Patrick Lamar Wooten is director of Patient Navigation and Community Health Programs at Eskenazi Health. In this position, he provides leadership in a broad range of patient care activities, including assisting in the improvement of patient relations between staff and patient/families by identifying and eliminating barriers to patient care and access. He also provides directorial oversight of the Eskenazi Medical Center Community Health Center Social Workers, Healthy Families, Patient Navigation, RN, Population Health Coordinators, and Prenatal Care Coordination Programs.

He was featured as "Corporate Brass" in the 6th Edition of Who’s Who in Black Indianapolis and selected to represent Eskenazi Health in the 8th edition of the publication, which is the nation’s largest annual directory featuring influential leaders in the African-American community.

Patrick is a board member for the Central Indiana Affiliate of Susan G. Komen and serves as a community advisory board (CAB) member for Goodwill's Nurse Family Partnership. He is also an active affiliate of IU Simon Cancer Center-IUSCC Health Disparities Advisory Committee and serves as a critical link between IUSCC and Eskenazi by identifying effective strategies that focus on efforts of reducing cancer risks, early detection, accrual to clinical trials, and enhancing the survivorship and quality of life for cancer patients.

Patrick was the 2006 recipient of the Indiana Wesleyan University Outstanding Professional Achievement Award for exemplifying leadership, character, integrity, achievement, initiative, cooperation and enthusiasm. He graduated magna cum laude from the university with a Bachelors of Science in Business Management and summa cum laude with his Masters of Science in Management and Health Care Management. He earned two Certificates of Achievement in American Sign Language from the Indiana Association of the Deaf and currently serves as an affiliate of the Indiana School for the Deaf.
