The goals of the Home-Study Post Release Services Program (HS/PRS) are to: Assess the safety and suitability of a sponsor before an unaccompanied child is released into a sponsor's care (Home Study). Provide case management support to ensure the continued safety and stability of children in sponsor's homes and linking children to resources in the community (Post Release Services). Required Education and Experience Bachelor's degree in the behavioral sciences, human services or social services fields. Bilingual in English and Spanish (both oral and written). Job Specific Duties and Responsibilities Home Study Services Assess potential sponsors' ability to provide a safe living environment for youth through a combination of in-home visits and interviews, phone calls and other investigative activities. Gather information to make placement recommendations for youth, including: interviews with shelter staff, clients, and prospective sponsors, home visit observations, informational documents, and background investigations. Conduct a collaborative psycho-educational process with the youth and sponsor to identify areas where support, resources or information are needed to ensure successful reunification. Educate and prepare the prospective sponsor for reunification with the youth, assisting with accessing needed supports and services. Produce a comprehensive written report containing the home study findings and recommendations within 10 days of case assignment. Post Release Services Ability to contact youth and sponsor within 48 hours of case assignment and facilitate an in-home visit with youth and sponsor within 14 days of case assignment. Conduct home visits and phone contacts with the dual purpose of assessing safety and appropriateness of the youth's placement with his/her sponsor and connecting the youth to services in the community to address risk factors. Perform ongoing case management with to address the youth's needs in the following domains: safety, placement stability, guardianship, education, health and medical care, individual mental health and family stabilization, legal/immigration proceedings, substance abuse, gang prevention, independent living, juvenile/criminal justice, trafficking assistance (as applicable) and child enrichment activities. Facilitation of case management meetings for with youth and sponsor, as well as applicable informal supports and external providers, that continually reassess the needs of the youth and family and ensure that services in place are effective. Submit detailed and thorough documentation in compliance with KYUMH and ORR requirements and reporting deadlines. Expected Hours of Work This position regularly requires flexibility based on the availability of youth and families served and regularly requires non-traditional hours such as evenings and weekends. Eligible for flex schedule. Non-essential during inclement weather. Travel Travel is required and will include local as well as out-of-area and overnight travel. Frequent local travel will be required to serve the home communities of youth and families served by the program. Must be willing and able to travel to other states by flying on commercial airlines, driving a rental car and/or using other transportation, and stay overnight in hotels. Ensure that every single item in inventory is tagged with client initials (no items in the inventory room that are not connected to a particular client). Immediately ensure that any donations leave the clinical wing and are taken to the Teen Shop for processing. The ideal YCA will: speak, read, and write in Spanish and English, work well on a team, accept and appreciate diverse cultures, have a high attention to detail, be passionate about helping at-risk youth, and calm under pressure. A day in the life of a Youth Care Associate: Provide supervision of youth to maintain a safe environment and oversee daily routine. Be a welcoming, nurturing presence to encourage and support youth. Ensure consistent implementation of client treatment plan in cooperation with treatment team. Develop and participate in unit activities such as life skills exercises, recreation activities, cultural education, and spiritual life. Ensure first aid and health care is provided for clients as specified by KyUMH policies. Assume responsibility for a variety of campus activities, including but not limited to: driving a campus vehicle, transporting clients, clothing or supply shopping. Record documentation according to policies and communicate professionally with external stakeholders as needed. De-escalating or physically managing children when they are a danger to themselves or others (training will be provided). For this position, you must: Have a High School Diploma one year of child welfare experience working with children and/or adolescents Have a valid driver's license and acceptable driving record. The ability to speak, read and write English Pass a background check, including federal and state fingerprint checks. Pass a drug test pre-employment.
