Job Summary:
The Physical Therapist is responsible for the development of an individualized therapy program consisting of physical assessment, treatment, planning, implementation, education, and communication to maximize a patient's progress toward achieving functional goals. The Physical Therapist will also maintain a positive working relationship with other health professionals and ancillary staff.
Duties/Responsibilities:
· Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists. Reviews patients' functional needs and adjusts care plans to ensure optimal patient outcomes.
· Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by significant others whenever it seems appropriate.
· Conducts therapy practice within defined standards of care.
· Directs the assistance of support staff to achieve patients' functional goals.
· Communicates effectively with other professional and support staff to achieve positive patient outcomes.
· Practices facility and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards.
· Documents evaluations, treatment goals, and plans. Regularly updates documentation and maintains prescriptions and signatures.
· Enhances professional growth and development of self and others through participation in educational programs, in-service meetings, and workshops. Keeps up to date on professional literature.

Supervises Physical Therapy Assistants according to the company's policies/procedures and state regulations
Provides/submits accurate and timely documentation in accordance with the plan of care and company policy and procedures
Complies with applicable local, state, and federal laws and regulations regarding professional standards and practice standards
Performs other duties as assigned/necessary
Minimum Requirements:
Current licensure as a Physical Therapist in the state of practice
Graduate of an accredited school of Physical Therapy
One (1) year of prior professional Physical Therapy experience preferred
Complies with all relevant professional standards of practice
Participation and completion of Amergis' Competency program when applicable
Current CPR if applicable
TB Questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Successful completion of new hire training as applicable to job site
