MGMA Certification strongly preferred

Job Summary
As a Patient Service Representative, you will serve as the first point of contact for patients both in person and over the phone. Maintain cleanliness and organization of the patient waiting area
Provide coverage across JPCHC locations during staffing shortages
Participate in departmental and organizational meetings

Required Skills and Qualifications:

The HR Generalist reports to the HR Manager and supports HR operations across compensation, hiring, onboarding, training, and performance management.
Key Responsibilities
Recruitment & Onboarding
Partner with hiring managers and the Talent Acquisition team on all open requisitions from need identification through new hire orientation.
Assist with onboarding processes as needed and serve as a backup for New Hire Orientation.
Recruit, interview, and help select candidates for vacant positions (in collaboration with the Recruiter).
Employee Relations & Support
Respond to employee inquiries related to policies, procedures, and HR programs.
Investigate employee relations concerns and advise the HR Manager on appropriate resolutions.
Maintain a high level of integrity, professionalism, confidentiality, and compassion at all times.
Compliance & Policy Administration
Ensure compliance with federal, state, and local employment laws and regulations.

Job Summary
The Behavioral Health Consultant (BHC) works alongside Primary Care Providers and Specialty Medicine Providers, serving as an integral member of the medical team to address a broad spectrum of behavioral health needs within the clinic population. The BHC conducts brief biopsychosocial assessments, provides solution-focused, evidence-based interventions, and supports early identification, treatment, and prevention for as many patients as possible.
The BHC provides same-day consultative services, assists in managing chronic conditions, supports psychoeducation, and works closely with the care team to improve recognition, treatment, and management of psychosocial and medical conditions. The BHC also provides brief follow-up, triage, and referral to specialty mental health care when needed.
Essential Duties & Responsibilities (include but are not limited to):
Patient Care & Intervention
Provide behavioral health assessments, diagnostic impressions, and brief psychotherapeutic interventions.
Deliver same-day brief interventions for coping strategies, treatment compliance, and behavioral change.
Conduct risk assessments and support crisis intervention.
Provide brief follow-up, relapse prevention, and education.
Support treatment of chronic conditions (e.g., diabetes, insomnia, pain, medication adherence).
Proactive Identification & Care Coordination
Review/scrub provider schedules daily to identify patients with unmet behavioral health needs.
Proactively recommend same-day BHC consultations to medical teams.
Participate in daily huddles, rounding, and interdisciplinary team meetings.
Collaborate with enabling services and referral departments for case management needs.
Triaging and referring patients to specialty behavioral health when appropriate.
Offer ongoing consultation and monitoring for patients who require continuity services.
Integrated Team Collaboration
Provide timely and succinct feedback to medical providers with consultation findings and recommendations.
Assist physicians, advanced practice providers, and medical staff in managing overall patient health.
Work as a core member of the care team to build clinical pathways or best practice programs for targeted populations (e.g., chronic pain, diabetes, heart disease, obesity).

Deliver services in exam rooms when possible to reduce stigma and reinforce team-based care.
Education & Program Development

Job Summary
The Clinical Supervisor provides daily leadership and operational support for the practice site. In partnership with the Practice Manager, this role helps guide the site's clinical operations, fosters collaboration among staff, and ensures efficiency, compliance, and exceptional patient outcomes. The Clinical Supervisor is responsible for supervising clinical support staff, monitoring workflows, and serving as a patient advocate all while maintaining a culture of teamwork and continuous improvement.

Oversee daily clinical workflows, including staff assignments, patient scheduling, room assignments, medication and supply management, and lab testing

Provide training and mentorship for new employees and conduct competency assessments
Support inventory management, ordering, and oversight of clinical supplies and vaccines (including VFC program and CHIRP)

Participate in staff hiring, development, evaluation, and disciplinary processes
Serve as a liaison between clinical and non-clinical staff and providers

Patient Advocacy

Required Skills and Qualifications
Current state licensure as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) required

