What you'll do Under the direction of the DON, the Resident Care Supervisor (RCS) has 24-hour responsibility for the effective implementation of the philosophy, goals, policies, and procedures of the hospital and the nursing home and their effect on resident care in a designated unit. The RCS has authority to make decisions on that unit in the areas of resident care, human resource, and financial management. * Assists the DON in development, implementation, and evaluation of on-going service programs that assure quality nursing and resident care programs consistent with Bassett Health Network mission * Participates in program development proposal as requested by DON * Monitors/Maintains unit compliance with regulatory, accrediting and hospital policy for resident services, and environmental and personnel safety * Accurately identifies real/potential problems affecting the service and implements solutions with follow-through and communication * Assesses need for personal growth and development and takes the initiative in seeking opportunities to meet this need. * Oversees unit CQI programs that monitors and evaluates critical aspects of care * Fosters a service that is consistent with the resident care beliefs and shared management philosophy of the department * Consults with residents/families though primary nurse request, direct family request, and/or direct assessment of resident/family response to condition change. Assesses resident/significant other survey responses regarding nursing care and services * Responsible for on-going development of staff and unit specific resident care programs * Responsible for effective and efficient fiscal management of department operations to ensure proper utilization of organizational and financial resources * Responsible for department's operational excellence; ensures department delivers quality services in accordance with applicable policies * Manages human resources to assure quality services and promote positive employee relations * Promotes a positive image of professional nursing and serves as a role model to staff * Attains all agreed to goals and objectives within specified time frames, as part of the organization's overall mission * Attends all required safety training programs and can describe his/her responsibilities related to general safety, department/service safety, and specific job-related hazards * Follows the hospital exposure control plans/blood borne and airborne pathogens * Demonstrates respect and regard for the dignity of all residents, families, visitors, and fellow employees to ensure a professional, responsible, and courteous environment * Promotes effective working relations and works effectively as part of a department/unit team inter- and intra-departmentally to facilitate the department's/ unit's ability to meet its goals and objectives * Accepts Supervisor assignment as requested * Provides resident education as needed * Accepts charge responsibility as directed * Performs medication administration in an efficient manner as needed * Ensures adequate staffing for all shifts. What we need from you Education: * 2 Year / Associate Degree and Current registration as RN in NYS, required * 4 Year / Bachelor's Degree with concentration in Nursing, preferred * *All RN's hired and do not currently hold a BSN will be expected to demonstrate enrollment in a bachelor's prepared nursing program within 4 years of hire. BSN must be completed within 10 years. RN's licensed on or before 12/19/17 are/remain exempt from this requirement. The discharge planning process needs to begin on admission. * Collaborates with the health care team and appropriate department in the management of care across the continuum of care, including pre-admission, discharge, post-discharge, planning length of stay, and utilization of resources. * Utilizes own special body of knowledge and evidence-based guidelines to provide leadership and guidance to the health care team in formulating an individualized multidisciplinary plan of care to include: pre-hospitalization, acute hospital care, discharge education, transition to home and use of community resources. * Facilitates and participates in health care team care conference for patients with complex problems. * Facilitates patient and family education and the discharge process to promote continuity of care and optimal patient outcomes. * Demonstrates experience in the referral process and use of community resources. * Reviews data from admission screening to clarify admission diagnosis, establish appropriate length of stay, and identify any potential outliers and determine appropriateness of admission based on institutional standards and evidence-based guidelines. * Contacts payer source to confirm/negotiate benefits and provide concurrent reviews. * Identifies capitated patients to determine appropriate utilization of series and coordinates post hospital care using defined standards. * Identifies high-risk patients based on clinical and financial criteria for collaboration with patient financial services to problem-solve available resources. * Ensures that appropriate medical/legal documentation is contained in patient's records. * Complies with regulations established by third party payers including but not limited to notices of non- coverage reinstatement and continued stay. * Collaborates with the health care team in implementing strategies to reduce length of stay/resource consumption to optimize patient health status for an assigned service patient. * Assesses educational needs and provides learning opportunities for health care professionals relevant to particular cases and selected patient care groups. * Collaborates with case management leadership to compile and report aggregate variances and data for specific patient care services. * Communicates and analyzes aggregate variances with members of the health care team and develops strategies for variance reduction.
