Integrated Team Counselor - Mobile Adult Community Clinical Services (ACCS) Integrated Teams provide clinical interventions and peer and family support to facilitate engagement, support functioning and maximize symptom stabilization and self-management of individuals and to assist individuals in developing skills, establishing natural supports and resources to live successfully in the community. Working as part of the ACCS team under the guidance of the Team Leader, staff will provide services and interventions identified in the Treatment Plan, including education, supervision, and direct supports to allow individuals served to maintain their physical and emotional health, live safely in the community, and develop the skills necessary to meet their goals. Interactions will be guided by an appreciation for those being served as unique individuals and adults, with the expectation and goal of mutual learning and growth. In this vital role, the ITC will: * Provide input to person-centered assessment and service planning, and advocacy, coordination, and linkage with other services and the community. Along with other members of the team, provide rehabilitation interventions, including skills training in community environments. * Provide education, training, and physical intervention when needed to assist persons served in learning life skills including, budgeting, banking, transportation, housekeeping, personal hygiene, self-medication, nutrition and health * Model and teach skills for development of positive relationships, and negotiation of conflict. * Assist persons served with accessing necessary and appropriate health care * Maintain regular daily, monthly, and annual clinical records in keeping with DMH, agency, and division policies and procedures, and within required timelines. * Consistently perform all medication administration and documentation procedures in accordance with Riverside and State policies and regulations relating to the Medication Administration Program (MAP). Salary USD $22.07/Hr. Description and Responsibilities The Intensive Home-Based Therapeutic Care program provide services for youth with emotional challenges and/or multistressed families. The scope of services includes flexible individualized treatment, rehabilitation, supervision, and support that vary in intensity, depending on youth and family needs. The services are delivered by a core team of staff using a Wraparound Philosophy support which helps family, support members, and service providers work together to support the family or child. In addition, all services are delivered using a Trauma Informed Care approach. The Outreach Counselor will provide direct support weekly, and ongoing, to each assigned family and youth. The services will be tailored to meet the unique needs of each youth and family served, providing opportunity to maximize their ability to spend time together at home and in the community. They will also collaborate with the team to promote a culture in which each individual served, and their family member's point of view, experiences, and preferences are recognized, understood, and respected, and in which self-determination and decision-making in treatment planning are maximized and supported. Description and Responsibilities Open Sky's DDS Employment Services serve individuals ranging in age from 22-55 who receive employment and/or community-based day support. The goal of Employment Services is to support individuals to choose, acquire, and maintain a job of their choice, and/or develop a valued role in their community. Individuals may have unique safety, behavioral or other clinical challenges. Each program offers a staffing ratio that allows the individuals in their program to obtain support as well as gain and maintain a preferred job in the community. The Job Developer networks with community members and organizations that can share resources, provide employment opportunities and other valued community roles to individuals served in the Employment Services program. They also maintain active membership with business networking groups, Chambers of Commerce, and other community groups that will support the program's employment efforts and match skills and interests of people served to these community based opportunities. Other Key Responsibilities: * Negotiate and execute letters of agreement and contracts between Open Sky, employers and community organizations. * Assist with service facilitation including assessing, developing, and implementing Individuals' service plans, career plans, and behavioral support plans. * Maintain a data base of contacts and job development resources. * Monitor current job trends and labor market information in order to be responsive to current market conditions. Open Sky's Respite programs are short-term group living environments that provide treatment of intermittent crisis and stabilization for adults who are diagnosed with a mental health and/or substance use disorder.
