The Family Health Center of Worcester / University of Massachusetts Department of Family Medicine and Community Health Primary Care Psychiatry Fellowship seeks to meet the growing need for improved access to outpatient psychiatric care in the United States, particularly among structurally-marginalized populations, through an intensive one-to-two-year training program (12 month, 18 month, and 24 months tracks available) based in an urban community health center with an affiliated academic medical center. This post-graduate fellowship program trains primary care providers to become leaders in the delivery of comprehensive, integrated, and holistic physical and mental health care in primary care settings serving structurally-marginalized urban communities by training them as clinicians, advocates, community health scholars, and teachers. We specifically seek to help fill the gap that exists between the current psychiatric model, wherein massive limitations exist in accessing quality care for most of the country, and the compassionate, collaborative, and equitable care that all patients deserve. We focus longitudinally on two main principles: 1) the structural and socioecological determinants of mental health, and 2) the impact of traumatic stress on the physical and psychological wellbeing of structurally-marginalized communities. Manages a database of patients requiring psychiatric consultation to ensure follow-up. Engages patients in health coaching and motivational interviewing. Provides patient advocacy and navigation for FHCW services and services in the community. Sets up room with appropriate equipment supplies for visit such as psychotherapy, pelvic exam, suturing, incision and drainage. Directs patients into the exam room in a timely fashion after registration to determine reason for visit. Completes medication reconciliations. Reviews information based on patient presentation and discusses appropriate medical concerns with nursing staff or the provider. Maintains and prepares note templates based on the purpose of the encounter. Maintains a repository of psychiatric clinical questionnaires, registration and intake forms, and consent documents in multiple languages and explains the purpose of all forms to the patient, emphasizing need for completion. Demonstrates and documents quality control measures - glucometer- and procedures for point of care devices Demonstrates and documents procedures for Infection Control for sterilization and cleaning. Responds to patient complaints and escalates those requiring further action as appropriate. Assists in a root cause analysis. The Staff Physician - Walk-In Center/Urgent Care will participate fully in the care of patients seen in the Walk-In Center for same day appointments and urgent care. Educational Requirements: Doctor of Medicine or Osteopathy. Experience Required: Family Practice Residency or Med/Peds Residency required; A minimum of two years of experience in treatment and care of all members of family unit required; experience in urgent care setting or emergency room preferred. Experience in the role of Primary Care Physician preferred. Professional Licensure/Special Skills and Certification Required: Board Certified in Family Medicine or Internal Medicine/Pediatrics (Med/Peds). Must have current licensure by Massachusetts Board of Registration for Medicine, current DEA/MA Controlled Substance Registration; BLS required. In this role, the Spanish Medical Interpreter will facilitate effective communication, provide context, and support comprehensive patient care within our primary care setting. The Community Health Worker/Patient Navigator (CHW) will work to outreach, identify, recruit, and support patients to engage in care and/or care management services; and will provide patient education and advocacy to assist patients with accessing services of FHCW and make and follow-up on referrals for external services. CHW provides HIV, HCV, STI's (Chlamydia, Gonorrhea, and Syphilis) and TB screening, prevention, counseling & testing, case management services to health center patients and community resident by performing venipuncture and collect urine. The CHW will provide patient education and advocacy to assist patients with accessing services of FHCW and appropriate, make and follow-up on referrals for extern services. The CHW also provides education on HIV, HCS, STI's and TB to the staff and clients as well as the Worcester Community via outreach to shelters and residential programs. The CHW must be available to do community outreach during off hours and on weekends when needed. The CHW will support the HIV team with patient follow-up reminders and will address barriers to care. The CHW will document all services as required and will assist in meeting all program requirements. Educational Requirements: High school diploma or GED required; Bachelor's degree in a related discipline preferred. Experience Required: Minimum of two years relevant work or college-level internship experience required. Data entry skills and proficiency with MS Office including Excel and Outlook. Proven ability required to perform patient interviews, coordinate and collaborate with internal services and external community partners. Must have demonstrated organizational skills and the ability to work independently. Must have a certification in phlebotomy or successful completion of training and certification within six months of placement in position. Bilingual in English and another language appropriate to FHCW patient population required. Community Health Worker Core Training required within first year of employment. The Assistant Director of Technology Services will support the Director of IT in leading the organization's technology operations, strategic planning, and team management. This role serves as second-in-command and often assumes Director responsibilities in their absence.
