Description and Responsibilities Open Sky supports adults with developmental and intellectual challenges who are referred by the Department of Developmental Services. Awake Overnight Counselors support individuals with a developmental disability and/or mental illness live as independently as possible, while providing guidance and performing various levels of assisted daily living skills. Counselors act as strong advocates for individuals served and understand the importance of community integration. Other Key Responsibilities: * Assist in implementing individualized service plans. * Assist with limited personal care, which includes direct care duties, occasional lifting, medication administration (with training), documentation, transportation, behavioral intervention, role modeling, and recreational activities. * Implement all procedures that ensure the health, safety, and well-being of individuals and general cleanliness and safety of the physical site. * Assist individuals to access and/or provide transportation. * Review and complete all required documentation. Riverside's Family After-School Program provides a structured, therapeutic after-school environment for children ages 6-12 in the Cambridge and Somerville communities who have experienced challenges such as neglect, homelessness, and trauma. The program offers clinically intensive, trauma-informed services that help children build coping skills, self-expression, and healthy peer relationships, while actively engaging families through regular events and collaboration. Description and Responsibilities Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky's talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served. Residential Counselors support individuals to live as independently as possible, while providing evidence-based treatment and care within a group living environment. They provide guidance, support, and perform various levels of activities of daily living. Through implementing procedures that ensure the health, safety, and well-being of individuals and general cleanliness and safety of the physical site, counselors ensure individuals are living a healthy, productive, and independent life. Counselors are strong advocates for individuals served, believe in the power of community, and acknowledge the importance of community integration! Other Key Responsibilities: Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans. Assist individuals to access and/or provide transportation. Implement activities that create opportunities for the development of valued roles and personal relationships in the community.
