What You'll Do
Provides occupation therapy services to patients according to a written physician's plan of care and as defined in the state Occupational Therapy Practice Act.
Develops treatment program and establishes goals for improved function. Communicates plan of care to referring physician and other Organization professionals as needed.
May teach new skills or retrain patient in once familiar daily activities that have been lost due to illness or injury, in accordance with our policy.
May fabricate splints and instruct patient in the use of various types of adaptive equipment to improve function.
May train patient in the use of prosthetic and/or orthotic devices.
Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient's condition. Submits these records in accordance with our policy.
Maintains contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care.
Qualifications
Graduate of an occupational therapy curriculum accredit ed jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Occupational Therapy Association.
Registered with the National Registration Examination of the American Occupational Therapy Association.
At least Two (2) years of clinical experience as an occupational therapist. (Preferred)

Job Requirements
Bachelor of Science Degree in Physical Therapy from an accredited institution

A Few Must-haves for This Position:
Currently licensed as a Physical Therapist in the state of Minnesota.
In the last five (5) years, two (2) years of experience as a Physical Therapist that is relevant to the patient population assigned preferred.
Able to travel within geographic area serviced by office. Valid drivers license, reliable transportation, and proof of auto insurance is required.
CPR/BLS certification
Some functions of what you will do as a Physical Therapist:
Assists the Physician or other licensed practitioner in evaluating the level of patient function and performance and equipment needs as well as in developing the resulting plan of care/treatment including appropriate long and short-term treatment goals, selection of interventions, and the intensity, frequency and duration of services to meet the needs of an individual patient.
Participates in the development and evaluation of the plan of care in partnership with the patient, representative (if any), and caregiver(s).
Regularly assesses changes in the patient's status since the last visit/treatment day, and determines whether the planned services should be modified, performing a re-evaluation and obtaining revision orders from the Physician as indicated.
Advises and consults with the family and/or other caregivers to promote the patient progress toward mutually agreed upon goals and planning for discharge.
Effectively communicates with other members of the interdisciplinary healthcare team providing care to promote the coordination of patient care and planning for discharge.
Completes and submits timely, legible, accurate, relevant and sufficient documentation

POSITION SUMMARY
The Physical Therapist is responsible for the evaluation, assessment and documentation of individual treatment plans. Considerable independence must be used along with the responsibility and accountability that exist in the interpretation of physicians' orders, patient assessment and the exercise of professional judgment.
DUTIES/RESPONSIBILITIES
1. Perform physical assessment and examination including range of motion, measure of strength, balance and coordination, posture, muscle performance, respiration and motor functions.
2. Provide treatments and procedures according to physician orders.
3. Determine patient's ability to be independent and reintegrate into general population (or the community or workplace) after injury or illness.
4. Develop treatment plans describing a treatment strategy.
5. Formulate short- and long-term goals and implement treatment plans to meet patient goals.
6. Document all patient interaction, treatment and outcome for quality patient care.
7. Maintain written records and reports as necessary.
8. Participate in staff meetings, department continuing education and other employee meetings.
9. Conform to Wexford Health Sources, Inc. and correctional regulations, as appropriate, to include, but not be limited to: dress code; schedule/time sheet; safety and security regulations
10. Maintains the confidentiality of inmate records
JOB REQUIREMENTS
The following requirements list the minimum to qualify. An equivalent combination of education and experience may be accepted.
LICENSURE: Current New Mexico Physical Therapist license
CERTIFICATION: Current CPR Certification; accredited by the American Physical Therapy Association's Commission on Accreditation of Physical Therapy Education (CAPTE) preferred
EDUCATION: Degree from an accredited Physical Therapist program
