The Occupational Therapist in Home Health is responsible for the assessment and evaluation of patient care needs related to functional status, activities of daily living, fine motor coordination, home assessments and adaptive equipment, and other occupational therapy needs as defined by medical conditions.
Provides service within the scope of practice as defined by the state laws governing the practice of Occupational Therapy, in accordance with the plan of care, using evidence-based techniques, and in coordination with other members of the health care team.
Evaluates the patient's functional status, status of all body systems as required for CMS documentation, and occupational therapy needs. Consults with the physician in the development of the therapy plan of care.
Observes, records, and reports to the nurse supervisor and the physician the patient's response to treatment and changes to the patient's condition.
Conducts patient assessments evaluating the level of function by applying diagnostic and prognostic functional ability tests.

Oregon State Hospital, a Joint Commission and CMS certified public psychiatric hospital, provides psychiatric hospital services and patient treatment services to persons committed by the Oregon courts as part of the state mental health system.
The Oregon State Hospital Salem Campus is a 610-bed facility supported by approximately 2,000 staff.
The CTRS provides individualized treatment to patients based on their discipline specific education, training, licensing, and board certification in Recreation Therapy.
The CTRS completes clinical assessments, develops clinical interventions to meet individualized persons served treatment needs, creates program and treatment goals, and provides rehabilitation-focused clinical services to persons served.
The CTRS completes clinical assessments in accordance with Oregon State Hospital, specific discipline, and professional standards within timelines established by policies and protocols. They participate in the development of program plans based on persons served needs and goals. They provide discipline specific and evidenced-based interventions that support individualized treatment plans and coordinate those interventions with overall clinical delivery of treatment services. They evaluate the effectiveness of treatment plans and track persons served progression or regression towards goals.
The CTRS reassess persons served treatment goals by modifying, adapting, and progressively updating the treatment plan to report out to the interdisciplinary group as an active member of the team representing the Treatment Services Department.

Track and document in Electronic Medical Records persons served progress or regress toward their individual treatment goals.
Reassess persons served treatment goals by modifying, adapting, and progressively updating treatment care plans.

EXPERIENCE AND TRAINING
* Must possess, or obtain within six (6) months of hire, a current MHACBO (Mental Health & Addiction Certification Board of Oregon) CADC I (Certified Alcohol and Drug Counselor) or higher. The CADC I certification requires education, supervised experience hours and successful completion of a written examination. See MHACBO website for requirements.

KNOWLEDGE, SKILLS AND ABILITIES
Working knowledge of the general concepts of substance use and mental health disorders; knowledge about community resources.
Skills and abilities: delivery of addiction counseling and evaluation and individual, group, and family counseling techniques; making referrals for persons experiencing domestic violence, crisis, mental health challenges, etc.; keeping privileged information confidential; establishing and maintaining positive working relationships with other employees, persons served and their families, other agencies, and the public; following directions in a professional manner; preparing documentation of services in a timely and professional manner.
PHYSICAL REQUIREMENTS
Stands; sits; moves about the work area; bends forward; climbs 1 floor of stairs; reaches overhead; lifts up to 10 lbs.; operates a keyboard; speaks with a clear & audible voice; distinguishes colors; hears a normal speech level; uses required Universal Precautions when dealing with any hazardous situations of coming into contact with body fluids.

Job Requirements:
