The ideal candidate will provide high-quality rehabilitative care to residents, helping them regain mobility, strength, and independence. This role involves assessing patient needs, developing individualized treatment plans, and working collaboratively with an interdisciplinary healthcare team.

Physical Therapist Key Responsibilities:
Evaluate residents' physical conditions and functional abilities through assessments.
Develop and implement personalized treatment plans to improve mobility, strength, balance, and overall function.
Provide hands-on therapy techniques, exercises, and interventions tailored to residents' needs.
Educate residents, caregivers, and families on techniques to enhance recovery and prevent further injury.
Monitor patient progress, adjust treatment plans as necessary, and document therapy sessions accurately.
Collaborate with physicians, nurses, occupational therapists, speech therapists, and other healthcare professionals to optimize patient outcomes.
Ensure compliance with state and federal regulations, as well as facility policies and procedures.
Maintain accurate and timely records in the electronic medical record system.
Participate in discharge planning and recommend appropriate adaptive equipment or continued therapy services.
Physical Therapist Qualifications:
Education: Doctorate (DPT) or Master's Degree in Physical Therapy from an accredited program.
Licensure: Active Physical Therapist license in New York State or eligibility to obtain one.
Experience: Prior experience in a skilled nursing facility or geriatric rehabilitation preferred but not required.
Skills: Strong communication, problem-solving, and patient-care skills. Ability to work independently and as part of a team.

The purpose of a Trauma Systems Therapy (TST) Therapist in Preventive Service Department is to provide the TST model for families with child(ren) (ages 5 and over) who are impacted by trauma and there may be risk for foster care placement. The Preventive Services Department provides child welfare services using Evidence Based models to ensure that families are able to provide safe environments for their children, keep children safe in their homes and reduce the need for foster care. All services are provided in a family's home and community.
POSITION OVERVIEW:
The Therapist works with families to assess their needs, develop intervention plans, and deliver therapy sessions that are tailored to the family's unique circumstances. Assists in the maintenance and cleanliness of the department and equipment.
Provides supervision to assistants and aides/techs.
Completes weekly reports and patient summaries.
Participates in weekly meetings.
Updates and renews patient orders and plan of care as indicated.
Participates in monthly meetings.
Assures complete monthly billing.
Renews patient orders and updates plan of care as indicated.
Participates in quarterly quality improvement initiatives as Directed by DOR.
Participates in quarterly reviews of long-term residents as directed by DOR.
Participates in annual review.
Assists DOR in annual review of other staff as appropriate.
Performs other duties as required.
Qualifications:
Must possess a current, unencumbered license to practice as a licensed Physical Therapist in the State of New Jersey.
CPR certification preferred.
Must be able to lift a minimum of 50 pounds without assistance.
Schedule: 8am-4pm, Monday - Friday.
Education:
Graduate of an accredited college or university with a bachelor's degree in physical therapy.
Must possess a current, unencumbered license to practice as a licensed Physical Therapist in the State of New Jersey.
CPR certification preferred.
Previous Experience: No previous experience required.
