Qualifications:
Education: Graduate of an approved nursing education program.
Licensure: Current RN licensure with State Board of Nursing in the applicable state of practice or current RN temporary work permit in the applicable state of practice
Certifications: Certification as a Certified Lactation Consultant (CLC) must be obtained within two years of date of hire or transfer. Must have a current American Heart Association Healthcare Provider Care or complete a course within their probation period.

PURPOSE AND SCOPE:The student intern supports FMCNA's mission, vision, core values and customer service philosophy. As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
Utilizes FMS Ultra Care patient education programs, established social work theory and methods and quality of life measurement instruments as part of assessment and care planning to resolve barriers and meet patient treatment goals.
Provides established outcomes driven psycho-educational counseling methods aimed at improving areas of need, treatment adherence, vocational/educational rehabilitation and/or quality of life.
Provides supportive and goal directed counseling to patients who are seeking transplant.
Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.
Will provide general information about Do Not Resuscitate Orders and Advanced Directives.
Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes.
Provides usual care and/or Social Work Intensive to address non-adherence and quality of life concerns for all patients.
Knowledgeable of and adheres to FMCNA Social Work Policy, including documentation.
Patient Education:
Assess patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style and needs.
With other members of the interdisciplinary team, provides appropriate information about all treatment modalities. Facilitates the transplant referral process and collaborates with interdisciplinary team, if delegated by Clinical Manager.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviewed patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Insurance and Financial Assistance:
Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
Understands FMCNA financial policies and procedures as they related to patient services. Collaborate with Financial Coordinators to educate patients about programs that may be available to assist with uncovered services, how to resolve billing concerns, and understand financial responsibilities.
Staff Related:
Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources).
Develops and provides training to staff pertaining to psychosocial topics as needed. Trains staff in Ultra Care Programs as needed.

About this Position
The Testing Coordinator is responsible for assisting the Manager of Testing & Assessment to ensure the uniform implementation of proper testing policy and procedures, coordinating all testing activities for all exams with the testing team and manager, completing special projects, and working with the manager to assure adequate office/proctor coverage. About the role:

You will report to the Plasma Center Manager and will perform as a plasma donor screener and perform phlebotomy to support plasma center operations.
How you will contribute:
· You will greet donors as they enter and exit the donor floor.
· You will perform venipuncture of donors and programming of plasmapheresis machine.
· You will monitor donors during the donation process and manage donor reactions.
· You will perform all tasks required for the setup, verification, operation, and troubleshooting of plasmapheresis equipment within scope of training.
· You will install, prime, and disconnect disposable sets on the plasmapheresis machines
· You will stock supplies, break down empty cartons and assist with proper disposal.
· You will take and record donor pulse, blood pressure, and temperature measures and monitor electronic donor questionnaire system.
· You will perform finger sticks, test sample, and record other donor measures to include hematocrit, total protein, and weight.
· You will enter donor information into the Donor Information System (DIS).
· You will coordinate donors to donor floor and compensate donors using the Debit Card system.

Provide individual academic guidance and counseling designed to encourage and assist participants in completing a rigorous secondary school program of study.
Coordinate all program activities in assigned target schools.
Maintain and manage student records and documentation of participants' program eligibility, need for services, services provided, and progress.
Plan and conduct workshops (either in-person or virtual) related to career exploration, college orientation, and financial aid awareness.
Develop Individual Career and Academic Plans (ICAP) for participants and provide academic planning guidance.
Assist participants with admissions and financial aid for college.
Assist the Project Director in preparing and gathering data for the Annual Performance Report/Review (APR) required by the Department of Education.
Provide early retention intervention activities focusing on 6th-12th grade participants to assist with secondary school completion and postsecondary enrollment.
Performs on-going evaluation of the effectiveness of the programs services and monitoring of all program objectives, participant retention and activities.
Help supervise and transport participants on visits to post-secondary institutions, businesses and cultural events.
Provide parental involvement workshops on the availability of financial aid, FAFSA submission, the importance of post-secondary education and other educational topics.
Provide academic services designed to encourage reentry of eligible populations into secondary or postsecondary education-including dropouts and veterans.
Completes all mandatory training and participated in a minimum of two professional development each year.
Performs other duties as assigned.

Working Conditions:
Must be able to work flexible hours to include some nights and weekends.
