The Family Health Center of Worcester / University of Massachusetts Department of Family Medicine and Community Health Primary Care Psychiatry Fellowship seeks to meet the growing need for improved access to outpatient psychiatric care in the United States, particularly among structurally-marginalized populations, through an intensive one-to-two-year training program (12 month, 18 month, and 24 months tracks available) based in an urban community health center with an affiliated academic medical center.
This post-graduate fellowship program trains primary care providers to become leaders in the delivery of comprehensive, integrated, and holistic physical and mental health care in primary care settings serving structurally-marginalized urban communities by training them as clinicians, advocates, community health scholars, and teachers. We specifically seek to help fill the gap that exists between the current psychiatric model, wherein massive limitations exist in accessing quality care for most of the country, and the compassionate, collaborative, and equitable care that all patients deserve. We focus longitudinally on two main principles: 1) the structural and socioecological determinants of mental health, and 2) the impact of traumatic stress on the physical and psychological wellbeing of structurally-marginalized communities.
Fellows perform a mix of outpatient clinical work, self and collaborative study, clinical case review, didactic learning, conference attendance, and teaching.
Fellows provide 5 sessions of direct patient care each week in the health center. After receiving training over the first few months of the fellowship, fellows also conduct psychiatric evaluations with patients internally referred to our team and provide curbside consultation services to PCPs in the practice.
Fellows have 4 sessions of educational time set aside for didactics, academic seminar, clinical case reviews, and self-directed study of educational modules created for the fellows.

The Triage RN is responsible for providing nursing care, in a family practice/urgent care setting. The Triage Nurse will adhere to evidence based nursing triage protocols, with the goal of providing each patient with the care he/she needs, at the time he/she needs it. The Triage Nurse will provide health care advice, and assist in the coordination of other health care services.
Program Specific Essential Duties and Responsibilities:
• Triages patients utilizing the appropriate “Lippincott Triage Manuals” for evidence based recommendations, providing triage, education & self-management support as needed.
• Utilizes care in communications to foster the patient's trust & confidence.
• Arranges for the use of Interpreter staff as required based on primary language needs.
• Demonstrates competent clinical/professional judgment, in addition to the use of “Lippincott” protocols.
• Sorts through multiple patient cues to focus on the primary patient complaint or issue.
• Assesses all patients using three specific criteria: 1.) clinical priority/acuity, 2.) the need for self-care guidance or guidance for the parent or guardian, 3.) the need for anticipatory guidance based upon the patient's health, behavior and developmental level. Assesses presenting signs/symptoms prioritizing urgency based on assessment and use of protocols.
• Follows-up on abnormal labs and other patient requests by communicating with Team Nurses and Providers to facilitate the appropriate care.
• Schedules appointments and arranges for immediate care through Primary Care, Same Day/Walk-In/Urgent Care, or the ER, as appropriate.
• Ensures that patient needs are met; documents patients understanding & agreement with plan, or if the patients is refusing the recommended triage advice.
• Follows protocol and documents in the patient's EHR, following the documentation policy.
Supervisory Responsibility:
May be asked to provide guidance to LPNs, medical assistants or non-clinical Call Center staff by providing direction with protocols to facilitate patient care.
Qualifications and Education Requirements:
Graduate of an accredited school of nursing; required BS in nursing preferred
Must be licensed as a RN by the Massachusetts State Board of Registration
BLS required
Two years full time or part time equivalent nursing experience, with at least one year of telephone triage experience in an ambulatory health care of family practice setting preferred.
Must have experience working in an EMR environment
Reports directly to the Medical Director of the Walk in Center

Program Specific Essential Duties and Responsibilities:
Serves as a resource for the entire psychiatric services team, particularly in regards to any specialized patient populations served by their provider.
Assists with special projects being developed by the psychiatric services team.
Attends regular team meeting with psychiatric providers to discuss high-risk cases and develop strategies for delivering patient-centered and trauma-informed care.
Serves as primary point of contact and care for patients between visits via direct telephone line provided to all patients.
Ensures all scheduling needs are met for their assigned provider(s).
Completes and documents clinical tasks at the request of the provider, including completing and tracking prior authorizations, sharing lab results, and making medication-related calls to patients or relevant third parties.
Tracks follow up and health maintenance needs for all patients on the assigned providers' panels to ensure proper care for at-risk populations.
Manages a database of patients requiring psychiatric consultation to ensure follow-up.
Engages patients in health coaching and motivational interviewing.
Provides patient advocacy and navigation for FHCW services and services in the community.
Sets up room with appropriate equipment supplies for visit such as psychotherapy, pelvic exam, suturing, incision and drainage.
Directs patients into the exam room in a timely fashion after registration to determine reason for visit.
Completes medication reconciliations.
Reviews information based on patient presentation and discusses appropriate medical concerns with nursing staff or the provider.
Maintains and prepares note templates based on the purpose of the encounter.
Maintains a repository of psychiatric clinical questionnaires, registration and intake forms, and consent documents in multiple languages and explains the purpose of all forms to the patient, emphasizing need for completion.
Demonstrates and documents quality control measures - glucometer- and procedures for point of care devices
Demonstrates and documents procedures for Infection Control for sterilization and cleaning.
Responds to patient complaints and escalates those requiring further action as appropriate.
Assists in a root cause analysis. Responsibilities may include - present the challenge - explain what occurred -suggest future recommendations
