UPRISING ADDICTION CENTER will take all necessary and appropriate measures to maintain individual client records and information in a confidential and professional manner. SUMMARY The Therapist at UPRISING ADDICTION CENTER is responsible for providing addiction and mental health treatment to patients in the Inpatient Detoxification, Resident Treatment (RTC), and Partial Hospital Program (PHP). As part of a multidisciplinary team, the Counselor will perform direct services, including individual, family and group psychotherapy, assessments, consultations and crisis interventions within the scope of the respective individuals license. Appropriately documents clinical service provided. Ensures to use the utmost of care in protecting the confidentiality of client records in accordance with State and Federal laws Collaborates with medical, nursing, and counseling staff regarding detox clients Provide Professional and ethical therapy to chemically dependent patients in treatment through group, individual counseling, family counseling, and education. Documenting casework in a timely manner in compliance with agency policies and procedures, including writing objective case notes, progress and discharge reports, and other timely status reports as requested by the supervisor and according to Louisiana & Joint Commission standards. Presents changes, improvements or additions to protocols or behavioral health services to the team. Understands and adheres to UPRISING ADDICTION CENTERs compliance standards as they appear in UPRISING ADDICTION CENTERs Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Keeps abreast of all pertinent federal, state and UPRISING ADDICTION CENTER regulations, laws, and policies as they presently exist and as they change or are modified. Keeps abreast of current literature and developments in the field, and attending required agency training. Appropriately documents clinical service provided. Performs other duties and tasks as required AGE SPECIFIC COMPETENCIES (ADULT, 18yrs+) 1. Demonstrates ability to systemically collect, record and analyze data and other relevant interactive social systems, including mental status, special precautions (suicidality, homicidality, elopement); physical problems/medical acuity (withdrawal and nutritional needs); and ethnic concerns (social, religious, or cultural needs). 2. Uses assessment data to identify problem list and other interactive social systems. 3. Initiates/develops a treatment plan with specific goals and interventions unique to the needs of the patient. 4. Intervenes as guided by the treatment plan to implement appropriate interventions that promote, maintain, or restore physical and mental health, prevent illness effect rehabilitation, and restore developmental progression in adulthood. 5. Provides health teaching for adult/family through individual and group interactions. 6. Recommends special precautions for adults, as needed. 7. Evaluates the response of the adult/family to interventions in order to revise the plan, including discharge criteria. 8. Ability to apply ASAM criteria for appropriate level of care determination. MINIMUM JOB REQUIREMENTS Education, Training, and Experience Required: Mental health professional licensure in Louisiana, to include Licensed Psychologist (Ph.D., Psy.D., Ed.D.), Licensed Clinical Social Worker (LCSW/LMSW), Licensed Professional Counselor (LPC/P-LPC)), Licensed Marriage and Family Therapist (LMFT/P-LMFT). Successful completion of continuing education as required by license issuing body. Required: possess a Masters Degree in social work, counseling, psychology, addiction or related field from a regionally or nationally accredited university. Current CPR certification (will provide training at facility) Current SAMA/CTI/CPI certification (will provide training at facility) Required Knowledge, Skills, and Abilities: Knowledge of industry standards: organization, theory, principles, and research processes, accreditation and regulatory standards, ability to assess clinical care requirements based on psychotherapeutic and human development principles, scope of practice, and regulatory standards. Understanding of Addiction: Basic knowledge about substance use disorders Treatment knowledge: familiarity with behavior change and recovery models Application to practice: Methods for applying intervention and recovery knowledge to practice Professional readiness: Issues related to self-awareness, diversity, ethics, ad continuing education Knowledge of PC software programs, particularly Microsoft Word and Excel is required. Accurately and timely enters clinical documentation using electronic medical records (EMR) software. Written and oral communication skills; must be able to concentrate on work amid distractions such as noise, conversations and foot traffic Must maintain self-control in volatile or hostile situations such as when verbally or physically confronted in an aggressive manner Must be able to work independently with little direct supervision in an unstructured environment with multiple demands Reading and writing level that can clearly express or understand complex concepts, assessments, directions and processes or sequences of events - level equivalent to at least two years of college. Ability to represent the program well in interactions both inside and outside the facility Working knowledge of (American Society of Addiction Medicine) ASAM criteria. Assesses acute mental health and behavioral medicine conditions in beneficiaries who present with such issues. Maintainsappropriate documentation of patient care in accordance with current unit and hospital standard operating procedures (SOPs) and current DoD, DHA PCBH, Federal and state regulations. Assists primary care providers with the treatment of patients with known high-risk factors such as suicide, history of trauma, and co-occurring substance use disorders. Conducts PCBH model treatment, monitors, approves, and documents all, treatment goals. Reviews and approves the appropriate modality of treatment for each patient. Develops treatment processes and procedures for orientation of patients. Evaluates information/education processes. Institutes changes to increase effectiveness of treatment efforts. Evaluates appropriateness of treatment goals. Determines appropriate dispositions for severe cases. Provides structure and procedures for organization of case staffing information.Establishes policies for case consultation. 