school regarding any concerns with a child's physical/emotional health condition.
Other health and educational duties as assigned and delegated by licensed school nurse according to IHP and IEP, or as assigned by building principal.
May need to perform special nursing procedures under the delegation/direction of the licensed school nurse and according to policies.
Perform duties in accordance with data privacy laws.

Position Title: ES Nurse
Reports To: Building Principal and LSN

Position is 7 hours per day, student contact days.
Job Goal: The nursing team will provide the best possible educational opportunity for each district student by minimizing absence due to illness and creating a climate to maintain optimal health and well-being for each student in the district.
Team Responsibilities:
The nursing team will respond to all student healthcare needs in the district and provide coverage when needed to all district buildings.
1. Administer nursing care, emergency care and first aid within the state laws of their license, to individual students and decide if an individual student should be sent home, parents/guardians contacted, ambulance called or if other professional help is needed and notify Licensed School Nurse supervisor.
2. Safely administer medications to a student with the appropriate form on file with appropriate signatures of a parent/guardian and/or physician per school policy/procedure.
3. Complete required daily and follow up documentation. This includes any student seen in health services, medications administered, accident report forms completed per procedure and updating/maintaining student health records.
4. Distribution and collaborated development with the LSN of pertinent health bulletins to students, parents/guardians and/or staff.
5. Monitor and refer students with health problems to parent/guardian for appropriate follow up with a health care provider and notify LSN supervisor.
6. Under the delegation of the LSN, work with the nursing team to provide early childhood screening, vision and hearing program, immunization program, maturation program and other classroom programs for students and parents. Under delegation of the LSN, and as trained and supervised, will provide referrals and follow-up for individual students with health concerns, including vision and hearing concerns.
7. Assign and monitor nursing tasks or activities within scope of practice to unlicensed assistive personnel.
8. Work and collaborate with the LSN/RN, as needed, with time sensitive health issues on a district level.
9. Maintain cumulative student health information files in each office.
10 .Maintain security of school health supplies and medications.
11. Order supplies as a team as needed for the offices.
12.Test and record monthly AED machine checks and take corrective action as appropriate. Will notify LSN of any AED needs.
13. Participate, under LSN delegation, with in-service training programs for school personnel and classroom education for district students. Provide health care information to individual teachers and students.
14. Maintain CPR certification and attend the mandated continuing education needed for nursinglicensure.
15. Collaborate with the nursing team on developing best practice school health policies/procedures. Participate with others in the formulation of health policy/procedure and curriculum development for the district.
16. Report all student emergencies to the building site principal and the LSN.
17. Implement policies/procedures of exclusion and readmission of students connected with infectious and communicable diseases in collaboration/communication with the LSN/district nurse.
18. Assist with gathering information for student health care plans in collaboration with RN. RN will write and sign care plans. Responsible for direct and indirect care given to an individual student in compliance with school district policies, professional standards of practice, physician's orders and the Individual Health Plan and the Individual Education Program.
19. Monitor and data enter immunization records for district students. Contact and collaborate with parents/guardians, building administrator and district nurse when there are non-compliance issues.
20. Monitor ongoing student health conditions within the scope of their nursinglicensure. Examples are asthma, seizure disorders, diabetes, etc. Inform and interpret plan of care (individual plan of care (IHP) and emergency care plan) as needed (based on legitimate right to know) to school personnel regarding deviations that may affect learning.
21. Follow all state and federal laws regarding data privacy information and child abuse/neglect reporting requirements.
22. Attend nurse's meetings and actively participates in decision making for the nursing department with a team approach. Exhibit an attitude that promotes teamwork in the nursing department. Represent self and nursing department in a professional and ethical manner at all times.
23. Is the coordinator of the Emergency Response Team for their building, under the leadership of LSN.
24. Complete required state reporting to the State of MN following departmental procedure.
25. Employee is responsible to be knowledgeable about all district policies. Takes professional responsibility for self-protection during required work tasks in potentially hazardous situations (ex: handling of blood and body fluids).
