An Illinois State Board of Education certified therapeutic day school, Thresholds High School serves students ages 16 to 21 years old with ongoing serious mental health needs. Operating year-round, the school provides a distinct combination of flexibility and support to help students achieve both their high school diploma and their transition goals. Students also benefit from the Young Adult Program's rich therapeutic supports that address anxiety, frustration, school refusal, and other issues that derailed their academic success in other schools. As the lead teacher in the classroom, you will educate a class of up to 8 students. You will develop a course of study according to curriculum guidelines of the school district and state requirements. These lesson plans will be tailored for three or more levels of academic ability and previous achievement, and in alignment with the students' IEPs. You will supervise the students in the classroom and instruct the class each day with the assistance of a teacher's aide. You will monitor and grade student performance, supporting the youth in their academic achievement, graduating, and preparing for their next steps. The Transition Coordinator serves as the primary transition facilitator for youth and young adults who are discharging from the Young Adult Residential and Transitional Living Program. They provide the coordination of mental health treatment, housing, benefits, and linkage to other services and resources that will be needed post-discharge. They also assist in skill-building through coaching and one-on-one work with young adults as they prepare for the transition. ESSENTIAL DUTIES & RESPONSIBILITIES Provides direct service to members through transition, including case management, crisis intervention, and psycho-social rehabilitation services. Develops and implements the transition/discharge plan, in cooperation with other members of the transition/treatment team. Connects member with community and Thresholds resources, including appropriate housing, mental health and psychiatric services, physical health services, public assistance benefits, resources for social/leisure activities, and developing connections to family and others. Provides direct assistance to help members move, obtain household items, and explore their new community at time of discharge from residential program. Maintains contact with members post-discharge from the residence to provide continuity of care and track outcomes. Primary contact between agency, families, and other collateral resources to assist with members' transition goals and needs. As a Crisis Worker, you will respond to mental health crises in the Uptown, Lakeview, and North Center communities, acting as an alternative to or in conjunction with hospitalization or police response. You may connect the caller to an appropriate service or provider, or if a face-to-face intervention is necessary, you and a Peer Engagement Specialist will meet the caller at home, their workplace, or another location in the community. As a pair, you will assist the person in crisis with the least restrictive intervention possible. This can look like anything from walking someone through a panic attack to assisting them with hospitalization. Community interventions occur year-round, in all weather conditions. The team may also distribute basic necessities to individuals in crisis, such as first aid items, water, or warm clothes. After responding to the crisis, you will offer a follow-up to the client based on their needs as well as document the services provided. During downtime between calls, you will have the opportunity to focus on professional development through self-directed projects and community outreach efforts. You may connect the caller to an appropriate service or provider, or if a face-to-face intervention is necessary, you and a Peer Engagement Specialist will meet the caller at home, their workplace, or another location in the community. As a pair, you will assist the person in crisis with the least restrictive intervention possible. This can look like anything from walking someone through a panic attack to assisting them with hospitalization. Community interventions occur year-round, in all weather conditions. The team may also distribute basic necessities to individuals in crisis, such as first aid items, water, or warm clothes. After responding to the crisis, you will offer a follow-up to the client based on their needs as well as document the services provided. During downtime between calls, you will have the opportunity to focus on professional development through self-directed projects and community outreach efforts. The Intern, Community Mental Health collaborates with Program Directors, Team Leaders, and Community Support Specialists to provide direct service to members. This includes but is not limited to group counseling/therapy, individual counseling/therapy, case management, teaching, assessment, assertive community treatment, job support, treatment planning, crisis intervention, and psycho-social rehabilitation services. (This internship is designed to be completed for credit as part of a degree program. This is not a paid internship.) ESSENTIAL DUTIES & RESPONSIBILITIES * Attend New Staff Orientation prior to starting the internship. * Provides direct service to members, as indicated by the service plan, while supervised by an appropriate, designated staff person. Consistent with the intern's level of experience and training, this potentially includes but is not limited to individual counseling/therapy, case management, teaching, assessment, assertive community treatment, job support, managing medications, treatment planning, crisis intervention, and group counseling/therapy. This may be done in the community or on site at a Thresholds location, but the majority of services will be provided in the community unless otherwise specified by a supervisor. * Completion of all assigned paperwork in a timely fashion, such as individual and group documentation, attendance reports, service plans and evaluations as assigned by the supervisor. * Provide accurate and timely information to the supervisor on all topics and issues important to the rehabilitation and well-being of Thresholds members. * Attend staff and team meetings as assigned by a supervisor. * Maintain a professional demeanor with members, colleagues, and collateral contacts consistent with agency ethics and code of conduct guidelines. EDUCATION * Enrolled in Bachelor's or Master's degree program with a major in Social Work, Counseling, Psychology, Rehabilitation Counseling, Vocational Counseling, Family Therapy, or other related human service programs. * 1st or 2nd year Master's students preferred. EXPERIENCE * Social service experience and/or experience working with individuals with mental illness preferred. * Ability to synthesize and summarize information and make judgments regarding member care. * Must communicate effectively both verbally and in writing in order to provide information to members and healthcare personnel. * Demonstrated ability to function independently. * Computer skills required; knowledge of database functions and maintenance necessary. SKILLS/CERTIFICATIONS * Current and valid driver's license required.
