Education Qualifications: • Bachelor's Degree, Master's Degree, or Doctorate Degrees from a physical therapy accredited program Key Responsibilities: • Provide therapy evaluation, develop individualized treatment plans, implement evidence-based interventions, provide patient/caregiver education, and discharge planning for a full patient caseload. • Provide all ancillary/administrative requirements for a full caseload of patients, including documentation, billing, scheduling management, and other administrative duties • Demonstrates appropriate communication, professionalism and supervision of support staff (licensed assistants, aides, volunteers) • Collaborate with a multidisciplinary care team to ensure optimal clinical and financial outcomes. • Work closely with Essentia Health leadership and coordinate with referring physicians to ensure cohesive patient care. • Comply with organizational policies and procedures and the code of conduct • Meet professional organization core values, code of ethics, &/or scope of practice • Work where the patient need is highest (including flexing to other departments) when home department schedule allows • May serve as a clinical instructor, participate in department or therapy discipline committees, and complete credential/certification that would benefit patient care Rural Healthcare: This is primarily an outpatient physical therapy position with a Saturday on-call rotation in inpatient care within the Deer River Hospital. Deliver high-quality care to patients (both adult & pediatric) with varying needs and conditions. There is a $20,000 bonus for this position Schedule for this role is generally Monday - Friday 8:00am - 4:30pm. On-call Saturday rotation for Deer River Hospital is once every six weeks. Cultivate working relationships with potential service collaborators, partners, and other appropriate stakeholders. Cultivate working relationships with CLUES staff within and across departments, to support one another's work, facilitate client referrals, and improve client flow throughout the agency. Participate in care coordination with other providers who may be supporting clients in different capacities. Contract and Program Compliance Ensure goals for weekly billable client contact are met. Maintain up-to-date clinical documentation per department standards and all requirements. Ensure client data is up to date to facilitate timely processing of service lines for billing purposes. Adhere to contract expectations related to the provision of mental health therapy and related activities. Respond to all internal & external data collection & reporting requests. Participate in all internal and external activities required as stated in program contracts. Assist in the delivery of contracts outcomes and relationships with contractor stakeholders. General Maintain client confidentiality in accordance with HIPAA, agency policies and other applicable laws, regulations and ethical standards. Contribute to the collaboration of the Behavioral Health department with other programs within CLUES to achieve service integration. Participate in weekly supervision meetings, biweekly team/group meetings for Behavioral Health, "All Staff" meetings as scheduled, and other meetings as assigned. Follow the code of ethics of the applicant's professional board. Carry out all other duties as assigned by supervisor. QUALIFICATION REQUIREMENTS Education, Experience & Key Abilities Master's degree or doctoral degree in Social Work, Counseling, Marriage and Family Therapy, or Psychology from an accredited university required. Licensed as a social worker (LGSW or LICSW) or working toward clinical licensure as a marriage and family therapist (LAMFT/LMFT), professional counselor (LPC/LPCC), or psychologist (LP) in the State of Minnesota required. With clinical licensure, must be eligible to be credentialed with all major insurance companies. Must have a deep understanding of Latine cultures, values, and traditions, and demonstrate cultural responsiveness in every aspect of the role, including communication with colleagues, staff, clients, community members; creating forms and documents; hiring and training staff; and in clinical supervision and consultation. Language Skills Effective communication in Spanish and English, both verbally and in writing, required. Ability to read, analyze, and interpret written materials, including reports, contracts, and proposals. Ability to effectively present ideas and information in an organized and effective manner, including writing reports. Ability to maintain accurate documentation and respond appropriately to questions from clients, co-workers, management and external parties. Computer Skills Proficient working knowledge of email software, the Internet, databases, and common Windows-based programs, including Microsoft Office Word, Excel and PowerPoint required. Must be willing to attend training to advance technical skills and maintain appropriate skill levels to perform job duties.
