The Department of Pharmacy, consistent with the mission of Johns Hopkins Medicine, places a priority on teaching, research and patient care. At Johns Hopkins Hospital, the department is responsible for providing acute care pharmacy services for a diverse population of medical and surgical patients, with additional services provided within the Johns Hopkins Outpatient Center and affiliated clinics. Within the department, there are 45 acute care and primary care clinical pharmacy specialists as well as 31 general and specialized pharmacy residents.
The Johns Hopkins Hospital is seeking a motivated individual to assume an Anticoagulation Clinical Pharmacy Specialist role. The individual will work collaboratively with members of the healthcare team, including physicians, advance practice providers, nurses, and other pharmacists to provide comprehensive medication management. The Clinical Pharmacy Specialist will be responsible for the provision and oversight of anticoagulation pharmacotherapy for patients in an ambulatory anticoagulation clinic (located at Johns Hopkins Bayview Medical Center) and in the Center for Perioperative Optimization (located at The Johns Hopkins Hospital). The specialist will also collaborate with the hemostatic and antithrombotic stewardship program and support transitions of care for patients.
The Ambulatory Clinical Pharmacy Specialist- Antilcoagulation will work with the multi-disciplinary team to develop therapeutic plans, monitor for desired outcomes, and provide patient education. The pharmacist has the opportunity for advanced practice via a drug therapy management agreement to initiate, modify, continue, and discontinue medication therapy.
The Clinical Pharmacy Specialist will also provide education to students, residents, pharmacists, and other health care practitioners and serve as a preceptor for pharmacy residents of ASHP-accredited PGY-1 pharmacy practice and PGY-2 specialty practice programs. Additionally, the Clinical Pharmacy Specialist will support and lead quality improvement and research projects and participate as an active member of hospital and departmental committees. Faculty appointments are available through affiliations with the University of Maryland College of Pharmacy, the University of Notre Dame of Maryland College of Pharmacy, and the Johns Hopkins University School of Medicine.
Preferred qualifications for this position include the completion of a PharmD degree, PGY-1 residency or equivalent experience, and PGY-2 specialty residency or equivalent experience in thrombosis and hemostasis management, ambulatory care, internal medicine, or pharmacotherapy. Board of Pharmacy Specialties (BPS) certification is required within two years of start date.

The JHH Adult Medicine, Emergency and Surgery (AMES) Pharmacy provides clinical pharmacy services and medication preparation and dispensing for all adult areas of the hospital and outpatient center. These are inclusive of cardiology, critical care, emergency medicine, medicine, neurology, oncology, peri-operative, psychiatry, surgery, and solid organ transplant services among others.
Responsibilities of the ClinicalPharmacist 1 (Night Shift) include but are not limited to providing clinical pharmacy services including medication order review and verification, therapeutic drug monitoring, providing recommendations to the multidisciplinary team, and emergency code response. They also support operations of the medication use process, including checking medications prior to dispensing them to the patient. They are also able to be involved in precepting, projects, and committee work.
Schedule: Night Shift - 7 days on / 7 days off (six 12-hour nights per week and one 8-hour night per week).

General Summary: As Clinical Staff Pharmacist, Internal Medicine you will:
Serves as an advanced pharmacist practitioner engaged in providing pharmaceutical care
Regularly monitors patient-specific therapies and collaborates with other health care team members.
Participates in quality-assurance activities, represents department on hospital-wide committees, and serves as a resource for other health care practitioners.
Actively participates as a preceptor for the Pharmacy Residency Program and for students
Primarily severs as the Internal Medicine Pharmacist, however, is available to help central pharmacy functions if needed
This position reports directly to the Clinical Pharmacy Practice Manager
EDUCATIONAL REQUIREMENTS/PREFERENCES:
Bachelor of Science Degree in Pharmacy from accredited school; PharmD preferred.
PGY-1 from an ASHP-accredited program is required
PGY-2 from an ASHP-accredited program preferred and/or 3-5 years of relevant clinical experience
Current Maryland Pharmacist license required.
2-4 years previous hospital pharmacy experience required. Advanced clinical certifications preferred. Advanced degree or years of experience may be accepted in lieu of residency.
Shift: Full-Time (40 hours per week), Rotating shifts including weekends & holidays.
Salary Range: Minimum 56.57/hour - Maximum 93.30/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility. The Oncology Clinical Pharmacy Specialist will be responsible for providing and overseeing pharmacotherapy for adult patients with hematologic malignancies in the ambulatory care setting. This position will also partner with other ambulatory hematologic malignancy clinical pharmacy specialists to provide cross-coverage and support to other oncology teams, as needed. The primary role of the Clinical Pharmacy Specialist is to work alongside the multi-disciplinary oncology care team to educate, monitor, and support patients throughout their oral anticancer therapy. The pharmacist also serves as a resource to the disease team regarding both oral and infusion anticancer therapy, toxicity management, appropriate dosing, and drug information.
As part of a multidisciplinary team, responsibilities will include direct and telemedicine-based encounters for the provision of education and initiation of oncologic therapies, follow up to monitor the initiation and side effects of oncologic therapies, medication reconciliation, and communication with Specialty Pharmacies.
The Clinical Pharmacy Specialist will also provide education to students, residents, pharmacists, and other health care practitioners and serve as a preceptor for pharmacy residents of ASHP-accredited PGY-1 pharmacy practice and PGY-2 specialty practice programs. Additionally, the Clinical Pharmacy Specialist will support research conducted by the institution and participate as an active member of hospital and departmental committees to assist with formulary management and policy development.
Minimum Qualifications:

