SUPERVISOR: Director of Crisis Outreach Services
DEFINITION: Position may be contract, hours will be discussed in interview. This position involves providing crisis intervention for individuals with Serious Mental Illness as part of a team. The employee will be a member of a two-person team which will provide time limited, home-based services. The purpose of the team is to provide immediate, focused services in the community on an as-needed basis to prevent hospitalization, assist with de-escalating crisis situations, and to prevent unnecessary admissions to jails and emergency rooms.
This team operates primarily 3p-11p, with flexibility of hours as needed.
DESCRIPTION OF DUTIES:
Assess consumers and individuals in crisis and implement clinical and care coordination services.
Coordinate with MAMHA team members and community resources to link individuals in crisis to the appropriate level of care.
Provide direct in-home intervention services to include individual and family therapy and case management.
Perform the function of a mental health therapist by providing mental health services individually. Services include evaluation/assessment, individual and group therapy/counseling, medication monitoring and education, family services, and other clinical services, focusing on mental disorders, environmental stress and family relationships.
Provide mobile crisis services to consumers.
Participate in staffing of all assigned cases on a regular basis with other treatment team members.
Participate in clinical supervision as required.
