_Case Management, Hospice Education and Leadership of HOA Inter-Disciplinary Team:_
- Leads and supervises the interdisciplinary team which provides clinical care to all In-Home clients and ensures that care plans, approved by the client/family are developed, revised and implemented in a timely manner.
- Completes initial comprehensive bio-psycho-social assessments that include physical, emotional, financial, cognitive, and social needs of the client.
- Completes an individualized plan of care in collaboration with other clinical staff members, the client and their families, that includes home safety and fall prevention measures.
- Provides assistance to clients in gaining access to needed medical, social, educational, and other programs and services as needed.
_Bereavement Program Management:_
- Develops, implements and tracks HOA's bereavement program in accordance with State of Alaska Volunteer Hospice regulations.
- Provides 13 months of bereavement support via phone and mail for deceased client's families.
- Provides grief support for community members.
- Organizes and conducts community grief support events, as needed.
- Leads educational presentations for staff, volunteers, professionals, and community groups on grief and end of life topics.
- Conducts bereavement evaluations in accordance with Senior In-Home grant requirements.
_Outreach:_
- In conjunction with the Executive Director, leads HOA outreach community events and presentations; develops and maintains good working relationships with a variety of partners, individuals, community agencies/organizations, and governmental agencies in order to promote interagency cooperation and to facilitate appropriate referrals.
_Reporting:_
- Serves as Primary Care Manager and Clinical Manager for the Senior In-Home Grant: Completes assessments, maintains accurate records, completes and submits timely reports and narratives, and follows all rules and regulations of that program.
- Provides the Board with periodic reports, via the ED, regarding the services provided and recommendations of changes to, or additions of, program services.
_Compliance:_
- In conjunction with the ED, ensures compliance by HOA to all local, state, and federal laws and standards including but not limited to: Department of Labor, OSHA, Alaska State law and regulations, hospice care standards and non-profit accounting standards.
- In conjunction with the ED, monitors and complies with the Board approved budget and cash flow realities.
- In conjunction with the ED, reviews all policies, procedures, and programs to ensure they are current and meet applicable standards, laws, and regulations.
- In conjunction with the ED, provides the Board with information necessary to facilitate Board decisions, including the needs of the community and HOA.
- Implements a quality control program that ensures effective and efficient delivery of all clinical programs; corrects identified problems and revises policy or procedure as needed.
- Ensures compliance with all clinical GEMS reporting requirements.
_Miscellaneous:_
- Attends/participates/leads in staff meetings and other meetings as assigned and appropriate to include but not limited to: in-service programs, outreach events, development meetings and fundraising meetings.
- Informs Resource Center Program Manager of community needs in order for HOA to best supply our library and loan closet.
- Supports HOA fundraising and development efforts.
- Performs other duties as assigned.
*QUALIFICATIONS:*
- Bachelor's degree in social work, psychology, sociology, human resources or a closely related field with a background in gerontology, long-term care, behavioral health, medical social services, care coordination or other direct social services preferred.
- Master's degree in social work mandatory.
- LMSW or LCSW preferred.
- Minimum two full-years' paid work experience providing direct social services preferred.
- At least two years of paid experience as a supervisor of one or more staff (working in a human services field or setting) in a position with responsibility for planning, development, management or operation of programs and service delivery, fiscal management, needs assessment, program evaluation and similar tasks preferred.
- Working knowledge of hospice and/or palliative care; knowledge of, and commitment to, the mission of Hospice of Anchorage and willingness to function as part of Hospice of Anchorage's interdisciplinary team.
- Working knowledge of laws, rules, regulations and precedents governing volunteer hospice service and activities: the Individuals with Disabilities Education Act (IDEA), The Health Insurance Privacy and Portability Act (HIPPA) and the Americans with Disabilities Act (ADA) including terminology used.
- Knowledge of community resources for seniors, disability, chronic and acute medical care, housing, long-term care, and other local and national supportive services/resources including entitlement programs.
- Knowledge of medical, behavioral, habilitative and rehabilitative conditions and requirements of the HSB Waiver Services program's population.
- Ability to conduct comprehensive assessment of treatment needs for individuals experiencing physical, cognitive and/or intellectual disabilities.
- Must be able to pass background check and fingerprinting.
- Must maintain and provide proof of CPR and First Aid Training.
- Must provide own transportation to/from work including but not limited to patients' residences, health fairs, and off-site meetings. The Case Manager RN coordinates and facilitates patient care across the healthcare continuum, ensuring optimal use of resources and improved patient outcomes. They assess patient needs, develop comprehensive care plans, and collaborate with multidisciplinary teams to provide efficient, cost-effective, and high-quality patient care.
Essential Work Functions:
Assess patients' medical, psychosocial, and functional needs
Collaborate with interdisciplinary teams, including physicians, therapists, social workers, and case managers, to develop patient-centered care plans
Develop and update individualized care plans, aligning with patient goals and clinical guidelines
Coordinate transitions of care, ensuring seamless services and resources across the continuum of care
Facilitate communication between patients, families, and interdisciplinary team members
Monitor and evaluate patient outcomes, and adjust care plans as needed
Ensure compliance with regulatory standards and insurance requirements
Advocate for patients' preferences, addressing barriers to care and promoting equitable access to services
Conduct utilization reviews and manage length of stay, ensuring adherence to evidence-based guidelines for care delivery
Educate patients and families on health conditions and available community resources
Document case management comprehensively and accurately in the patient's medical record
Participate in interdisciplinary team meetings
Perform other duties as assigned within scope of the position
Adhere to hospital safety protocols, infection control guidelines, and regulatory standards
Required Essential Skills:

About the Role
The Inter-Island Ferry Authority (IFA) is seeking dedicated maritime professionals to join our team as Mate/Relief Master. These positions are critical leadership roles aboard IFA vessels, providing safe navigation, strong crew leadership, and ensuring the safety and satisfaction of every passenger who travels with us.
As part of a close-knit crew, you'll work a steady 4-on/4-off schedule, gain experience in the challenging and beautiful waters of Southeast Alaska, and play a key role in providing a reliable transportation link that supports both residents and visitors.

Key Responsibilities
Mate
Serve as the second in command, assuming command if the Master is incapacitated.
Assist with safe navigation of the vessel; stand as Officer in Charge of Navigational Watch (OICNW).
Supervise and direct deck crew during departures, landings, and loading/unloading.
Monitor and maintain safety and lifesaving equipment.
Ensure compliance with state and federal security and operational requirements.
Act as first responder in medical and emergency situations.
Promote a positive, safety-first work culture onboard.
Relief Master (Captain)
Serve as the highest-ranking officer aboard IFA vessels.
Safely navigate in diverse weather and tidal conditions, day or night.
Lead all pre-sail checklists, departures, and landings.
Ensure compliance with all USCG and CFR requirements, drills, and training.
Oversee vessel logs, security plan, and ADA compliance.
Lead and motivate crew while ensuring discipline, safety, and professionalism.
Act as Vessel Security Officer and ADA Complaint Resolution Official.
Review crew payroll and assist with evaluations in coordination with the Operations Manager.
Qualifications & Credentials
Required USCG License: Master of Motor Vessels of 100 Gross Tons or Larger Upon Inland Waters.
Vessel Security Officer (VSO) Endorsement.
Radar Observer Endorsement.
FCC Marine Radio Operator Permit.
TWIC Card (required).
Current First Aid/CPR Certification.
Valid State Driver's License.
Knowledge & Skills
Deep knowledge of Southeast Alaska waterways, tides, currents, and weather conditions.
Strong understanding of federal/state maritime security and vessel operations requirements.
Ability to lead and command crew during both routine operations and emergencies.
Excellent communication and leadership skills, fostering professionalism and positive morale.

Uniform & Professional Standards
IFA crew members represent both our vessels and our community. All crew are expected to maintain a professional appearance and wear the IFA uniform (black pants with IFA shirt/vest/jacket, optional hat).
Travel Pediatric Physical Therapist - PT
Anchorage, AK
Dynamic and growing pediatric outpatient therapy clinic is in search of an energetic and skilled physical therapist to join their team on a full time basis. The Wasilla-Palmer area is located in the Mat-Su Valley, about 40 miles outside of Anchorage. This position would be a split between the Palmer and Wasilla offices.
* You' ll work with an all pediatric caseload in a large up to date facility
* Much of the caseload involves development delays in gross motor and motor coordination
* Many of the kiddos are in foster homes
* Clinic is known for helping with feeding and sensory integration
* Multiple clinic locations in the area; they offer speech, OT and PT services
Requirements:
*Alaska license to practice as a PT or eligible
*Strong written and oral communication skills
*Ability to communicate successfully with caregivers, guardians and parents to achieve best outcomes for your patients
*Willingness to travel between clinics as needed and to work in a telehealth format if required
*Ability to evaluate patient, plan, develop and execute effective treatment plans; goal oriented
*Previous pediatric experience preferred (fieldwork counts!)
Qualifications: You will need to have a Doctorate Degree (or equivalent) in Physical Therapy and a current state license (or be eligible for same).