This role involves rotating rounding with the Neill, Pediatric Oncology or Pediatric Hematopoietic Cell Transplant team to optimize complex pharmacotherapy regimens, performing medication order review and verification for general pediatrics and oncology patients. Additional responsibilities for the position include: comprehensive medication management, interdisciplinary decision making, emergency code response, care coordination, education, policy/procedure development, oversight of chemotherapy production and research activities. They will provide drug information and education to patients, students, residents, pharmacists, and other health care practitioners. The work schedule is typically dayshift Monday through Friday but will provide occasional weekend and evening coverage.

Job Summary:
This position serves in the Adult Medicine, Emergency, and Surgery Pharmacy Division which provides comprehensive pharmacy services to all adult patients at JHH with the exception of oncology patients. Applicants are preferred to have completed, or be in the process of completing, an ASHP accredited PGY1 residency program or have equivalent experience. This ClinicalPharmacist 1 is responsible for working collaboratively with multidisciplinary health care teams to promote the safe and effective use of medications during the medication order review and verification process as well as addressing clinical pharmacy questions and concerns. Additional responsibilities for the position include: medication management, emergency code response, care coordination, drug information response, and education to students, residents, pharmacists, other health care practitioners and patients and family members. There is not typically a routine rounding component to this position.
Qualifications:
Graduation from an accredited College of Pharmacy resulting in Pharm. D. or BS degree.
Completion of an ASHP accredited residency program or equivalent experience is desired.
Current licensure as a pharmacist in the State of Maryland required.
Hospital and departmental education requirements completed within 90 days of hire. ACLS and/or PALS certification is desired.
Previous clinical health-system experience is desired.
Salary Range: Minimum 51.02/hour - Maximum 84.26/hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

The ClinicalPharmacist II in General Pediatrics and Pediatric Oncology provides clinical pharmacy services for general pediatrics and pediatric oncology patients, serving as the drug expert helping to manage the complex pharmacotherapy regimen of each patient. This role involves rotating rounding with the Neill, Pediatric Oncology or Pediatric Hematopoietic Cell Transplant team to optimize complex pharmacotherapy regimens, performing medication order review and verification for general pediatrics and oncology patients. Additional responsibilities for the position include: comprehensive medication management, interdisciplinary decision making, emergency code response, care coordination, education, policy/procedure development, oversight of chemotherapy production and research activities. They will provide drug information and education to patients, students, residents, pharmacists, and other health care practitioners. The work schedule is typically dayshift Monday through Friday but will provide occasional weekend and evening coverage.

The Department of Pharmacy's mission is to provide high quality pharmaceutical care to our patients in an atmosphere of educational growth, shared respect and communication. We are currently seeking a Pharmacist Coordinator A to join our pharmacy team within the Weinberg Oncology Pharmacy (inpatient and ambulatory oncology infusion) as part of the Sidney Kimmel Comprehensive Cancer Center at The Johns Hopkins Hospital.
The Weinberg Oncology Pharmacy services 100 beds including medical oncology and hematologic malignancy service lines. The pharmacy also serves approximately 180-200 patients each weekday at the three ambulatory oncology infusion centers located at The Johns Hopkins Hospital and Green Spring Station.
The Pharmacist Coordinator A will review, assess, and verify complex medication orders (including chemotherapy), complete therapeutic drug monitoring (e.g., vancomycin, methotrexate, anticoagulation), and collaborate with the Oncology Pharmacy Clinical Specialists and interdisciplinary team. They will also provide oversight of the complex oncology pharmacy operations that supports 100 inpatient beds, ambulatory bone marrow transplant (BMT)/cellular therapy patients, and 3 ambulatory infusion clinics.
The pharmacist will serve as a preceptor for pharmacy students and residents of ASHP accredited PGY-1 pharmacy practice and PGY-2 specialty practice programs. Additionally, the successful candidate will participate in longitudinal operational projects, support drug research conducted by the institution, and seek to assume a leadership role in the profession of pharmacy.
