Contacts all other disciplines involved in home health patient's care with update within 48 hours of admission (or sooner if patient's condition warrants it) as well as ongoing. Ensures there is appropriate follow-up with MD if there are discrepancies with medications or treatment protocols. Updates the MD with changes in home health patient's condition, including such things as patient blood level results; vital signs or weights that are outside established parameters; compliance issues; etc. Demonstrates effort is put forth in ensuring patient hospitalizations are avoided by exhibiting exemplary professional judgment and by doing such things as: making extra therapy visit(s) as warranted; contacting MD of any significant changes and ensures there is follow-up with the MD when there is no response; following up with the patient and/or caregiver timely by phone or visit when there is initiation of a treatment or medication change, etc. Exhibits sound professional judgment which positively affects individual patient outcomes, as evidenced by quality improvement reports. Implements new physician orders timely and accurately. Assesses for and provides patient and family teaching related to patient's health situation on an ongoing basis (i.e., medications, disease processes, treatments, etc.) and provides written teaching information as appropriate. Ensures supervisor or clinician covering the office is notified if call is placed to physician with update in case there are call-backs and office staff follow-up is necessary. Updates supervisor regarding status of patients on regular basis, including a consultation prior to any discharges, as appropriate. Completes comprehensive assessment for home health admits, and it requires minimal corrections by supervisor when reviewed. Documents towards patient goals based on home health physical therapy POC at each visit. Completes required home health recertification paperwork within allotted 5-day window. Supervises home health aide at least every two weeks and documents supervision. Documents coordination of care of assigned case load with all other disciplines involved in patient's care weekly. Ensures all hospice visit notes are completed in computerized system within 24 hours of visit. Ensures all documentation included in the hospice medical record is thorough, complete and accurate, requiring minimal corrections from supervisor when reviewed. Turns MD verbal orders into office within 24 hours of receipt for processing and mailing. Completes medical supply worksheets timely and accurately. Completes payroll paperwork timely and accurately. Turns in visit schedule to office by 3:30PM each Thursday for the following week and updates schedule as it changes. Determines frequency of visits for patients and receives minimal recommendations for changes from supervisor when reviewed. Develops and updates patient goals which require minimal corrections from supervisor when reviewed. Makes initial assessment visit to patient to determine appropriateness for home health services. Questions when an admission may not be appropriate by contacting PT Coordinator for direction.     The Unit Technician is a paraprofessional who acts under the direction of a physician, RN, or LPN to provide direct patient care, clerical support and other functions to facilitate patient care. Essential Duties & Responsibilities: Assist with management of patient information to facilitate flow and communication needs within the admission, transfer, and discharge processes (i.e. transfer paperwork). Monitors internal and external alarm systems and initiates communication as needed within the building to maintain staff, patient and visitor safety. (i.e. Severe weather alerts, facilities alarms, fall alert system, etc.) Maintains expertise in internal/external communication systems and completes notifications as delegated for routine daily patient needs (i.e. paging of physicians) as well as for urgent and emergent needs (i.e. RAT calls, BERT Codes or any other overhead paging). Serves as an organizational resource and communication point during episodes of downtime for the electronic medical record system. Utilizes professional customer service skills to give direction and assist internal and external customers with questions and concerns. Under the direction of an RN or LPN collects and documents data to support the patient plan of care, including but not limited to vital signs, blood glucose values and intake and output measurements (may include emptying of drains and catheter devices); monitoring skin for abnormalities or areas of breakdown during provision of routine cares. All above information will be reported to the RN or LPN. Provides support for patient admissions, transfers and discharges including but not limited to initial vital signs, weight and height measurements, management of patient belongings, safety checks and early set-up of safety devices as requested by the Nurse (safety precaution needs). Provides for and documents patients' hygiene needs and other nursing-based interventions (i.e. ADLs and safety checks) throughout their stay in collaboration with the assigned Nurse Caregiver. Works in conjunction with the health care team to ensure all call lights are answered and consistently participates in safety checks as applicable to the unit. Ensures throughout each shift that appropriate communication items and safety equipment are in use in support of fall prevention and safe patient handling. Provides for unit organization and cleanliness, including but not limited to removing portable items and linens from patient rooms following discharge, disinfecting reusable patient supplies per infection control guidelines, and various stocking responsibilities as appropriate to the unit to keep supplies ready for the provision of care. Supportive of family or caregiver involvement in the plan of care, taking into consideration the patients' individual preferences and safety. Actively participates in service area/organization initiatives through committee/project participation and attendance of service area/organization meetings. As a member of the health care team, looks for mechanisms to improve efficiency and patient care delivery. Demonstrates professionalism with a consistently supportive, positive demeanor in workplace interactions. Performs all other related duties as assigned.
