About the Position:
In our residential programs, the Substitute Worker has the responsibility of working closely with residents who are progressing toward independent living. The Substitute Worker provides the daily support needed for each resident to function at the highest possible level. These supports will include emotional support, demonstrating and teaching living skills, coordinating program activities, and facilitating groups, and client activities.
At our Crisis Respite Center (CRC), the Substitute Worker will assist individuals in managing their crisis and will offer information, advocacy, and referrals for continued assistance and support after they leave the CRC. The (CRC) is a program designed to provide any Sacramento / Sacramento County resident (18 years and older) who is experiencing a mental health hardship/crisis a place to go to receive support and gain hope for a resolution. The CRC is a person-centered program, providing a welcoming and non-judgmental environment for individuals who need relief from the stresses of life, and who need assistance in stabilizing their mental health crisis. The individuals who come to the CRC may be experiencing a wide variety of mental health issues.
Please note, this position is on an as-needed basis, there is no guaranteed hours and/or shifts.
Essential Duties & Responsibilities:
Support individuals with active listening, demonstration of empathy and carry a message of hope and recovery
Observe, assess, intervene and refer (within one's scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues
Develop service/goal plans with clients and meet with the clients as needed to encourage, support and advocate for progress and assist with skill development
Provide positive public relations, community referrals, information and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified
Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, Medi-Cal billing, Avatar forms, HMIS)
Shop for food and household supplies to meet program or client needs
Ensure the office, house, rooms, units and yard are clean and fit for occupancy, where applicable. Provide and assist with cleaning as needed
Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults and elder/senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm
Ensure the protection and confidentiality of all Protected Health Information
Qualifications:
Ability to work with individuals who have a mental health diagnosis. Experience is highly desirable
Lived experience as a mental health consumer or a family member/loved one of a mental health consumer is desirable
Knowledge of Sacramento community organizations and community resources is highly desirable
Mental health crisis intervention experience is highly desirable
Experience with persons who have substance abuse problems, preferably individuals who are dually diagnosed (mental health & substance abuse) is highly desirable
Knowledge of 12-Step Programs and/or personal experience with substance abuse recovery is highly desirable
Other Skills, Abilities, and Job Requirements:
Ability to pass clearance of the Federal Bureau of Investigation (FBI) and/or Department of Justice (DOJ) background screenings

Services will be offered from a Trauma-informed perspective and will utilize Motivational Interviewing and other Evidence-Based Practices as appropriate.
Essential Duties & Responsibilities:
Support individuals with active listening, demonstration of empathy, and carry a message of hope and recovery.
Observe, assess, intervene, and refer (within one's scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues, and homelessness.
Develop service/goal plans with clients and meet with the clients as needed to encourage, support and advocate for progress and assist with skill development.
Provide positive public relations, community referrals, information, and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified.

About the Position:
The Personal Services Coordinator works as part of multi-disciplinary team to provide and coordinate services and resources for clients in the community. The goal of the Personal Services Coordinator is to provide referrals, crisis intervention, support and encouragement to enable the client to access and use mental health and other services that prevent hospitalization and incarceration, and help the client find and maintain permanent housing and mental stability. This position assists clients with financial matters and entitlement issues in collaboration with Benefits/Employment/Housing specialist, and gives the clients the care and attention needed to enable them to learn better coping skills, independent living skills, and techniques to empower clients with recovery, a sense of responsibility, and independence.
The Personal Services Coordinator is the point of contact for client care and is responsible for ongoing maintenance of chart and services. This role includes coordinating with additional service team member.
Essential Duties & Responsibilities:
Support individuals with active listening, demonstration of empathy and carry a message of hope and recovery.
Observe, assess, intervene and refer (within one's scope of practice) in situations with clients regarding mental health symptoms, physical health symptoms and substance use issues.
Develop service/goal plans with clients and meet with the clients as needed to encourage, support and advocate for progress and assist with skill development.
Work with a multi-disciplinary team to provide integrated psychosocial rehabilitative services to clients in the community and their homes, and attend meetings to ensure continuity of care for clients.
Provide positive public relations, community referrals, information and support by linking clients to needed resources; advocate with and on behalf of clients and significant support persons the clients have identified.
Accurately complete all necessary forms, reports and paperwork required by the program on a timely basis (e.g., progress notes, special incident reports, discharge summaries, Medi-Cal billing, EHR forms, HMIS).
Facilitate planning of social, cultural, educational, supportive and recreational group activities.
Shop for food and household supplies to meet program or client needs.
Ensure the office, house, rooms, units and yard are clean and fit for occupancy, where applicable. Provide and assist with cleaning as needed.
Act as a designated Mandated Reporter for the observed or suspected abuse and/or neglect of children, disabled individuals, dependent adults and elder/senior citizens. Mandated Reporter also includes “Duty to Warn” which includes the responsibility to notify authorities and the intended victim(s) of physical harm.
Transport clients.
Ensure the protection and confidentiality of all Protected Health Information (PHI) by following all policies and procedures in the Agency HIPAA Manual.