3. Evaluates and approveseducational materials for use by clinic staff and patients. 4. Provides consultation regarding interpretation and approval of outcome measures and screening instruments. Develops, implements, and evaluates professional development activities, coordinates inter and intra-departmentalin-service training on PCBH related topics. The Primary Care Behavioral Health Program provides a team-based approach to healthcare by integrating behavioral health providers and registered nurses into Primary Care Clinics to deliver evidence-based biopsychosocial assessment, treatment, prevention and management of behavioral health problems and biopsychosocially influenced health conditions for Military Health System beneficiaries. MAJOR DUTIES 1. Serves as a Clinical Psychologist Behavioral Health Consultant (BHC) for the Clinic and Behavioral Health System of Care (BHSOC) Primary Care Behavioral Health (PCBH) program. Makes mental health diagnoses in accordance with the most recent version of the Diagnostic and Statistical Manual of Mental Disorders (DSM) when differential diagnosis is required. Provides individual and group treatment, using various modalities as clinically indicated to include brief problem-focused interventions, cognitive-behavioral therapy, and psychoeducational classes. Assesses acute mental health and behavioral medicine conditions in beneficiaries who present with such issues. Maintains appropriate documentation of patient care in accordance with current unit and hospital standard operating procedures (SOPs) and current DoD, DHA PCBH, Federal and state regulations. Maintains performance and conduct in accordance with current unit and hospital SOPs; current DoD, DHA, Federal and state regulations; and professional ethics. Assesses and provides intervention for individuals with a wide range of chronic medical conditions, and behavioral health issues affecting medical conditions to include assessment and disposition of beneficiaries with acute and chronic risk. Provides treatment via targeted and efficient appointments for initial and follow-up visits. Performs PCBH biopsychosocial evaluations using expert knowledge of clinical and clinical health psychology. Conducts outpatient group classes, stress management, relaxation training, assertive/social skills training, smoking cessation training, substance abuse identification and brief related interventions appropriate for primary care, as well as evidencedbased interventions for mastery of behavioral health conditions (e.g. anxiety, depression, post traumatic symptoms etc.) and behavioral medicine conditions (e.g. insomnia, acute and chronic pain, diabetes, weight management etc.) as required. Participates in quality assurance activities. Provides psychological and clinical consultation to physicians, other professionals and paraprofessionals through established consultation channels. Provides feedback to Primary Care Managers (PCMs) regarding consultation findings and recommendations as directed by the current DHA PCBH Administrative Instruction. Consults with primary care providers to select, approve, and document treatment priorities in SMART goal format. Serves as the authority on primary care psychology for the clinic. Educates clinic staff on the principles of primary care behavioral health via trainings and briefings. Conducts meetings and conferences to review the level of care for appropriateness of treatment in primary care and evaluate and approve referral sources. Establishes cooperative agreements among various providers and clinics throughout the Military Treatment Facility (MTF). Monitors, evaluates, and approves use of community resources. Provides reports and approves consults. Uses objective behavioral outcome measures to evaluate progress in treatment. 2. Provides evidenced-based individual treatment and psychoeducational classes for patients enrolled in primary care clinics. Provides expertise in human behavior as a primary care team member and provides consultation to primary care and specialty clinic providers regarding psychological treatment options to optimize health outcomes. Assists primary care providers with the treatment of patients with known high-risk factors such as suicide, history of trauma, and co-occurring substance use disorders. Conducts PCBH model treatment, monitors, approves, and documents all, treatment goals. Reviews and approves the appropriate modality of treatment for each patient. Develops treatment processes and procedures for orientation of patients. Evaluates information/education processes. Institutes changes to increase effectiveness of treatment efforts. Evaluates appropriateness of treatment goals. Determines appropriate dispositions for severe cases. Provides structure and procedures for organization of case staffing information. Establishes policies for case consultation. 3. Executes performance initiatives (PI) through revision of clinical standards and procedures; and performs a variety of quality management/quality assurance (QM/QA) duties to meet the Joint Commission (TJC) requirements. Evaluates clinical records and monitors all objective instrument administration and scoring for quality assurance. Evaluates environment of care. Monitors all risk management issues and takes appropriate action. Establishes and executes performance initiatives. Works with primary care team to develop processes to increase same day “warm handoffs” of patients from the primary care provider to the BHC. Works to increase overall penetration into the enrolled clinic population by targeting common conditions with behavioral components that are amenable to BHC intervention. Performs daily activities to increase BHC clinical productivity (e.g. scrubbing PCM appointment lists, circulating in the clinic and consulting with staff, being vocal in morning huddles etc.). Establishes and monitors objective behavioral outcome measures. Evaluates and revises standard operating procedures. Conducts program evaluation. Consults with ancillary care providers. Substantiates the results of crisis intervention. Reviews data provided in reports of special actions. Reviews statistical data to modify program goals. Evaluates and approves educational materials for use by clinic staff and patients. 