Position Summary:
The School-based Counselor/ Social Worker provides (1) early intervention therapy and counseling services in a designated school district, (2) case management and mental health support services at the school and in the homes of at-risk students, and (3) consultation services to school personnel, liaison support to community agencies, and training programs for school and community populations.
Duties & Responsibilities
SERVICES TO STUDENTS:
• Support students experiencing truancy by developing and implementing appropriate interventions and conducting home visits as needed.
• Provide case management for students and families requiring multiple resources.
• Provide home-based services to the families of students who have experienced or are at risk of experiencing trauma, including those who are low-income, homeless, involved in the child welfare system or involved in the juvenile justice system.
SERVICES TO SCHOOL PERSONNEL:
• Provide staff with essential information to better understand factors (cultural, societal, economic, familial, health, etc.) affecting a student's performance and behavior.
• Provide direct support and interventions to students exhibiting challenging behaviors
-Employ evidenced-based strategies and techniques to de-escalate behavioral incidents.
-Offer counseling and coaching to help students understand and manage their emotions and behaviors.
• Develop proactive strategies to prevent behavioral issues including classroom accommodations and modifications when necessary.
• Respond to crisis situations while working closely with school security and counseling teams.
SERVICES TO DISTRICTS:
• Identifying and reporting child abuse and neglect.
• Providing case management for students and families requiring multiple resources.
SERVICES TO PARENT/FAMILIES:
• Work with parents to facilitate their support in their children's school adjustment.
• Alleviate family stress to enable the child to function more effectively in school & community.
• Assist parents in accessing and utilizing school and community resources.
• Work with those problems in a child's living situation that affect the child's adjustment in school (home, school, and community).
SCHOOL-COMMUNITY LIAISON:
• Obtain and coordinate community resources to meet students' needs.
• Provide health services and intervention strategies by coordinating the services provided by eligible applicants and coordinated care organizations, public health entities, nonprofit youth service providers and community-based organizations.
• Foster and promote communication between the school entity, community and law enforcement.
• Increase access to quality trauma-informed support services and behavioral health care by linking the community with local trauma support and behavioral health systems.
• Help school districts receive adequate support from social and mental health agencies.
• Mobilize family, school, and community resources to enable the child to learn as effectively as possible in his or her educational program.

Reporting to the Associate Director, the Student Success Navigator, provides high-quality recruitment, intake, onboarding, orientation, and bridge experiences designed to get students off to a solid and supported start.
The Success Navigator will work with the entire City College Academy team, Enrollment Management, Educational Support Services, and other areas at the College to successfully take students from the application and enrollment process through orientation, bridge, and onboarding, providing a broad range of guided supports and services to the students. The Success Navigator also works with other members of the City College Academy team to support students as they near completion of their workforce pathway training and prepare to transition to the municipal employment process. This includes, but not limited to, ensuring the students meet all eligibility requirements for a seamless onboarding experience; helping students' complete application, supporting students in the financial aid process, identifying potential barriers to success and connecting them to supports and resources at the outset; connecting all students to their respective Success Coach and working with students throughout their time at the City College Academy to ensure student success and completion.

Specific Responsibilities
Serve as the primary liaison between incoming students and Enrollment Management, supporting students throughout the intake and onboarding process.

Actively participate in Student Assistance Team (SAT) meetings with City College Academy staff to regularly review student progress and identify and address the mitigation and removal of barriers impacting retention and completion.
In collaboration with the Office of Marketing and Communications, assist in the coordination of marketing materials and maintain the program website, ensuring all information is accurate and regularly updated.
Assist the Associate Director in creating and facilitating virtual and in-person events such as webinars, open houses, and incoming student success events.
Travel between the College's Main Campus and Regional Centers and the broader community to conduct outreach, ensuring all students are fully supported throughout the application, enrollment, intake, financial aid, and onboarding process.
Accurately document, maintain, and track all student interactions, information, progression, and outcomes throughout the onboarding, enrollment, and bridge processes.
Work with other members of the City College Academy team to design and facilitate bridges from training to employment as students approach completion of their workforce pathway training and prepare to for municipal employment.
Ensure confidentiality of students' records and information.

During a typical workday, counselors are required to stand approximately 30-35% of the day on flat surfaces. Reaching - Counselors may reach on frequent basis while performing typical job functions, primarily between the knee and shoulder levels.
Dexterity - Fine manipulation is a requirement of this position while operating a computer. Counselors must also demonstrate legible handwriting, which can be understood by students and adults. Specific vision abilities required include close vision such as to read handwritten or typed material, and the ability to adjust focus. The position requires the individual to meet multiple demands from several people and interact, both orally and in writing, with the public and other staff. The employee is directly responsible for safety and well being of students in their charge. The noise level in the work environment is moderate to loud.

Position Summary:
The School-Based Counselor will provide (1) early intervention therapy and counseling services in Coatesville school district, (2) case management and mental health support services at the school/schools and in the homes of at-risk students, and (3) consultation services to school personnel, liaison support to community agencies, in order to meet the requirements of the Coatesville School district's STOP School Violence Grant. SERVICES TO STUDENTS:
• Identify students in need of trauma support and to provide prevention, screening, referral and treatment services to students potentially in need of services.
• Provide trauma-informed therapy and counseling interventions (group, individual and/or family) for at-risk students.
• Provide mentoring and other intervention models to children and their families who have experienced trauma or are at risk of experiencing trauma, including those who are low-income, homeless, in foster care, involved in the criminal justice system, unemployed, experiencing a mental illness or substance abuse disorder or not enrolled in or at risk of dropping out of an educational institution.
• Provide case management for students and families requiring multiple resources.
• Provide quality trauma support and behavioral health care services.
• Provide home-based services to the families of students who have experienced or are at risk of experiencing trauma, including those who are low-income, homeless, involved in the child welfare system or involved in the juvenile justice system.

• Assist teachers with behavior management strategies.
