Description and Responsibilities Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, and in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky's talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served. The Deaf and Hard of Hearing Counselor will provide support to people who are either deaf or hard of hearing and are diagnosed with a mental illness and/or substance use disorder and live on their own, or with families, in the community. They will act as a liaison between Persons served, community agencies, and families to coordinate treatment including employment and education entities, Care Coordination Entities, Emergency Service Providers, and outpatient clinics. Outreach Counselors encourage and empower individuals to see beyond and live beyond perceived limitations by providing them with the tools required for success. Other Key Responsibilities: Deliver interventions identified on the Treatment Plan with supervision from licensed clinical staff. Implement activities that create opportunities for the development of valued roles and personal relationships in the community Provide side-by-side support and coaching to help individuals socialize. Conduct both 1:1 and group activities geared toward increasing Persons' readiness to engage in rehabilitation. Ensure individuals have adequate financial support through assisting with finding gainful employment or applying for entitlements. Participate in the On-Call rotation. Description and Responsibilities Open Sky's Acquired Brain Injury programs serve adults with acquired brain injury and physical or developmental disabilities who are referred by the Massachusetts Rehabilitation Commission. These programs provide individualized services to help participants live independently in their own apartments. Our services include finding accessible housing, coordinating healthcare, supervising PCAs and arranging for transportation when needed. The Awake Overnight Counselor provides guidance, support, and perform various levels of activities of daily living during the overnight hours. Through implementing procedures that ensure the health, safety, and well-being of individuals, counselors help individuals live a healthy, productive, and independent life. Counselors are strong advocates for individuals served, believe in the power of community, and acknowledge the importance of community integration! Other Key Responsibilities: * Provide support and direction for individuals requiring nighttime assistance. * Assist with limited personal care, which may include occasional lifting, medication administration (with training), documentation, transportation, behavioral intervention, role modeling, and recreational activities. * Assist individuals to access and/or provide transportation. * Review and complete all required documentation. Our mission is to provide homes that are safe, nurturing, and address the individual's personal needs and preferences. Every individual in an Open Sky residential program is encouraged to play an active role in community life, whether it be by volunteering, taking classes, participating in group outings, or joining a health club. Our dedicated staff ensures that individuals have the necessary supports and skills to build a successful and satisfying life in the community while enabling individuals to lead meaningful and active lives. The Asleep Overnight Counselor works with the Awake Overnight Counselor to support people with developmental disabilities during the overnight and morning hours. The Asleep Overnight counselor may sleep during the overnight hours, but is available in the event of crisis and assits in the morning hours.
