Essential Job Functions: Perform initial and ongoing assessments to evaluate mobility, coordination, strength and current level of function. Ensures that plans developed are appropriate to the ages and populations served and includes input from the client and family. Provides hands-on support, training, and mentorship to families. Delivers therapeutic services using a multi-model approach on an individual and family basis to include when appropriate, behavioral modification techniques. Service delivery may occur in-home, in-school, in-office, or in the community based on family need. Schedules regular visits with assigned client/family. Provides on-going assessment, evaluation, treatment, discharge planning and other services as required.
