Partnering with the Director of Laurelwood-Assisted Living, Nurse Case Manager, and the other RCA team members, the primary purpose of your job is to assist residents with their daily living activities and assist with personal care.
Overall Essential Functions & Responsibilities:
Resident Care:
+ Assists residents with personal care and activities of daily living (ADL's) as per Individual Service Plan.
+ Assists in observing overall health status of residents and informing the Director or designee of an concerns or issues.
+ Safeguards medical records of residents, maintains confidentiality and upholds resident rights.
+ Assists with recreational activities as needed.
+ Assists residents and staff during meal service.
+ Answers resident emergency call system promptly, evaluates the situation and resolves as needed, including contacting emergency personnel and/or physicians and families, and arranging for transportation for treatment as necessary.
+ Listens and responds to resident concerns and reports issues to the proper discipline.
Housekeeping:
+ Performs light housekeeping in apartments and common areas as needed.
+ May be asked to assist with linen changes and personal laundry weekly and as needed per care schedule.
Required qualifications- Education & Experience:
+ Must have a minimum of a high school diploma and/or GED equivalent.
+ Must have a passion for caring for seniors. RCA II's- Must also have prior medication administration knowledge and experience.
+ At least 1-2 years' experience working as a Care Giver, or Resident Aide in an Assisted Living, Memory Care, Home Care or Skill Nursing Setting and/or previous experience in a related field strongly preferred.
+ Certification as an HHA or PCA is not required but can be helpful.
Experienced base-pay!
RCA I- NE-Pay Range: $16.50hr-$23.50hr.
RCA II- NE-Pay Range- $17.00hr.$23.50hr.

Utilizes personal protective safety equipment and safe work habits.
III. Other Job Duties:
Performs daily rounds; as assigned, completes preventive maintenance on a monthly basis to include kitchen equipment, cleaning ice machine filters, etc.
Completes casualty prevention inspections to include checking fire extinguisher's monthly; sprinkler drop tests; emergency generator tests; provides on-call maintenance coverage as assigned, maintains tools and equipment; performs other duties as required.

Job Summary:
The incumbent in this position is responsible for providing twenty-four hour supervision to residents of a supervised living environment under the extensive advisement and consultation of the Site Supervisor. Duties are carried out in accordance with general work instructions and established practices, policies and procedures; however, the incumbent must make independent decisions concerning the safety and well-being of the residents. The incumbent is on call for the caseload of thirteen residents and urgent facility needs, sharing on call responsibilities with the Site Supervisor and Senior Counselor.
Nature and Scope:

Supervises resident medication(s) to ensure that residents medicate as prescribed. Monitors prescription expiration dates and orders refills to ensure resident medications are available in the unit. Encourages residents to gain skills necessary to medicate more independently. Consults with medical and mental health staff regarding both medical and psychiatric care.
Assesses resident level of functioning, skills and rehabilitative counseling needs in the area of daily living skills.
Teaches, assists and monitors self-care skills in the area of personal management. Assesses resident clothing and hygiene supply needs and coordinates the purchase of items.
Teaches, assists and monitors residents in the area of nutritional management. Coordinates nutritional needs and establishes resources as needed. Ensures that meal preparation is complete and kitchen areas are clean and safe.
Teaches, assists and monitors residents in the area of money management by providing basic budgeting skills. *Assists residents and staff during meal service.
*Answers resident emergency call system promptly, evaluates the situation and resolves as needed, including contacting emergency personnel and/or physicians and families, and arranging for transportation for treatment as necessary.
*Listens and responds to resident concerns and reports issues to the proper discipline.
Housekeeping:
* Performs light housekeeping in apartments and common areas as needed.
* Assists with linen changes and personal laundry weekly and as needed per care schedule.
Documentation:
* Assists the Director of Assisted Living, and RN Case Manager in planning, developing, organizing, implementing, and evaluating resident care services, in accordance with the current rules, regulations and guidelines that govern the Community.
*Completes all necessary documentation including resident care logs, chartable events, shift reports, incident reports and communication logs.
* Ensures the accurate transcription and implementation of all physicians' orders on their shift.
*Communicates with families, physicians and other health care providers regarding the needs and concerns of residents Performs other duties as assigned by Director of Assisted Living and/or their designee.
Qualifications
Education and Experience: Must possess a high school diploma or GED equivalent, and a valid NYS LPN license and three-five years of prior work experience in an Assisted Living and Memory Care community.
Full time, varied 7pm-7am shifts, 12hr shifts- including rotating weekends. *Generous shift differential offered!

RESIDENT COUNSELOR RESPONSIBILITIES:
The Resident Counselor is responsible for providing support to 13 individuals in a group home for residents 55+ years of age, with mental health diagnoses.
Teach residents problem-solving skills and development in the following areas: Nutritional Management, Money Management, Time Management, and Home Management.
Allow residents to express their needs and values without imposing personal values.
Assess the resident level of functioning, counseling needs, and skills in the area of daily living skills.
Formulate resident service plans, and identify goals, objectives, and plans of action.
Write plans that are behavioral in nature, clearly stated, and reflect principles of rehabilitation.
Meet with residents on a regular basis to monitor performance and develop new objectives and activities.
ATTRIBUTES OF SUCCESSFUL COUNSELORS:
High School Diploma or GED
Minimum age requirement of 18 years
Valid New York State Driver License
At least 6-months of experience working in Human Services or Mental Health Services
Strongly prefer a 2-year or 4-year degree in Human Services or a related field
