This position will be located in Primary Care practices and will work collaboratively with medical providers and a Psychiatric Consultant. Applicant can be LISW or LPCC. This position is responsible for providing behavioral health services to patients, including, as appropriate, completion of brief biopsychosocial assessments, formulation of treatment plans and brief treatment interventions, in order to support and/or enhance biopsychosocial functioning. This role will receive referrals for individuals from at-risk populations from interdisciplinary team members (including physicians, other and members of the care team).
