ESSENTIAL DUTIES & RESPONSIBILITIES

* Evaluates the organization's administrative support structure and plans for continual improvement of the efficiency and effectiveness of the group. Coaches department level leadership as they implement the strategic plan and ensure efficient operations.
* Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws, including interviewing, hiring and training employees; planning, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
* Performs other related duties as assigned.
EDUCATION
* Bachelor's degree from an accredited college or university required.
* Master's Degree in Business Administration or Nonprofit Administration (MBA or MNA) preferred.
EXPERIENCE
* A minimum ten (10) years of progressive experience with broad management, fiscal and operational responsibility, including significant not-for-profit experience.
* Demonstrated experience with program development and program evaluation, including creation of new programs, setting measurable goals and objectives, data collection and analysis, monitoring and evaluation, and adjustment of goals in response to benchmarks.
* Previous leadership and oversight of strategic planning process, quality improvement programs and process improvement/lean thinking.
* Previous management and leadership experience of a broad spectrum of professional administrative staff in the areas of IT, Facilities and Real Estate Asset Management preferred.

As part of building the medical summary report, you will gather supporting information and complete the applications using the SOAR method (SSI/SSDI Outreach, Access, and Recovery). Assistant Program Director, Crisis Response - South Side

Job Description
The Assistant Program Director (APD) will supervise a number of 24/7 Crisis Response Teams (including Mobile Crisis Response teams and a Crisis Stabilization Urgent Care Unit) that will provide community-based interventions to individuals experiencing a behavioral health crisis wherever they are; at home, work, in the community, or in a crisis center. This new program will be located on the south side of Chicago. The APD will help to establish the necessary collaborations and protocols necessary to make this program successful.
The APD reports to the Program Director and is a part of the management structure. The APD is responsible for the overall and day-to-day operation, administration, and evaluation of the Crisis Response program as well as the general welfare of the members and staff. The APD oversees all aspects of the Program in absence of the Program Director.
ESSENTIAL DUTIES & RESPONSIBILITIES
Ensures the delivery of all program and casework services align with agency objectives and promulgate agency traditions, practices and organizational goals.
Provides direct service to members and their families, functioning as the senior clinician or "player coach" of your team. This includes participation on the team in a manner consistent with team expectations and in a fashion which models the functioning expected of all team members.
Ensures the team functions optimally, in conjunction with other parts of the program, while meeting the community support and rehabilitation needs of our members, as well as commercial insurance standards. This is to be accomplished in a fashion which is consistent with the agency's mission statement, goals and objectives, formal and informal policies and procedures, as well as with the expectations and regulations of the various funding sources and with the highest professional standards. This may include but is not limited to the standards and documentation requirements as established by: CARF, Medicare, Medicaid, DMH, CILA, DOL, Social Security (PASS), DRS and/or DCFS.
Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing, hiring and training employees; planning, assigning and directing work; appraising performance; rewarding and disciplining employees; addressing complaints and resolving problems.
Monitors all required paperwork and maintains the quality and validity of individual case records according to CARF, Medicare, Medicaid standards and agency traditions. Assure that all assigned paperwork is completed accurately and in a timely fashion.
Oversees income streams and programmatic expenditures for this program in accordance with the agency's accounting procedures and within the program's approved budget as assigned by the Program Director.
Assures that all Thresholds procedures regarding the handling of member's funds are followed and that both the members and the Thresholds administration are assured that all safeguards, reports and accounting meets the highest standards.
Meets regularly with key staff of other Thresholds programs for the purpose of administration, coordination and development of agency programs. This shall include providing the Program Director with accurate and timely information on topics and issues related to the operation of the program and well‐being of its members. As assigned, appropriately transmit such information to the staff of the program.
Assists with special tasks as assigned, including but not limited to: writing proposals, publications, annual reports, representing the agency at meetings, conferences, conducting research, consulting with and training outside visitors and trainees, supervising students, orienting and training new staff.
Oversees and manages the physical location(s) utilized as program space. Assumes responsibility for all aspects of staff and member safety relative to the operation of the program(s).
Participates in and attends special activities related to the program as assigned.
Participates in the implementation of Thresholds emergency plans related to extreme weather or other emergency condition.
Performs and participates in crisis intervention when necessary.
Assures that all Thresholds records are organized in a systematic and organized fashion. Follows all appropriate safeguards with regard to the confidential files maintained at the Program. Follows all policies and procedures pertaining to all records including but not limited to restricting access to all records per agency policy.
Provides accurate and timely information to supervisor on all topics and issues related to the operation of the agency and the well‐being of its members. As appropriate, transmits such information to subordinates.
Provides crisis interventions while in the community.
Represents Thresholds when meeting with community partners and members of the community.
Other duties as assigned.
Be on call as needed for 24/7 programs, work a flexible schedule, including weekends or evenings, as needed.
Be available to the staff and members of the program on an on‐call basis after regular business hours. Provides after‐hours coverage as needed.
In the absence of the Program Director, provides leadership and modeling of evidence‐based practices and recovery that ensures program implementation according to the fidelity scales.
EDUCATION
Bachelor's degree in Social Work, Psychology, or a closely related field required
Master's degree in Social Work, Counseling, or a closely related field preferred
EXPERIENCE
Three (3) years of leadership experience is preferred.
Ability to synthesize and summarize information to make judgments regarding member care.
Computer proficiency, including the ability to use email, navigate and enter notes into electronic medical records.