This role is part of the HF MCPHD grant and supports the delivery of high-quality, integrated care to underserved communities in non-traditional settings. The Nurse Practitioner will operate under JPCHC's clinical standards while delivering mobile and community-based services in alignment with grant objectives.
This position is expected to remain active through the duration of the grant period ending (enter date). Continuation beyond that date is contingent upon renewal or extension of grant funding and HRSA scope approval.
Job Responsibilities (include but are not limited to):
Provide direct patient care including physicals, immunizations, PCP visits, and referrals.
Deliver services at both the ROCK and CARE Centers, utilizing onsite nurse's stations and clinical resources.
Adhere to JPCHC's clinical standards, preventive care protocols, and wellness service guidelines.
Coordinate care across departments and refer patients for services not available on site.
Collaborate with other healthcare professionals to meet the unique needs of each patient.
Contribute to the success of grant-funded objectives and ensure compliance with all related requirements.
Participate in the provision of mobile and/or home-based services as defined by the grant.
Maintain thorough and timely clinical documentation in accordance with FQHC and HRSA standards.

Required Skills and Qualifications
Licensed Nurse Practitioner in the state of Indiana.
Certification by a nationally recognized NP certifying body (e.g., AANP, ANCC).
Minimum of one (1) year experience as a practicing NP, preferably in community or primary care.

We are currently seeking a Licensed Behavioral Health Therapist (LCSW, LMFT, or LMHC) to join our team working in a School base system. This position offers the chance to serve an established panel of patients in a medically underserved community while working alongside in-house wraparound support, including Outreach Specialists, Care Managers, and Patient Care Coordinators.
Job Summary
As a Licensed Behavioral Health Therapist, you will provide psychosocial assessments, therapeutic counseling, and crisis intervention for patients and families. You'll collaborate with an interdisciplinary healthcare team to ensure continuity of care, support smooth transitions, and promote quality patient outcomes. This role is ideal for a mission-driven therapist who thrives in a collaborative, community-based setting.
Job Responsibilities (include but are not limited to):
Conduct comprehensive psychosocial assessments and develop individualized treatment plans
Provide short-term therapeutic counseling and crisis intervention to patients and families
Collaborate with physicians, nurse practitioners, and the broader care team to ensure integrated care
Coordinate services and support smooth transitions across levels of care
Maintain accurate and timely clinical documentation in the Electronic Health Record (EHR)
Apply knowledge of protective service mandates for both adults and children
Utilize de-escalation and crisis intervention techniques when needed
Build and maintain strong relationships with community resources, medical teams, and protective services
Participate in team-based care meetings and continuous improvement initiatives
Required Skills and Qualifications
Master's Degree in Social Work, Counseling, or Marriage & Family Therapy
Active Indiana licensure as LCSW, LMFT, or LMHC (required)
CPI (Non-Violent Crisis Intervention) certification preferred
Relevant experience in healthcare, mental health, or community-based services
Strong knowledge of disease processes, human behavior, and principles of growth and development
Ability to provide effective therapeutic counseling and crisis intervention
Excellent interpersonal and communication skills; ability to collaborate in a team-based setting

We are currently seeking a Certified or Registered Medical Assistant (CMA or RMA) to join our team. This role is ideal for someone passionate about patient care, collaboration, and community health.
Job Summary
As a Certified or Registered Medical Assistant, you will support the clinical team by preparing patients for their visits, assisting providers with procedures, and performing key administrative and clinical tasks. You'll be an essential part of our efforts to ensure a smooth, supportive, and high-quality experience for every patient.
Job Responsibilities (include but are not limited to):
Room patients and collect vitals
Perform EKGs, administer injections, and assist with minor procedures
Coordinate referrals and manage triage communications
Prepare and stock exam rooms
Document patient information accurately in the Electronic Health Record (EHR)
Perform CLIA-waived lab tests, prepare specimens, and support VFC tracking
Maintain clinical compliance with OSHA, CLIA, and ADA standards
Required Skills and Qualifications
Certification or Registration as a Medical Assistant (CMA or RMA)
Graduation from an accredited ABHES or CAAHEP MA program
BLS Certification (American Heart Association)
High School Diploma or equivalent
Computer literacy and basic math skills
Strong interpersonal and communication skills
Experience in an ambulatory care setting preferred
Familiarity with CHIRP, VFC, and VOMS systems a plus
