Provide initial physical behavior assessment, clinical specimen/evidence collection, and intervention to individuals experience sexual assault. Coordinate the provision of urgent medical evaluation, follow-up, and crisis intervention resources. Establish and maintain clinical and legal records and chain of custody. Provide testimony regarding assessment, evidence collection and handling and documentation.
Essential Functions & Responsibilities:
* Supports policies, regulations, goals and objectives of the program
* Obtains historical information regarding both the incident and health status of the patient.
* Conducts physical and forensic examination of individuals experience sexual assault for the purpose of detecting and treating injury/disease and collection potential evidence.
* Provides care in a compassionate manner, adjusting timing of procedures as needed to meet the psychological and physical needs of the patient.
* Identified and facilitates care for injuries that require medical treatment. Collects, processes, and maintains appropriate chain of custody of both clinical and non-clinical evidence and necessary records.
* Documents assessment, evidence collection, and interventions with careful attention to legal consideration.
* Provides crisis intervention
* Provides patient education regarding examination, procedures, post exposure pregnancy and STD prevention, follow-up care, HIV and other STD testing, an rape-trauma syndrome.
* Conducts activities with attention to patient right of privacy and confidentiality
* Coordinates with law enforcement and rape advocacy groups as part of the SART team
* Participates in the education of the Sexual Assault Nurse Examiners, health care providers, law enforcement and the public as necessary
* Maintains equipment and supplies necessary for examinations.
* Provides testimony, when requested, on examination procedures and findings.
* Participates in gathering statistical and program data.
* Provides activity reports as requested.
* Participate in out-reach and community education activities.
* Participate in necessary meeting and committee work.
* Participate in maintaining and achieving grant related activities, goals, objectives, and outcomes.

Qualifications:
* Graduate of an accredited nursing program required.
* Valid RN License from Virginia or reciprocal compact state required.
* Successful completion of an online adult/adolescent Sexual Assault Nurse Examiner (SANE) course through the International Association of Forensic Nurses required within 30 days of hire.
* AHA BLS Provider CPR required at hire.
* Must complete International Association of Forensic Nurses (IAFN) approved Comprehensive Training for Adults within four months of hire.
* Must complete International Association of Forensic Nurses (IAFN) approved Comprehensive Training for Pediatrics within one year of hire.
* Must be able to respond to exam site at Mary Washington Hospital within 60 minutes.
* Must have own transportation.
* Must be available to take on call time.
* Emergency / trauma experienced preferred.
* Membership in a professional organization preferred.

The RN Clinical Coordinator provides clinical leadership and coordination of the Forensic department at Mary Washington Healthcare. In collaboration with department leadership, this role supports compliance with applicable policies, procedures, regulatory, and accreditation standards. The RN Clinical Coordinator will assist leadership in ensuring the highest degree of clinical/operational performance and customer experience. This role participates in forensic patient care by providing an initial physical assessment, clinical specimen/evidence collection, and intervention. This role also maintains clinical and legal records, ensures chain of custody, provides testimony regarding assessment, evidence collection, handling, and documentation.
Essential Functions & Responsibilities:
* Works in partnership with management to review, monitor, and document Associate competencies. Supports and contributes to performance feedback for assigned staff.
* Supports management in the selection, hiring, orientation, and performance management for assigned staff.
* Assists management by contributing to the development, administration and monitoring the departmental budget to ensure cost-effective operations within authorized budget allocations.
* Participates in Quality Assurance/Performance Improvement (QA/PI) initiatives, helps develop action plans, and addresses performance issues to ensure safe, effective, and service-oriented patient care.
* Coordinates and organizes Forensic operations and resources to assure efficient and effective performance of services provided.
* Builds collaborative relationships with physicians, unit leadership and staff to identify/implement opportunities to improve patient care services.
* Serves as a clinical resource for the team members and nursing staff regarding Forensic patient care, offering direct coaching as necessary.
* Assists management in the development, revision, and adherence of departmental policies, procedures, protocols, and standards to ensure quality care, efficiency, and procedural compliance.
* Regularly communicates information, discusses safety, quality and regulatory issues and monitors continued understanding and compliance with MWHC and departmental policies. Informs departmental leadership of any immediate concerns.
* Maintains clinical nursing competencies and provides direct patient care to patients when needed.
* Conducts physical and forensic examination of individuals who experience sexual assault for the purpose of detecting and treating injury/disease and collection potential evidence.
* Identifies and facilitates care for injuries that require medical treatment. Collects, processes, and maintains appropriate chain of custody of both clinical and non-clinical evidence and necessary records.
* Documents assessment, evidence collection, and interventions with careful attention to legal consideration.
* Provides patient education regarding examination, procedures, post exposure pregnancy and STD prevention, follow-up care, HIV and other STD testing, and rape-trauma syndrome and crisis intervention.
* Coordinates with law enforcement and rape advocacy groups as part of the SART team. Participates in education as needed.
* Provides testimony, when requested, on examination procedures and findings.
* Participates in outreach and community education activities.
* Supports grant related activities, goals, objectives, and outcomes.
* Performs other duties as assigned
Qualifications:
* Valid RN License from Virginia or reciprocal compact state required.
* AHA BLS Provider CPR required at hire.
* Minimum of five (5) years of clinical RN experience in a Forensic program, required.
* Certified in SANE-Adults and SANE-Pediatrics, required.
* Graduate of an accredited school of nursing required; BSN preferred.
* Supervisory experience preferred.
* Must be able to respond to the exam site within 60 minutes.
* Must be available to take on call time.
* Membership in a professional organization is preferred.
* Demonstrates proficiency in verbal and written communication, as well as computerized systems.

