The Family Service Worker (FSW) will recruit and enroll children in the Head Start program and ensure that Head Start program services and community resources are available to enrolled children and their families. Complete the family assessment (RISE). Establishes and monitors goals for all families enrolled in the program.
Collaborate and maintain open channels of communication with parent/guardians, agency staff, and the community to provide seamless services.
Perform required data entry functions in the Management Information System (MIS) accurately and in a timely manner to facilitate program services the include case notes, goal status, follow-up for referrals, etc.
Protect the confidentiality of all information.
Refer to community agencies to ensure that families have accessibility to services.
Assist parents with transitional activities into Head Start and from Head Start to other educational settings.
Participate in social service training, staff meetings, center parent meetings, and other meetings as required to deliver and coordinate agency services as necessary for children and families.
Collaborate with the Center Coordinator to organize and conduct Child and Family Review (CFR) in order to coordinate individualized services for each child and family.

Collaborate with the Center Coordinator to plan, organize and attend parent meetings.

Confer with physicians, caseworkers, and other community partners to establish and monitor interventions on behalf of the child and family.
Report all suspected child abuse/neglect cases to Franklin County Children Services according to agency procedures and the Ohio Revised Code Section 2151.421.

1) A valid California Class C Driver License is required to carry out the essential job-related functions of this position. Mileage reimbursement and proof of automobile liability insurance must be maintained.

BACKGROUND INVESTIGATION
Job offers are contingent upon passing a background investigation which includes a fingerprint check and search of the Child Abuse Index before appointment. If an individual has been convicted of a crime other than a minor traffic violation, the individual cannot work or be present in any community care facility unless they request and subsequently obtain a criminal record exemption from the Community Care Licensing Division, Care Provider Management Bureau (CPMB). With the exception of minor traffic violations, all convictions, including misdemeanors, felonies, and convictions occurring a long time ago, require an exemption. CPMB also examines arrest records to determine if there is a possible danger to clients. What You'll Do:

What you'll do:

Consistently records client progress in relation to treatment goals. Provides accurate, concise narrative when needed on any documentation, including progress notes, treatment plans, behavior management programs and discharge planning on an accurate and timely basis.