4. Provides consultation regarding interpretation and approval of outcome measures and screening instruments. Develops, implements, and evaluates professional development activities, coordinates inter and intra-departmental in-service training on PCBH related topics. Evaluates scope of practice and makes adjustments for increased adherence and efficacy as needed. Evaluates training opportunities, development activities, and orientation activities. This definition of specialized experience is typical of work performed at the next lower grade/level position in the federal service (GS-12). UPRISING ADDICTION CENTER will take all necessary and appropriate measures to maintain individual client records and information in a confidential and professional manner. SUMMARY The Therapist at UPRISING ADDICTION CENTER is responsible for providing addiction and mental health treatment to patients in the Inpatient Detoxification, Resident Treatment (RTC), and Partial Hospital Program (PHP). As part of a multidisciplinary team, the Counselor will perform direct services, including individual, family and group psychotherapy, assessments, consultations and crisis interventions within the scope of the respective individual's license. Appropriately documents clinical service provided. Ensures to use the utmost of care in protecting the confidentiality of client records in accordance with State and Federal laws Collaborates with medical, nursing, and counseling staff regarding detox clients Provide Professional and ethical therapy to chemically dependent patients in treatment through group, individual counseling, family counseling, and education. Documenting casework in a timely manner in compliance with agency policies and procedures, including writing objective case notes, progress and discharge reports, and other timely status reports as requested by the supervisor and according to Louisiana & Joint Commission standards. Presents changes, improvements or additions to protocols or behavioral health services to the team. Understands and adheres to UPRISING ADDICTION CENTER's compliance standards as they appear in UPRISING ADDICTION CENTER's Corporate Compliance Policy, Code of Conduct and Conflict of Interest Policy. Keeps abreast of all pertinent federal, state and UPRISING ADDICTION CENTER regulations, laws, and policies as they presently exist and as they change or are modified. Keeps abreast of current literature and developments in the field, and attending required agency training. Appropriately documents clinical service provided. Performs other duties and tasks as required AGE SPECIFIC COMPETENCIES (ADULT, 18yrs+) 1. Demonstrates ability to systemically collect, record and analyze data and other relevant interactive social systems, including mental status, special precautions (suicidality, homicidality, elopement); physical problems/medical acuity (withdrawal and nutritional needs); and ethnic concerns (social, religious, or cultural needs). 2. Uses assessment data to identify problem list and other interactive social systems. 3. Initiates/develops a treatment plan with specific goals and interventions unique to the needs of the patient. 4. Intervenes as guided by the treatment plan to implement appropriate interventions that promote, maintain, or restore physical and mental health, prevent illness effect rehabilitation, and restore developmental progression in adulthood. 5. Provides health teaching for adult/family through individual and group interactions. 6. Recommends special precautions for adults, as needed. 7. Evaluates the response of the adult/family to interventions in order to revise the plan, including discharge criteria. 8. Ability to apply ASAM criteria for appropriate level of care determination. MINIMUM JOB REQUIREMENTS Education, Training, and Experience Required: Mental health professional licensure in Louisiana, to include Licensed Psychologist (Ph.D., Psy.D., Ed.D.), Licensed Clinical Social Worker (LCSW/LMSW), Licensed Professional Counselor (LPC/P-LPC)), Licensed Marriage and Family Therapist (LMFT/P-LMFT). Successful completion of continuing education as required by license issuing body. Required: possess a Master's Degree in social work, counseling, psychology, addiction or related field from a regionally or nationally accredited university. Current CPR certification (will provide training at facility) Current SAMA/CTI/CPI certification (will provide training at facility) Required Knowledge, Skills, and Abilities: Knowledge of industry standards: organization, theory, principles, and research processes, accreditation and regulatory standards, ability to assess clinical care requirements based on psychotherapeutic and human development principles, scope of practice, and regulatory standards. Understanding of Addiction: Basic knowledge about substance use disorders Treatment knowledge: familiarity with behavior change and recovery models Application to practice: Methods for applying intervention and recovery knowledge to practice Professional readiness: Issues related to self-awareness, diversity, ethics, ad continuing education Knowledge of PC software programs, particularly Microsoft Word and Excel is required. Accurately and timely enters clinical documentation using electronic medical records (EMR) software. Written and oral communication skills; must be able to concentrate on work amid distractions such as noise, conversations and foot traffic Must maintain self-control in volatile or hostile situations such as when verbally or physically confronted in an aggressive manner Must be able to work independently with little direct supervision in an unstructured environment with multiple demands Reading and writing level that can clearly express or understand complex concepts, assessments, directions and processes or sequences of events - level equivalent to at least two years of college. Ability to represent the program well in interactions both inside and outside the facility Working knowledge of (American Society of Addiction Medicine) ASAM criteria.
