OPPORTUNITY Central Minnesota Mental Health Center (CMMHC) has a rewarding opportunity to serve as a Clinical Trainee or Therapist within Outpatient Mental Healh (OPMH) Team in our Buffalo location. Provider will work in conjunction with a multidisciplinary team and staff; refer to other programs as needed. Therapy sessions may be one to one, in groups, or in sessions involving family members.

Our Outpatient Mental Health Program offers comprehensive mental health support for individuals, couples, families, and groups of all ages who are navigating challenges such as sadness, stress, anxiety, trauma, or difficulty adjusting to life changes. This therapy-based program is designed to support both short-term life challenges and long-standing mental health concerns. Our services empower clients by providing tools, coping strategies, and insights to enhance emotional well-being and improve functioning in relationships, work, school, family life, and overall self-care.

DUTIES AND RESPONSIBILITIES
Instruct, assist, and support clients in areas such as; interpersonal communication skills, community resource utilization and integration skills, crisis assistance, relapse prevention skills, health care directives, budgeting and shopping skills, healthy lifestyle skills and practices, cooking and nutrition skills, transportation skills, mental illness symptom management skills, household management skills, employment-related skills, and integration into community living.
Under the supervision of the Team Leader, provide evidence-based individual, group, and family therapy interventions.
Make changes in the treatment plan to ensure immediate and appropriate interventions are provided in response to changes in mental status/symptoms or behavior which may put the recipient at risk.
Provide medication monitoring and education regarding health maintenance to encourage a healthy lifestyle. These activities include: assist in maintaining consistent medication regime, deliver medication, discuss purposes and benefits of medications, and report any complaints or distress about medications.
Work collaboratively with a variety of community providers, employers, landlords, and families.
Maintain current knowledge of the mental health service system and the ability to locate information and resources for a variety of client needs within the mental health, medical, housing and social services system.
Participate in daily, mandatory, team meeting as well as all designated team activates.
Follow all current mandated reporter guidelines.
Requirements
WHAT YOU'LL NEED
Clinical Trainee
Master's Degree in Psychology, Social Work, or Marriage and Family Therapy
Working towards licensure as an LP, LPCC, LICSW or LMFT
Clearance from DHS and sexual exploitations screening.
Valid Driver's License and acceptable driving record.
Mental Health Professional
Master's or Doctoral Degree in Psychology, Social Work, or Marriage and Family Therapy
Licensed as an LP, LICSW, LMFT or LPCC
Clearance from DHS and sexual exploitations screening
Valid Driver's License and acceptable driving record.

COMPENSATION
Unlicensed Therapist Target Range - $55,000-$65,000 (based degree level, licensure & experience)
Licensed Therapist Target Range - $70,000-$75,000 (based degree level, licensure & experience)
*In addition to the base salary, eligible individuals can earn monthly and annual bonuses for services exceeding impact targets*

The Registered Behavior Technician 1 is responsible for implementing behavior intervention plans, conducting assessments, collecting data, and assisting the BCBA in developing treatment strategies for individuals with behavioral and developmental challenges. The Advanced RBT demonstrates a higher level of competency, expertise, and decision-making ability compared to a standard RBT.
Essential Functions:
Behavior Intervention: Implement behavior intervention plans developed by the BCBA to address behavioral goals and objectives of clients.
Data Collection and Analysis: Accurately collect data on client behaviors and outcomes, analyzing data to make informed decisions regarding treatment adjustments.
Assessment Support: Assist the BCBA in conducting assessments, including functional behavior assessments (FBAs) and other relevant assessments to inform treatment planning.
Program Development: Collaborate with the BCBA to develop individualized treatment plans that address the specific needs and goals of each client.
Training and Coaching: Provide training and coaching to other team members on behavior intervention strategies, data collection methods, and reinforcement techniques.
Documentation: Maintain detailed and accurate records of client progress, behavior data, and treatment implementation. Prepare and submit progress reports as required.
Crisis Management: Respond appropriately to crisis situations using approved crisis intervention techniques, ensuring the safety and well-being of clients and team members.
Collaboration: Work closely with the BCBA and other team members to ensure consistent and effective implementation of behavior intervention plans and strategies.
Professional Development: Stay current with the latest developments and best practices in the field of ABA by participating in ongoing training, workshops, and professional development opportunities.
Ethical Standards: Adhere to the ethical guidelines and professional standards set forth by the Behavior Analyst Certification Board (BACB) and relevant state regulations.
Creates session notes and converts time within 24 hours
Delivers Treatment to clients 85% of hours worked

Physical Demands:
While performing the duties of this job, the teammate may be exposed to a client that exhibits physically aggressive behavior which can include kicking, pinching, punching, biting, etc. The teammate may have to use appropriate behavior management techniques frequently with a client under such circumstances. Such response requires agility, quick reflexes, and the ability to use physically evasive movement. The teammate will be constantly required to stand, walk, sit, grasp, reach, bend, kneel, crouch and squat. The teammate must be able to independently and repeatedly lift, move and carry objects weighing up to thirty-five (35) pounds. Strong interpersonal communication and teaching skills.