Current and valid driver's license required.
Daily access to a personal vehicle is preferred, or a willingness to take public transportation to work sites.

Thresholds' Emerging Adults Program is seeking a licensed clinician to fill a Therapist role with our Emerge CST team. This program uses an evidence-informed coordinated care approach to treat young adults struggling with serious mental illness. These services are aimed at changing the trajectory of mental health needs for young people and is part of a national innovative movement in services. The multidisciplinary team provides specialized assessment, individual therapy, psychoeducation, medication management, vocational/educational support, community outreach, and case management services. The new therapist will receive training in our models.
The Therapist will be responsible for completing assessments and generating treatment plans, providing individual therapy, case management and community support services. In addition, they will assist with facilitating workshops, activities, and programs that will contribute to Young Adults' overall development. The Therapist works within a team-oriented, strengths-based approach, ensuring collaboration within the team and with other providers to facilitate integration of services.

ESSENTIAL DUTIES & RESPONSIBILITIES
Attend New Staff Orientation prior to starting the internship.
Provides direct service to members, as indicated by the service plan, while supervised by an appropriate, designated staff person. Consistent with the intern's level of experience and training, this potentially includes but is not limited to individual counseling/therapy, case management, teaching, assessment, assertive community treatment, job support, managing medications, treatment planning, crisis intervention, and group counseling/therapy. This may be done in the community or on site at a Thresholds location, but the majority of services will be provided in the community unless otherwise specified by a supervisor.
Completion of all assigned paperwork in a timely fashion, such as individual and group documentation, attendance reports, service plans and evaluations as assigned by the supervisor.

The Security Manager oversees physical security operations across 90 Community Mental Health locations throughout metropolitan Chicago. This role is responsible for safeguarding staff, clients, and property by implementing and monitoring security protocols, coordinating with external vendors, and ensuring incident response aligns with all applicable laws and regulations. Reporting to the Vice President of Facilities and Fleet Operations, the Security Manager collaborates closely with IT, Legal, and Real Estate teams to maintain a secure and compliant environment.

The Community Support Specialist - Residential (Adult Program) is responsible to facilitate mental health rehabilitation interventions and supports necessary to assist individuals with mental illnesses to achieve rehabilitative, resilience and recovery goals within a Thresholds operated residential program.
ESSENTIAL DUTIES & RESPONSIBILITIES
Provides direct service to members through individual and group counseling, case management, job support, crisis intervention, and psychoâsocial rehabilitation services.
Under direct supervision, gathers member information and assesses the need for service, makes recommendations for treatment, identifies and investigates existing natural resources and supports.
Identifies risk factors related to the relapse and development of relapse prevention plans and strategies.
May develop review and assist with members' Individual Treatment Plans (ITP). This requires face to face meetings with member or guardian to identify goals, objectives, specific mental health services needed, and identification of staff responsible for delivering the services.
Teaches members independent living skills, such as housekeeping, shopping, meal preparation and personal hygiene as well as the development of functional, interpersonal, support system and coping skills.
Provides support to members with collateral sources, including other agencies, Social Security Administration, medical providers, Chicago Housing Authority, etc. Advocates and assists with problem solving/resolution that will help the member access and utilize support from the community or support system.
Performs comprehensive member assessments, with the team and team leader, in order to identify the member's mental health service needs and to make recommendations for treatment.
Supports and promotes member selfâadvocacy and participation in decision making, treatment and treatment planning.
Ensures that the Thresholds Service Plan addresses the member's needs and includes objectives and techniques for accomplishing each goal.
Works cooperatively with inpatient hospital staff, keeping logs and other records of all contacts as required by agency practice. Performs and participates in crisis intervention when necessary.
Monitors Self Administration of Medication. This includes all procedures associated with the documentation of medication monitoring of selfâadministration in keeping with Thresholds standards, as well as procedures to follow in the event that there is any disruption to the medication schedule.
May be required to communicate significant information regarding members to other staff or leadership. Provides accurate and timely information to your supervisor on all topics and issues related to the operation of the agency and the wellâbeing of members.
Assures that all Thresholds procedures regarding the handling of member's funds are followed and that both the members and the Thresholds administration are assured that all safeguards, reports and accounting meets the highest standards.
Keeps the residence neat and clean at all times. Advises Supervisor, Team Leader or designee of any maintenance problems or issues. Responsible for the creation of a safe, clean, interesting and inviting atmosphere for members.
Attends all required meetings and completes monthly clinical supervision trainings in timely fashion.
Meets required billing targets and completes all documentation within assigned time frames.
Uses personal vehicle for work. Obtains and maintains agency-required liability insurance coverage of $100,000/$300,000/$100,000 subject to validation every 6 months.
May be required to maintain the health and safety log and conduct safety checks when a member has not been seen in a 24 hour period.
May be required to purchases the weekly groceries for the house and assists in meal preparation.

