The Medical Social Worker collaborates with the multidisciplinary team to provide input in the development of the plan of care for those patients requiring social work intervention. The Medical Social Worker provides medically necessary intervention to patients who have social or emotional problems that are, or are expected to be, impediment(s) to the effective treatment of the medical condition and/or rate of recovery. Additionally, the Medical Social Worker provides these services in accordance with physician's orders.
Essential Duties and Responsibilities:
Provides medically necessary social work intervention in accordance with physician's orders
Assesses patient's psychosocial needs; plans for, evaluates, and modifies interventions to meet the established patient goals
Collaborates with other disciplines to plan and evaluate team goals for each patient
Facilitates patient referrals to community agencies when appropriate
Provides financial counseling when necessary to achieve the treatment goals established by the multidisciplinary team
Assists patient/family/caregivers in planning for present and future psychosocial needs related to the patient's current status and prognosis, including terminal illness
Adheres to company policies and procedures while providing services and preparing/submitting required paperwork
Observes and reports patient progress and/or changes in patient status to physician and appropriate multidisciplinary team members
Functions as a resource person for other members of the health care team, providing inservice education and consultation in understanding the social and emotional factors related to health problems.
Maintains knowledge of the field including Medicare, Medicaid, and other third-party reimbursement sources.
Participates in planning and implementing quality improvement activities as necessary
Provides positive experiences in assisting patients, families, physicians, other departmental employees, referral resources, service agencies, and others.
Adheres to facility and Amergis policies at all times
Minimum Requirements:
Baccalaureate degree in Social Work (BSW) or Master's degree from a school of social work accredited by the Council on Social Work Education per state/client requirements required
Minimum of one (1) year of social work experience in a health care environment in the last two years preferred
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
Understand patient confidentiality and HIPAA requirements
Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required
Computer proficiency required
Must be at least 18 years of age

ACT provides ongoing care to individuals who frequently experience episodes of homelessness, hospitalizations, and/or incarcerations. This ACT Behavioral Health Therapist II provides treatment interventions in a person-centered and patient-driven manner.
Responsibilities include crisis intervention, assessment, treatment planning, care management, individual and group therapy, and coordination of treatment services. Complies with federal, state and agency policies and procedures. Demonstrates the ability to function as a member of a busy, mobile clinical team utilizing advanced clinical skills. Will be required to transport individuals in program vehicles.
Schedule includes days, evenings, overnights, weekends, and holidays. This position has On-Call rotation responsibilities and eligible for Emergency Overtime and On-Call pay and is eligible for a $6,000 hiring bonus in accordance with the County's Hiring and Retention Bonus Program.
Minimum Qualifications
Master's degree; license eligible within one (1) year of hire or possess professional licensure through the VA Department of Health Professions Board as a: Licensed Clinical Social Worker (LCSW); Licensed Professional Counselor (LPC); Licensed Marriage and Family Therapist (LMFT); Licensed Clinical Psychologist (LCP). Job Summary:

Sufficiently mobile to travel outside the office for client contact, court appearances, community events, and to conduct family assessments and investigations of allegations of child abuse and neglect. Ability to communicate clearly and concisely, both orally and in writing. Ability to use automated technology. Must be able to operate a motor vehicle and make use of personal vehicle. Ability to lift up to 40 lbs. All duties performed with or without reasonable accommodation.

The pay range for this position is $70.019.40 minimum to $84,872.00 maximum. Pay for this position is determined on a number of factors, including but not limited to, license, years and level of related experience.
