Description and Responsibilities Open Sky is seeking an experienced Facilities leader to provide oversight of the strategic planning and day-to-day management of facilities operations for the agency's portfolio of approximately 100 properties. Properties include administrative offices, clinical sites, residential facilities, and day program locations. This role ensures that all facilities are safe, compliant, well-maintained, and support the agency's mission of delivering high-quality community health and human services. The Director of Facilities will provide leadership to facilities, maintenance, and vendor teams and oversees capital projects, manages budgets, ensures regulatory compliance, and collaborates with program leadership to meet operational needs of their programs. Other Key Responsibilities: Develop and implement a comprehensive facilities management strategy aligned with agency goals and growth. Description and Responsibilities The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection. The Enrollment and Care Coordination Specialist will screen indivdiuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback and corrective action to housing specialists and assign and track housing specialists caseloads in collaboration with the team lead, as needed. Other Key Responsibilities: Outreach to community providers to develop relationships and develop referral pathways. Verify insurance prior to enrollment and monitor insurance eligibility for each member. Obtain authorizations from the insurance provider for each member. Description and Responsibilities The DD Clinical Team is made up of services designed to create in-home and community-based services to support adults with Intellectual Developmental Disabilities and Autism Spectrum Disorder, in combination with mental health challenges. The goal of the clinical program is to maintain individual stability and effective functionality for people served in their homes, group homes, and out in the community by providing clinical services such as coaching supports, group therapy, and individual therapy using therapeutic skill-building interventions such as DBT-informed Skills System treatment model. Other Key Responsibilities: Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans. Assist individuals to access and/or provide transportation. Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