Adoray is looking for a full-time Physical Therapist to join our team. ESSENTIAL DUTIES AND RESPONSIBILITIES
Implements PT plan of treatment.
Contacts all other disciplines involved in home health patient's care with update within 48 hours of admission (or sooner if patient's condition warrants it) as well as ongoing.
Ensures there is appropriate follow-up with MD if there are discrepancies with medications or treatment protocols.
Updates the MD with changes in home health patient's condition, including such things as patient blood level results; vital signs or weights that are outside established parameters; compliance issues; etc.
Demonstrates effort is put forth in ensuring patient hospitalizations are avoided by exhibiting exemplary professional judgment and by doing such things as: making extra therapy visit(s) as warranted; contacting MD of any significant changes and ensures there is follow-up with the MD when there is no response; following up with the patient and/or caregiver timely by phone or visit when there is initiation of a treatment or medication change, etc.
Exhibits sound professional judgment which positively affects individual patient outcomes, as evidenced by quality improvement reports.
Implements new physician orders timely and accurately.
Assesses for and provides patient and family teaching related to patient's health situation on an ongoing basis (i.e., medications, disease processes, treatments, etc.) and provides written teaching information as appropriate.
Ensures supervisor or clinician covering the office is notified if call is placed to physician with update in case there are call-backs and office staff follow-up is necessary.
Updates supervisor regarding status of patients on regular basis, including a consultation prior to any discharges, as appropriate.
Completes comprehensive assessment for home health admits, and it requires minimal corrections by supervisor when reviewed.
Documents towards patient goals based on home health physical therapy POC at each visit.
Completes required home health recertification paperwork within allotted 5-day window.
Supervises home health aide at least every two weeks and documents supervision.
Documents coordination of care of assigned case load with all other disciplines involved in patient's care weekly.
Develops accurate and individualized 485 POC within 48 hours of admission.
Develops accurate, individualized PT treatment plan within 48 hours of eval.
Ensures medication profile is complete (i.e., "new" vs. "changed", start and stop dates, all coumadin doses, IV meds, reason for prn meds, etc.) and consistent with physician's orders.
Documents evidence of updates to physician for home health patients whenever there is a change in status.
Ensures all home health documentation to be included in the medical record (except 485 as noted) is completed in Cerner system within 72 hours of visit.
Ensures all documentation included in the home health medical record is thorough, complete and accurate, requiring minimal corrections from supervisor when reviewed.
Ensures all hospice visit notes are completed in computerized system within 24 hours of visit.
Ensures all documentation included in the hospice medical record is thorough, complete and accurate, requiring minimal corrections from supervisor when reviewed.
Turns MD verbal orders into office within 24 hours of receipt for processing and mailing.
Completes medical supply worksheets timely and accurately.
Completes payroll paperwork timely and accurately.
Turns in visit schedule to office by 3:30PM each Thursday for the following week and updates schedule as it changes.
Determines frequency of visits for patients and receives minimal recommendations for changes from supervisor when reviewed.
Develops and updates patient goals which require minimal corrections from supervisor when reviewed.
Makes initial assessment visit to patient to determine appropriateness for home health services. Questions when an admission may not be appropriate by contacting PT Coordinator for direction.
Determines need for other disciplines (aide, SN, OT, SpT, SW) or services, including telehealth, and obtains order and notifies office staff.
Meets productivity standards as annually established.
Participates in case conferences/care conferences as requested.
Supervises PTA in delegated therapy functions per procedure.
Maintains knowledge of current trends in home health and hospice care.
Independently seeks educational opportunities to maintain and upgrade individual knowledge base.
Always gives thorough verbal and/or written report to other staff members' who will be providing planned care patients in this case manager's absence
Other duties as assigned.
Requirements
Minimum Qualifications
Graduate of a physical therapy program with curriculum approved by the American Physical Therapy Association or equivalent (as outlined in the Federal Register defining qualifications).
Minimum of two years of clinical experience preferred.
Current Wisconsin Physical Therapist license required, and Minnesota preferred.
Excellent communication skills.
Working knowledge of community resources.
Working knowledge of Hospice and Home Health.
Ability to work autonomously and make independent judgments.
Ability to creatively problem solve.
Evidence of attendance of hospice training class or willingness to attend.
Basic computer skills.