EDUCATION:
Must meet Office of Mental Health Rule 132 credential requirements for a Rehabilitative Services Associate which requires a minimum age of 21. Hold a Master's degree in social work, counseling, psychology, or a closely related field, and be able to, demonstrate skills in the field of services to adults.

The Center for Mental Wellbeing South, Thresholds' Certified Community Behavioral Health Clinic (CCBHC) is a one-stop shop for mental health and substance use services based in the South Side of Chicago for families and individuals of all ages. The Center is committed to providing care regardless of ability to pay, and cares for those who are underserved, low income, are insured, uninsured, or on Medicaid. We offer a sliding scale program that provides discounted services to individuals based on family size and income. As the Therapist, you will provide individual, family, and group therapy in the clinic setting to children, adolescents, and families with a profound need.
Every week, you will work with up to 30 clients for outpatient therapy. Meet face-to-face with the client or guardian to assess needs, identify goals, and develop individual treatment plans. Often this will look like individual therapy sessions but you will also provide family and group therapy and facilitate therapeutic workshops. You will also do case consultation and crisis intervention.

ESSENTIAL DUTIES & RESPONSIBILITIES
Oversees the operations of select administrative departments and program review.
Formulates long-range objectives, plans, and programs for departments to ensure consistency across the organization.
Provides the leadership, management and vision necessary to ensure the organization has the proper operational controls, administrative procedures and systems in place to effectively grow and ensure financial strength and operational efficiency.
Works closely with Clinical Operations, Finance and other departments to establish new strategic partnerships that drive service innovation and organizational growth.

Maintains proactive communication with executive management with regard to financial management and industry issues that require contingency planning.

EDUCATION
Bachelor's degree from an accredited college or university required.
Master's Degree in Business Administration or Nonprofit Administration (MBA or MNA) preferred.
EXPERIENCE
A minimum ten (10) years of progressive experience with broad management, fiscal and operational responsibility, including significant not-for-profit experience.
Demonstrated experience with program development and program evaluation, including creation of new programs, setting measurable goals and objectives, data collection and analysis, monitoring and evaluation, and adjustment of goals in response to benchmarks.
Previous leadership and oversight of strategic planning process, quality improvement programs and process improvement/lean thinking.
Previous management and leadership experience of a broad spectrum of professional administrative staff in the areas of IT, Facilities and Real Estate Asset Management preferred.

In this role, you will be part of a team that works with Williams class members in their transition from specialized mental health rehabilitation facilities to living independently in the community.
Use your expert knowledge to construct each client's case for appropriate SSI or SSDI benefits. As part of building the medical summary report, you will gather supporting information and complete the applications using the SOAR method (SSI/SSDI Outreach, Access, and Recovery). You will also support your clients in getting access to other resources, including housing, community services, health care, substance use treatment, and mental health care. This includes coordinating case management and attending appointments with your client to provide support and advocacy as needed.
Ultimately, you will support the client as they transition to living independently and receiving ongoing services from a Thresholds Community Support Team. If they have not yet received SSI/SSDI at the point of transition, you will continue to work with them until they receive their benefits.
