The Clinical Supervisor is responsible for managing the provision of all therapeutic services provided to patients and oversees all aspects of coordination of care for these clients. HIRING REQUIREMENTS
A Bachelor's degree in health care administration or a related field.

ESSENTIAL RESPONSIBILITIES
Supervise day-to-day office/clinical operations. Supervise professional team members in client homes through written and telephone communication, and including onsite supervisory visits.
Perform initial assessments and ongoing reassessments to identify all client needs and develop a comprehensive case management plan to address all client needs, with written actions, objectives, and goals including timeframes. Assist client/family to obtain appropriate level and type of health, medical, financial, legal, and psychosocial care as needed.
Implement the actions per the plan, and coordinate and monitor all services the client requires through written and oral communication in person or by telephone to determine effectiveness ongoing and make modifications as needed. This includes appropriate interactions with all relevant components of the client's health care system such as physicians, family members, third-party payors, other health providers/professionals, and appropriate government/regulatory agencies.
Make appropriate referrals to therapists, social workers, and other health care professionals or programs/assistive technology to meet client/family's needs to include educational, rehabilitative, job development, and life planning issues.
Evaluate each clients plan at least every 60 days to determine effectiveness and appropriateness of current plan, modify plan as appropriate in agreement with client's family and physician.
Actively participant in the hiring process for clinical team members. Orientation and in-servicing of field and office staff to meet client needs.
Prepare 90 Day evaluations, annual evaluations, and conduct coaching and/or counseling sessions.
Conduct regular staff meetings with team members to review recertifications, patient/family client concerns, upcoming discharges, policy and procedures, and other related matters.
Review time sheets, mileage reports, time off requests, and approve payroll transmittal.
Review schedule prepared by the Scheduling Coordinator, approve monthly schedules and changes to schedule to ensure nurse's/therapists skill meets client's requirements.
Provide Educational material for family and staff on medical diagnoses, provision of care, and psychosocial aspects of chronic illness and disability.
Attend and participate in community education functions.
Function in capacity of staff nurse when necessary.
Ensure compliance regarding inservice records and development.
Take administrative on call as support to the on-call nurse.
Conduct Continuous Quality Assessment/Performance Improvement activities as assigned.
Conduct Skilled Nursing Visits as needed.
Participate on assigned committees.

Performs daily responsibilities to meet the needs of potential donors, donors, donor families, potential donor families, hospital personnel, physicians, and the OPO. In partnership with the In-House Donation Specialist (when applicable), develops, provides and evaluates in-house training and in-service educational programs on all aspects of the donation process for hospital staff. Key responsibilities this position will perform include:
Provides extensive on call services to obtain authorization for organ and tissue donation. On-call responsibilities may be affected by increased donor activity, staffing shortages, etc. Facilitates the authorization process for potential organ and tissue donor families prior to, during and after death declaration.
Evaluates potential donors. Assesses potential donor families, obtains appropriate authorization for donation, conducts Medical/Social History interview, and assists donor families through identification of potential end of life decisions, attending family meetings and providing donation information as needed. Responsible for medical management of donors prior to recovery activities. Directs the placement of anatomical gifts as necessary. Provides transplant surgeons with information necessary to determine appropriate recipients. Coordinates and assists in the surgical recovery of organs and perioperative management of the donor when necessary.
Provides support to Hospital Development Coordinator, in the assigned facility, to identify formal and informal leaders, assesses their respective roles, degree of influence and needs. Works collaboratively with these leaders and utilizes their expertise to improve and promote donation. Assists in policy and procedure development. Functions as an expert clinical resource for the hospital regarding organ and tissue donation.
Will visually assess donors, interpret charts, document information and communicate findings. Collaborates with hospital and medical staff to provide potential donor families with accurate and timely information regarding the patient's current clinical course. Maintains communication with hospital staff and attending physician regarding the potential donor family's understanding of the prognosis and acts as a family advocate to the health care team as necessary. In the event of neurological deterioration and/or cardiac cessation, provides education to the potential donor family to include signs and symptoms of brain death, the process of diagnosing brain death, or cardiac cessation and withdrawal of support. As appropriate, discusses organ Family Care Coordinator and tissue donation opportunities with the potential donor family with the goal being to obtain authorization for donation.
In the event the potential donor stabilizes and is no longer considered a potential organ donor, establishes an appropriate support system in collaboration with the health care staff, brings closure to the relationship with the family and returns if needed or requested.
Provides appropriate information for the bereavement program to all potential organ and tissue donor families who wish to participate. As appropriate, provides a follow-up letter to donor families. Facilitates donor family and recipient communication in accordance with company policy and procedure.
Performs other duties as assigned.

QUALIFICATIONS:

About the Role
We are seeking an experienced trach and ventilator nurse to help care for a high acuity patient in the Arcola, TX area. Patient is a one year old male, needing around the clock care, preferably a rotating schedule with another nurse.

A Day in the Life of a Care Options for Kids Nurse
You start your day by touching base with your care team, whether that's mom, dad, or the nurse you are relieving. For added ease, in-depth information on your client is available in our dedicated Point of Care platform.
Your client greets you warmly. Though she's non-verbal, she'll smile and reach out to touch your face and put her head to your chest for a cuddle she's excited to see you. After disconnecting her ventilator, which she only needs at night, it's time for breakfast and a light sponge bath.
From there, every day is a new adventure. You may decide to head outdoors to gaze at clouds, hunt for ladybugs, or make flower crowns. You may set up a living room campout or travel back in time with a book until it's time for lunch.
Throughout the day, you prepare and deliver her medications and perform her routine trach and g-tube care documenting the dosages and treatments within your dedicated Point of Care platform, where you're able to view and compare notes and assessments easily and electronically. While your client takes a late afternoon nap, you finalize your electronic documentation and touch base with the relieving member of your care team before heading home.

Responsibilities of Licensed Practical Nurses (LPNs) / Registered Nurses (RNs)
One-to-one nursing care in a home environment
Medication administration per physician orders
Physician ordered treatments for:
Nutrition via a feeding tube
Tracheostomy care
Suctioning nasal, oral and/or endotracheal
Ventilation care
Seizure assessment and treatment
Requirements for Licensed Practical Nurses (LPNs) / Registered Nurses (RNs)
Current, active TexasRN or LPN license
Current BLS CPR card (obtained in-person, not online)
G-tube, trach, vent experience, or willing to train
TB test results or a chest x-ray
