What You'll Do in This Role:
Join our vibrant senior living community and be part of a Culinary Team that makes a daily difference! As a Porter, you'll keep our kitchen shining and running smoothly-washing dishes and equipment, maintaining spotless floors, organizing storage areas, and helping with deliveries. Primary Responsibilities:
* Cleaning and sanitizing of all meal preparation areas as well as kitchen walls, stoves, sinks, ovens and grills.

* Ensuring that food mixers, slicers and other cooking equipment are in good working order.
* Ensuring that refrigerators, freezers and storage areas are clean and well organized.

What You'll Do in This Role:
* The Nurse Practitioner (NP) is a vital member of the interdisciplinary team, providing high-quality, participant-centered care to older adults in the PACE program. The NP delivers primary and preventive healthcare, including assessments, diagnosis and treatment of acute and chronic conditions, prescribing medications, and coordinating care with the team. This role supports the health, safety, and independence of participants through a holistic, geriatrics-focused approach and requires a proactive, compassionate provider committed to serving the aging population.
Primary Responsibilities:
Leadership & Collaboration:
* Partner with the Director of Interdisciplinary Operations to support projects and optimize provider workflows.
* Collaborate with community partners and vendors to enhance care delivery.
* Support the interdisciplinary team (IDT) in coordinating complex care.
Clinical Care:
* Conduct health assessments, physical exams, and screenings to monitor participant's health.
* Assess and treat acute medical needs to prevent hospitalizations.
* Lead advanced care planning discussions and update advanced directives.
Patient Education & Care Planning:
* Educate participants and caregivers on managing chronic illnesses and treatment plans.
* Contribute to care plans that promote independence and address medical, functional, and psychosocial needs.
Team Participation:
* Engage in IDT meetings, offering insights to optimize participant care.
On-Call and After-Hours Support:
* Provide rotating on-call and weekend support as needed to address urgent care needs.
What You'll Need:
Education & Certification:
* Master's degree in nursing (NP) from an accredited program
* Current, unrestricted Nurse Practitioner license in Michigan
* CPR certification or willingness to obtain upon hire
Experience:
* Minimum of 1 year as a Nurse Practitioner
* Experience with frail or older adult populations preferred
Skills & Competencies:
* Strong clinical assessment and decision-making skills
* Knowledge of geriatric care and psychosocial determinants of health
* Effective communication and collaboration within an interdisciplinary team
* Familiarity with electronic health records (EHR) and care coordination

Home-Based Support:
* Provide personal care, homemaking, and respite care tailored to each participant's needs and goals.
* Assist participants with activities that support their health, function, and overall well-being.
* Observe and report changes in condition or environment to the Interdisciplinary Team (IDT) to support timely care adjustments.
Life Plan Adherence:
* Deliver care according to the individualized Life Plan developed by the IDT.
* Ensure all tasks align with the participant's preferences, health goals, and plan of care.
Collaboration with the Interdisciplinary Team (IDT):
* Serve as a vital member of the care team, communicating regularly with the IDT.
* Share insights and observations that inform participant care planning and updates to the Life Plan.
Encouraging Independence:
* Support participants in performing tasks safely to maintain independence and functional ability.
* Promote a sense of dignity, respect, and empowerment in every interaction.
What You'll Need:
* High school diploma or GED required.
* Certified Home Health Aide (HHA) or Certified Nurse Aide (CNA) per state requirements.
* Prior experience providing independent home care services and supporting older adults in home-based settings.
* Previous experience providing care to older adults preferred.
* Excellent communication and observation skills.
* Reliable transportation and valid driver's license required.

What You'll Do in This Role:
* The Physician Assistant (PA) works collaboratively with LifeCircles medical providers and the interdisciplinary team (IDT) to deliver high-quality, participant-centered care. Acting within the scope of their practice, the PA provides medical care to LifeCircles participants across multiple settings, including day centers, home visits, hospitals, nursing homes, and assisted living facilities, ensuring comprehensive support and continuity of care.
Primary Responsibilities:
Patient Care & Assessment:
* Assess, initiate, and coordinate participant care in collaboration with the medical team, interdisciplinary team (IDT), and external agencies.
* Monitor chronic conditions, address acute health concerns, and provide treatment in the clinic, participants' homes, or care facilities (ALF, SNF, AFC).
* Facilitate advanced care planning discussions with participants and caregivers.
Education & Family Engagement:
* Educate participants and families on managing chronic conditions and treatment plans.
* Communicate effectively with families and caregivers to support informed decision-making.
Interdisciplinary Team Collaboration:
* Participate in daily IDT meetings and contribute to collaborative care planning.
* Coordinate with external partners to support smooth transitions and access to services.
Quality Improvement & Compliance:
* Support quality improvement initiatives and ensure compliance with organizational and regulatory standards.
On-Call & After-Hours Support:
* Provide rotating on-call and weekend support as needed to address urgent care needs.
What You'll Need:
Education & Certification:
* Completion of an accredited Physician Assistant program
* Current, unrestricted Physician Assistant license in Michigan
* Basic Life Support (BLS) and CPR certification (or willingness to obtain)
* Current DEA registration and Michigan Controlled Substance License
