    Job Description$35.7227 per hour to $ 42.5277 per hour ( based on RD experience) The Adult Day care in the Bronx is seeking a part time ( 22.5 hours per week) Registered Dietitian to join our team! This position will sit in 1200 Waters Place, Bronx, NY 10461. Work schedule is Monday to Friday 9:30 am to 2:30 pm. This position has a potential for full time as the program grows. Job Summary: Maintains all clinical nutritional services for residents in each community. Acts as liaison between Nursing and Food and Nutrition staff. The function of the position actively incorporates the Mission and Vision of the New Jewish Home. Duties and Responsibilities: Makes initial nutritional assessment of clients; reassesses and revises assessment at least quarterly or more frequently if necessary. Attends and participates in the Interdisciplinary Care Plan Meetings. Evaluates nutritional care and provides follow-up as needed for continuity of care. Assists in the development and presentation of in-services and nutrition education materials for clients as needed. Plans, calculates, and modifies normal and special diets as prescribed by physician's orders. Monitors diet prescriptions with physician orders and modifies as needed. Is available for diet counseling. Assists the community in PI programs/audits. Assists in the development, implementation and revision of cycle menus for general and therapeutic diets. Must understand and respect cultural and religious beliefs of residents served. Other duties as assigned. Requirements: Registered Dietitian. With clinical experience in long term care preferred. Specialized Skills and Competencies: Demonstrates the ability to assess the nutritional status of patients and residents in Sub[1]Acute and Long-Term care setting. Completes documentation which meets best practice and regulatory guidelines. Develops measure goals within individualized care plan Participates in interdisciplinary care conferences. Description of Duties: Conduct a complete nutrition assessment, determine nutrition risk, and enroll participants. Conduct a breastfeeding assessment and provide participant centered counseling that helps a mother initiate breastfeeding, establish a milk supply, exclusively breastfeed, and continue breastfeeding. Address common breastfeeding concerns and issue breast pumps. Develop and implement individual care plans (ICPs) and oversee high risk care to all participants. Refer high-risk participants to other health-related and social services, as necessary. Prescribe, tailor, authorize and reissue food package. Provide and document participant-centered nutrition education/counseling (individual/family sessions) for participants. Ensure the efficient flow of the assessment and education processes. Track high-risk participants' progress in improving their health and document for outcomes. Provide leadership in planning, conducting, and evaluating participant-centered nutrition services. Provide leadership in developing, conducting, and evaluating nutrition training for LA staff in nutrition education topics.
