Reviews patients' functional needs and creates Physical Therapy care plans to ensure optimal patient outcomes. Designs and implements a plan of care for patients based on a thorough assessment of potential development and incorporates participation by others when appropriate. Adapts patient's care plan and care techniques as needed based on patient's progress towards functional goals. Conducts therapy practice within defined standards of care. Directs activities of the physical therapist assistant in accordance with the care plan to achieve patients' functional goals. Practices organization and departmental policies and procedures, objectives, quality improvement program, safety, environmental, and infection control standards. Documents evaluations, treatment goals, and plans. Regularly updates documentation based on patient progress.
