A Brief Overview
Responsible for the daily coordination and delivery of physical therapy services implementing specific treatment program for individual patients and patient groups according to the principles and practices of physical therapy and established objectives of the department. What you will do
Evaluates assigned patients, gathering data from a variety of sources such as patient records, interviews, observation and team members. Develops and implements individualized treatment programs for the patients needing services.
Uses selected activities and/or tasks to promote and maintain health, and to achieve treatment goals, establish and modify intervention and strategies for patients' treatment program based on ongoing evaluation.
Provides direct therapy services to patients using various functional settings and strategies, plans therapy within the patient's individual physical, emotional and intellectual capabilities as indicated and coordinated with the rehabilitation, medical, and nursing care plans.
Prepares documentation as required by the profession and the department, such as evaluation results, individualized treatment plans, progress notes, and other such reports.
Communicates effectively with the patient, family, physician, and other disciplinary team members regarding the patient's status, progress and needs.
Establishes rapport and work cooperatively with interdisciplinary team members (including family/physician) throughout the patient's intervention program.
Enhances professional growth and development through participation in educational programs, current literature, in-service meetings, workshops, etc. Shares new knowledge with co-workers.
Participates in program development of the department.
Evaluates and monitors clinical internship with students affiliating in various universities/colleges.

Job Summary
The Mental Health Counselor or Mental Health Therapist coordinates, plans and delivers the best psychotherapeutic approach for each patient. Duties
* Apply biopsychosocial evaluations to propose behavioral health interventions.
* Provide evidence-based treatment, including behavioral, cognitive, and motivational approaches.
* Provide primary care providers and staff regular and concise feedback on the status of referred patients, to include recommendations for team-based care.
* Implement psychoeducational classes to target high frequency diagnoses found in the primary care clinic.
* Document information in the electronic health record during patient encounters.

Job Description
Psychologists are Licensed Independent Providers who engage in professional and scientific work that relates to behavior, capacities, personality, interests, emotions, and mental disorders. This work may include any of the following:
Education of patients, training activities, and program evaluation activities.
The incumbent diagnoses mental disorders, conducts psychological or neuropsychological assessments. The major duties and responsibilities include but are not limited to:
Provides a full range of psychological assessment services, provides education to patients, family members, and significant others.
Provides consultation to professional staff within Trusted Medical concerning clinical assessment findings.
Develops, implements, and documents the psychotherapeutic treatment plan for an assigned caseload of patients, including prompt completion of clinical reminders.
Functions independently as a member of the Trusted Medical Clinical Team. Assumes complete professional responsibility for his/her clinical assessment findings, and documentation.
Adheres to operational policies and procedures established by the Clinical Operations Lead.
Duties include the following.
Prepare for and deliver exceptional in- clinic, face- to- face Veteran care during all interactions
Complete and submit all required documentation expeditiously and accurately, including DBQs in the required format
Provide delivery and operational excellence by performing exceptional customer service, working collaboratively with multi-disciplinary teams, embrace change with agility and adaptability, and maintain accountabilities for all assigned commitments
Apply clinical skills to include data gathering and analysis, communications with an emphasis on relationship development and growth, and demonstration and application of knowledge and expertise in specialized areas
Participate and contribute to all work requirements and organized meetings, sharing points of view and recommendations. Support our organization by practicing fiscal responsibility as well as maintaining compliance with policies and regulations. Further assist by contributing to operational developments, process improvement initiatives, and strategic planning.
Education:
PsyD or PhD
Technological Skills:
Experienced in one or more major Electronic Health Records
Microsoft Office (Word, Excel, Outlook, PowerPoint, and Teams)
Required Knowledge and Skills:
Knowledge of and ability to apply professional psychological treatments to the full range of patient populations.
Ability to provide professional advice and consultation in areas related to professional psychology and behavioral health.
Knowledge of clinical research literature.
Preferred Experience: The incumbent will have experience performing Disability Medical Exams for Veterans.
Work schedule:
Monday through Friday 8:00am-6:30pm (4, 8, or 10-hour shifts)
No weekends