Serving as a consistent point of contact for patients and families during the high-risk post-discharge period, the role supports patients and families by addressing medical, psychosocial, and logistical needs. The Community C3 collaborates closely with providers, ancillary teams, and community agencies to reduce care fragmentation, prevent avoidable readmissions, and optimize follow-up care. The role contributes to improved patient outcomes and system-wide efficiency.
Essential Functions and Responsibilities:
* Serves as a consistent point of contact for patients, families, providers, and community resources post-discharge.
* Coordinates follow-up care across inpatient and outpatient teams and ensures timely initiation of post-acute services such as home health, DME, medications, and appointments.
* Facilitates access to long-term support services and resolves barriers to care, including service delays, affordability and insurance issues, and gaps in social determinants of health (e.g., transportation, housing, food insecurity, caregiver support).
* Manages post-discharge inquiries by serving as the central point of contact for patients, families, and providers, reducing callbacks to inpatient teams and improving care continuity.
* Coordinates with pharmacies, insurers, and providers to expedite medication access and prior authorizations.
* Documents interventions in Epic and contributes to system reporting on readmissions, follow-up volume, and care coordination outcomes.
* Educates patients and families on post-discharge care plans and available community resources to support recovery and prevent readmissions.
* Guides hospital and community staff on outpatient care coordination, available resources, and patient navigation processes.
* Performs other duties as assigned.
Qualifications:
* Associate's degree in nursing required.
* Valid RN license in the state of Virginia or a reciprocal compact state required.
* Minimum of three (3) years recent acute care nursing or case management experience.
* Bachelor of Science in Nursing preferred.
* Experience with Epic or equivalent electronic medical record system preferred.
* Prior case management/discharge planning experience preferred.
* Certification in case management (ACM, CCM) or other relevant specialty preferred.

This individual will assess physical, psychosocial, cognitive, and functional components, develop a treatment plan, order and interpret diagnostic components of the plan, and prescribe non-pharmacological and pharmacological agents.
The APP functions professionally under a practice agreement with the supervising physician and operationally under the Vice President Nursing Services and/or the designee.
Essential Functions & Responsibilities:
* In collaboration with physician, demonstrates ability to consul patients and family members about diagnosis, testing, surgery and risk of procedures.
* Performs comprehensive health assessments for the purpose of diagnosing and treating individuals.
* Formulates appropriate differential diagnosis, develop treatment plan to include appropriate diagnostics; identifies non-pharmacological interventions, appropriate pharmacological agents and education. The plan is based on mutually established priorities to maximize the heath potential of the patient.

Qualifications:

* Recent adult medical-surgical experience (1 year of surgical experience preferred), required
* Ability to work effectively in emergency situations and under stress
* Proficient leadership skills
