About this Position:
As an Occupational Therapist in our outpatient clinics, you will have the time and resources to provide one-on-one carewith 45-minute treatment sessions, ensuring high-quality interactions and better patient outcomes. With a maximum of 10 patients per day, you'll have the opportunity to focus on delivering personalized care without the burden of overwhelming caseloads. We also offer flexible scheduling options, including weekends and holidays off, to help you maintain a healthy work-life balance.
In this role you will be responsible for examining, evaluating and testing individuals with mechanical, physiological and developmental impairments, functional limitations, and disabilities or other health and movement-related conditions in order to determine a diagnosis, prognosis and plan of treatment intervention, and assess the ongoing effects of intervention. The OT is also responsible for alleviating impairments, functional limitations and disabilities by designing, implementing and modifying treatment interventions. Additional responsibilities include reducing the risk of injury, impairment, functional limitation and disability, including the promotion and maintenance of fitness, health and wellness in populations of all ages. The OT will be engaged in administration, consultation, education and research. The OT will be expected to conduct themselves in an effective professional behavior manner. The attributes, characteristics or behaviors required for success in this job include: critical thinking, communication, problem solving, interpersonal skills, responsibility, professionalism, use of constructive feedback, effective use of time and resources, stress management, and commitment to learning.

MINIMUM QUALIFICATION REQUIREMENTS:

Participates in organization's continuous quality improvement efforts and Positive Behavior Intervention Support.
ESSENTIAL FUNCTIONS:
1. Delivers intensive therapeutic services using a trauma informed care approach on an individual, sibling or family basis. Service delivery provided in a home setting.
2. Provides assessment services and caseload consultation. Provides on-going assessment, evaluation, treatment, discharge planning and other therapeutic services as required in a group or individual counseling sessions.
3. Completes accurate, timely clinical documentation of the highest quality and consistent with agency standards, rules and regulation, and Medicaid. Completes required assessments, treatment plans and progress notes.
4. Formulates, coordinates and implements programming associated with the treatment plan through full knowledge of the client history and the impact on course of treatment.
5. Facilitates the implementation of the treatment plan with the caregivers. Monitors the overall course of treatment services for the client and family. Convenes treatment team meetings as scheduled or as needed.
6. Performs case management and communicates with identified partners such as Department of Children and Families and Guardian Ad Litems.
7. Attends and participates in all treatment plan reviews, peer reviews, psychiatric consultations, internal/external utilization meetings, and/or any client review meetings requiring the therapist's input.
8. Facilitates a therapeutic transition and a positive placement for each client.
9. Attend scheduled psychiatric appointments with clients on Doctor's Day
10. Works within the philosophy, function and Human Resources practices of the Children's Home Network
11. Attends agency and required training as directed.
12. Prepares and conducts agency-wide or program specific training as directed.
