Description and Responsibilities Flexible Support Services are designed to assist youth between the ages of 6 and 22 and their families who live in Central MA. The overall goal is to assist children and their family members to develop skills, strategies and supports to sustain the child in the family's natural environment and to help the child successfully function in the community. This program supports each child's ongoing development of age-appropriate social, emotional, academic and pre-work skills. Services are highly flexible and individualized to meet the varying needs of children and their families. The Young Adult Peer Mentors use their lived experience to support young adults with setting and implementing goals in education, employment, housing, relationships, life skills and social needs. Mentors also help peers in identifying strengths and interests to assist in monitoring interventions and personal recovery goals. Description and Responsibilities Open Sky's Respite Programs are short-term group living environments that provide treatment of intermittent crisis and stabilization for adults who are diagnosed with a mental health and/or substance use disorder. The goal of the Respite program is to work collaboratively with individuals to develop skills to maintain their independence in the community and live a life that is not defined by their mental illness. The Residential Counselor supports individuals to live as independently as possible, while providing evidence-based treatment and care, within a group living environment. Counselors act as strong advocates for individuals served and understand the importance of community integration. Description and Responsibilities The DD Clinical Team is made up of services designed to create in-home and community-based services to support adults with Intellectual Developmental Disabilities and Autism Spectrum Disorder, in combination with mental health challenges. The goal of the clinical program is to maintain individual stability and effective functionality for people served in their homes, group homes, and out in the community by providing clinical services such as coaching supports, group therapy, and individual therapy using therapeutic skill-building interventions such as DBT-informed Skills System treatment model. In this role, the Clinician will provide mobile clinical case management support to individuals in the Central West area of Massachusetts- including Worcester County, North Central Area and the South Valley area. They will coordinate with other members of the treatment team, including family members, DDS, day programs, personal providers, and allied health providers, including the VNA. Other Key Responsibilities: * Provide training for the teams as needed to include IDD, Autism Spectrum Disorder, evidenced based and promising practices to include Trauma Informed Care and Positive Behavioral Supports. * Complete all required clinical assessments within 30 days of each individual's admission to ensure timely and focused treatment planning. Description and Responsibilities The Project North program is a model of care designed to assist MassHealth members who are Justice- Involved, either as current inmates or detainees, or on probation or parole. The goal of the program is to support individuals in navigating and successfully engaging with health care services, emphasizing Behavioral Health Services. The program is designed to improve health outcomes and decrease fatal overdoses and provides connections to health care and community services. This program serves individuals residing in Worcester and South County. The Navigator will provide In-Reach and Re-entry support, assessment, education, support and safety planning for individuals enrolled in the program. They serve as a point of contact for integrated care coordination and care transitions in collaboration with individuals enrolled, justice agencies, medical and behavioral health providers, pharmacies, various social service agencies and other agencies and supports as indicated. Other Key Responsibilities: Schedule and conduct outreach via in-reach visits within prescribed timeframes and engage eligible individuals by providing information about the program. Description and Responsibilities The Health Related Social Needs (HRSN) Program works in collaboration with MassHealth to address the immediate daily necessities that arise from the inequities caused by the social determinants of health, such as lack of access to basic resources including stable housing, a toxin-free environment, healthy food, heating and Internet access, transportation, healthcare, safety, education, employment, and social connection. The Housing Specialist operates within the Housing First Principles to set a foundation for pursuing access to daily necessities and meeting health goals with people served. The Housing Specialist works in collaboration with the individual served to assist in the development of a strength-based, solution-focused individualized goal and action plan that promotes permanent housing and self-sufficiency. Together, the individual and the Housing Specialist will determine the best housing match by securing rental subsidies through affordable housing programs, identifying ideal housing environments, and comparing to available housing. Other Key Responsibilities: Create and maintain a housing directory including contact information for all owners, developers, housing authorities, and other community organizations with available housing options. Assist with site visits, application, and lease negotiations. Establish and maintain mutually beneficial relationships with private landlords and developers to maximize housing opportunities for people served. Collaborate with landlord to ensure apartment is ready for occupancy, perform move-in/out inspection with person served and conduct safety and cleanliness inspections as needed. Description and Responsibilities Adult Community Clinical Services is a Department of Mental Health funded service that is designed to deliver evidence-based interventions within a clinically focused model. Individuals served are diagnosed with mental illness or dual diagnosis and reside in their own homes, apartments, and in group living environments. Our services focus on helping people transition to a more independent environment while maximizing their natural supports by building skills and achieving personal goals. The teams are tight knit, supportive and multidisciplinary. Open Sky's talented clinicians, substance use counselors, nurses, peer staff, housing counselors, outreach counselors, and direct care staff provide quality, compassionate care to the individuals served. The ACCS Housing Coordinator embraces and operates within the Housing First Principles to set a foundation for pursuing health and recovery goals with people served. The Activities Leader will facilitate a day of fun and social activities in the community, taking place on two Saturday's per month. They will promote a variety of inclusive, creative, recreational in-house and community-based activities to empower individuals to maximize their independence as well as teach and support the development of natural social, peer, and community relationships.
