PRINCIPLE DUTIES:
* Maintains floors and carpets within the Community.

Job Summary:
Under the direction of the Director of Innovation, the Outreach Coordinator facilitates referrals and client assignments for enhanced care management (ECM) between Agency providers and Medi-Cal managed care plans. The Director of Procurement is responsible for the procurement and management of material spend to support the production of plants for various groups across 8 Locations in 3 countries. Establish multi-year agreements across categories, including Raw Material, Outside Processing, and Indirect Materials, including MRO supplies.

Establish and achieve material cost reduction targets, cash flow improvement, and supply improvements (on a year-over-year basis).
Manage supply chain risk through intelligence and information gathering, analysis, and mitigating actions to eliminate risks, including cybersecurity, geopolitical, economic, and legislative/government policies.
Support Site Procurement Teams and work directly with location leadership to ensure alignment of goals covering supply, product profitability, cash flow, plant projects, and production volumes.
Achieve optimal inventories through the implementation and execution of Vendor Managed Inventory (VMI) programs, additional consignment, consolidation of supply with logistics, and further supply consolidation with key MRO partners.

JOB SUMMARY
Under the direct supervision of a Program Supervisor, the Facilitator coordinates the Child and Family Teams (CFTs) and the development and implementation of individualized service plans. Able to effectively engage, develop relationships with appropriate boundaries, and provide support service in accordance to program structure with assigned clients.
Effectively manages a flexible schedule.
Completes all required documentation according to Agency and program standards within required time constraints. These may include: client contact forms, CANS, service activity summaries, data tracking, daily progress notes for all mental health services provided, mileage documentation, and other administrative duties, etc. 6. If applicable, assists in monitoring Child and Family Plan approved expenses, budget, and/or FlexFunds.
Acts as the liaison between the program and community agencies and individuals such as Mental Health Coordinators/Case Managers, County Social Workers, and Probation Officers. As indicated, works with any and all agency/school staff to ensure the coordination of service delivery to Child and Family Teams.
Provides crisis stabilization and management to child/family teams when plans disrupt and/or crisis situations occur. If applicable, participates in the After Hours On-Call system as directed by the Executive Director.
Meets Agency service percentage standard reflecting expected time spent with clients, if applicable.

MINIMUM REQUIRED EDUCATION AND EXPERIENCE
Completion of a Bachelors' degree in Social Work, Psychology, Marriage and Family Therapy or a closely related field of behavioral science and two years of experience providing services to children and families in a mental health or social services setting -OR- a Masters' degree in Social Work, Psychology, Marriage and Family Therapy or a closely related field of behavioral science
DESIRED QUALIFICATIONS:

PRINCIPLE DUTIES:
* Assists with organizing and coordinating all activities for the Assisted Living residents to ensure all their activity needs are met. Maintains current and accurate documentation for residents daily activity participation and follows the individualized plan of care for each resident.

JOB SUMMARY
The Clinician is responsible for establishing and implementing quality clinical treatment and case management for children and adolescents with special mental health needs. Clinicians shall provide individual, group, and family therapy as well as rehabilitative services and case management in whatever setting is most beneficial for the identified client. In addition, the Clinician is responsible for completing all documentation and written reports required by the Agency.
ESSENTIAL FUNCTIONS
Provides comprehensive mental health assessments on assigned clients to determine the need for mental health services. Develops treatment plans and goals based on assessed areas of needs and strengths identified in the assessment. The Clinician shall monitor and evaluate client's progress on identified goals in the treatment plan.
Completes all required documentation in a timely manner for each assigned client and maintains a comprehensive set of treatment records as stipulated in the County Mental Health Plan (MHP) or other agencies responsible for program oversight.
Acts as the liaison between the program and family/community agencies including individuals such as Parents/Caregivers, Case Managers, County Social Workers, Teachers/Education Providers, and Probation Officers. The Clinician shall work with the family or care providers in the implementation of treatment plans and will maintain contacts with all important individuals in the client's life.
Provides individual, family, and group counseling based on the needs of the clients served.
Meets Agency service percentage standard reflecting expected time spent with clients, if applicable.
Completes other assigned program operational tasks in accordance with supervisory directives.
MINIMUM REQUIRED EDUCATION AND EXPERIENCE
Completion of a clinical program with a Master's or Ph.D. degree in a field of behavioral science from an accredited educational institution.
Holds a current license registration issued by appropriate state agency consistent with standards outlined in the Agency's Clinical Staff Licensing Requirements policy. Licensed Professional Clinical Counselor {LPCC) license may be considered with prior approval from the Regional Director in consultation with the Director of Quality.
Must have good oral and writing skills in the English language, including the ability to write concise, informative professional reports and assigned documentation in a timely manner.

Job Summary:
Under the direction of the Director of Innovation, the Lead Care Coordinator (LCC) provides direct service, as well as coordinates care management and functions as a part of a "Care Team" for the Enhanced Care Management (ECM) Program. The LCC oversees specific cases, coordinates health care benefits, provides education and facilitates client access to care in a timely and cost-effective manner.
