2. Screen and identify young adult participants in need of care coordination services. Conduct comprehensive assessments by interviewing young adult participants to identify their participant's needs and goals.
3. Collaborate with the SkillSpring Young Adult or Health Care Explorers teams, to identify problems or needs that require special planning, intervention, teaching or follow-up.
4. Actively support measures that prioritize the participant's needs and promotes the effective use of resources. Facilitate the participant's involvement in the direction of care.
5. Identify, plan and arrange for appropriate services by applying a knowledge of local regional and federal services and regulations. Collaborate with the participants, programming teams and community services.
6. Coordinate implementation of the care plan with case conferences and working collaboratively with community based partners on these conferences. to maximize continuity of care
7. Monitor the participant's needs and responsiveness to care plan interventions. Communicate with providers who are delivering care and services for the purpose of maintaining the quality of care. Maintain and revise care plan as the participant's needs change.
8. Document assessments, plans, interventions, the participant's goals and their response to care management.
