Observe, assess, and document member behavior, mood changes, and progress; promptly report concerns to clinical leadership. Facilitate structured group sessions and workshops supporting community reintegration, employment readiness, and life skills. Collaborate with treatment teams, including Re-Entry Specialists and Peer Coaches, to coordinate comprehensive care and goal achievement. Maintain accurate and timely clinical documentation in compliance with program standards and performance indicators (KPIs). Ensure member and staff safety while protecting confidentiality and securing all records and materials. Participate in regular supervision, training, and program development initiatives to uphold clinical and professional standards. Performing additional duties as assigned to support our mission.
