This position involves providing crisis intervention for individuals with Serious Mental Illness as part of a team. The purpose of the team is to provide immediate, focused services in the community on an as-needed basis to prevent hospitalization, assist with de-escalating crisis situations, and to prevent unnecessary admissions to jails and emergency rooms. This team operates primarily 3p-11p, with flexibility of hours as needed. DESCRIPTION OF DUTIES: Assess consumers and individuals in crisis and implement clinical and care coordination services. Coordinate with MAMHA team members and community resources to link individuals in crisis to the appropriate level of care.