Qualifications:
Ability to work with individuals who have a mental health diagnosis. Experience is highly desirable

Preferred Qualifications:
Meet the qualifications of a Mental Health Assistant III (MHA III):
A minimum of four (4) years of full-time equivalent (FTE) direct care experience in the mental health field providing mental health services; OR
Two (2) years of FTE direct care experience in a mental health-related field providing mental health services and two years of education (60 semesters or 90 quarter units) with a minimum of 12 semesters (18 quarter units) in a mental health-related subject such as child development, social work, human behavior, rehabilitation, psychology or alcohol and drug counseling; OR
Two (2) years of FTE direct care experience in a mental health-related field providing direct mental health services and a certificate of completion from the County Core Skills Training (MHRS highly desired)

About the Position:
The Director of Real Estate Development plays a pivotal role in advancing the agency's mission by expanding safe, stable, and affordable housing opportunities for individuals experiencing homelessness. This role manages projects from concept through completion, including acquisition, financing, construction, and stabilization. The Director oversees development staff and consultants, supervises the Asset Manager, and ensures the long-term viability of the organization's real estate portfolio.
The role requires a balance of technical knowledge in affordable housing development and a deep commitment to social impact, with the ability to champion housing as a cornerstone of recovery, stability, and community well-being. The Director will also represent the agency in external collaborations, advocating for housing solutions with public agencies, policymakers, and community partners, while building internal organizational capacity to sustain and grow housing development efforts into the future. This position collaborates across departments to align development projects with the organization's mission, operations, and service delivery.

Strategic Planning
Design and lead the organization's long-term housing development strategy.
Identify and assess new development and acquisition opportunities aligned with the agency's mission.
Work with executive leadership and the Board of Directors to establish goals and metrics for real estate growth and impact.
Recruit, supervise, and mentor staff involved in real estate and asset management.
Project Management
Oversee all phases of the development process, including feasibility analysis, site acquisition, entitlements, predevelopment, financing, construction, and lease-up.
Manage multiple active projects simultaneously, ensuring timely delivery and budget compliance.
Supervise and support internal and external project teams including architects, engineers, attorneys, lenders, and contractors.
Financing
Lead efforts to secure funding from diverse sources including Low-Income Housing Tax Credits (LIHTC), No Place Like Home (NPLH), CalHFA, HUD, HOME, CDBG, Homekey, BHSA, philanthropic or private equity.
Prepare and review financial models, pro formas, and underwriting materials for complex capital stacks.
Negotiate and coordinate financial closings with funding partners.
Stakeholder Representation
Serve as the organization's lead housing development representative to community stakeholders, city/county officials, and public funding agencies.
Present projects at planning commission hearings, community meetings, and public forums.
Collaborate with internal departments, including property management, finance, and behavioral health services.
Asset Management

Oversee capital planning, compliance, reserve analysis, and sustainability strategies.

Partner with internal program, clinical, and outreach leaders to integrate housing development with supportive services. Provide supervisory guidance when staff are assigned part-time to housing-related initiatives.
Provide coaching, mentoring, and career development for staff within the Housing & Real Estate division, helping to build organizational capacity for future housing expansion.

Excellent leadership, negotiation, and relationship-building skills.
Previous experience working with individuals with psychiatric and/or other disabilities, homelessness and substance abuse is highly desirable.

About the Position:
The Peer/Family Advocate will serve as a member of a multi-disciplinary team to provide peer and/or family support in collaboration with the Service Team.
