As a Supports Planner you help identify, access, and coordinate services and supports to maintain their health, safety, and independence. You will also help with waiver eligibility determination and maintenance. Requirements Minimum of 21 years of age MUST have Masters or Bachelors degree in Human services field. Must be a licensed driver and have own transportation with valid insurance. Must be a U.S. citizen or alien who is lawfully authorized to work in the United States Must have computer skills and know how to perform tasks utilizing Microsoft Word, and MS Excel.
