Description and Responsibilities Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, or in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Description and Responsibilities The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection. The Enrollment and Care Coordination Specialist will screen indivdiuals for CSP-HI, housing, nutrition program, navigation and transitional goods eligibility and will complete enrollment for individuals in the program including orientation to services, service agreements, consents, and data entry into the electronic health record software for each member. They also provide training, monitoring, feedback and corrective action to housing specialists and assign and track housing specialists caseloads in collaboration with the team lead, as needed. Other Key Responsibilities: Outreach to community providers to develop relationships and develop referral pathways. Verify insurance prior to enrollment and monitor insurance eligibility for each member. Obtain authorizations from the insurance provider for each member. Track the authorization expiration and obtain new authorizations. Participate in live review meetings with insurance company representatives for each member. The program is located in Whitinsville, MA at the Whitin Mill Complex. This is a respite service for families who are caring for their loved one with disabilities/Autism. The Activities Leader will facilitate a day of fun and social activities in the community, taking place on two Saturday's per month. They will promote a variety of inclusive, creative, recreational in-house and community-based activities to empower individuals to maximize their independence as well as teach and support the development of natural social, peer, and community relationships. Seeking Bachelor level interns to join our team as a Community Based Social Work Intern currently pursuing their degree in Social Work, Counseling, or related field preferred! Description and Responsibilities The DD Clinical Team is made up of services designed to create in-home and community-based services to support adults with Intellectual Developmental Disabilities and Autism Spectrum Disorder, in combination with mental health challenges. The goal of the clinical program is to maintain individual stability and effective functionality for people served in their homes, group homes, and out in the community by providing clinical services such as coaching supports, group therapy, and individual therapy using therapeutic skill-building interventions such as DBT-informed Skills System treatment model. In this role, the Clinician will provide mobile clinical case management support to individuals in the Central West area of Massachusetts- including Worcester County, North Central Area and the South Valley area. They will coordinate with other members of the treatment team, including family members, DDS, day programs, personal providers, and allied health providers, including the VNA. As a Relief Counselor, you gain experience and play a crucial role in supporting individuals with Acquired Brain Injury and Developmental Disabilities. Open Sky's Acquired Brain Injury programs serve adults with acquired brain injury and physical disabilities who are referred by the Massachusetts Rehabilitation Commission with the goal of providing individualized services to help adults live independently, in their own apartment. Services include finding accessible housing, coordinating healthcare, supervising PCAs and arranging for transportation. Our mission is to provide homes that are safe, nurturing, and address the individual's personal needs and preferences. Every individual in an Open Sky residential program is encouraged to play an active role in community life, whether it be by volunteering, taking classes, participating in group outings, or joining a health club. Our dedicated staff ensures that individuals have the necessary supports and skills to build a successful and satisfying life in the community and enable the individuals we serve to lead meaningful and active lives. Relief Counselors play a significant role in our mission to enhance the well-being of those we serve by providing direct support care, ensure daily living needs are met while maintaining a safe yet inspiring atmosphere. Other Key Responsibilities: Assist in developing individualized, person-centered treatment plans and play a lead role in the daily implementation plans.
